Conditions of registration: 1. The Home will limit its services to the following service user categories: No person falling within the category Older Persons (OP) can be admitted where there are already 8 persons of category OP already in the Home. The total number of service users in the Home must not exceed 8. 2. Date of last inspection 2nd February 2006 Brief Description of the Service: Rose Cottage is registered to provide personal care and support to up to 8 older people over the age of 65 who require such personal care and support due to their old age. Community health care professionals provide all health care needs. The home is located in a quiet residential area in the village of Woodford. It is an extended detached property offering ground floor accommodation in single bedrooms all with en-suite facilities. The home has two lounges and a dining area adjacent to the kitchen, and there is a well-maintained and attractive garden with pleasant patio area that is fully accessible to Service Users. The registered owner and manager live on site, on the first floor of the registered home, and the adjoining adjacent property. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for Service Users and their views of the service provided. This inspection was a ‘Key Inspection that focused on the key standards under the National Minimum Standards and the Care Standards Act 2000 for homes providing care for older people. The primary method of inspection used on this unannounced inspection was ‘case tracking’ that involved selecting three residents and tracking the care they receive through review of their individual care plans (that sets out how the home aim to meet the residents personal, social, spiritual and healthcare needs). The homes policies and procedures and records in relation to staff recruitment, complaints, medication and general maintenance and upkeep of the home were viewed, and discussion took place with residents, staff on duty and the registered manager and general observations of care practices were made. The registered manager Claire Williams was available at the home throughout the inspection. The Commission for Social Care Inspection sent out to the home a preinspection questionnaire and service users and visitors/relatives comment cards, prior to the inspection The pre-inspection questionnaires was returned to the Commission for Social Care Inspection along with several residents and visitors feedback cards which provided information on the management systems and outlined the general satisfaction of the residents living at the home and their representatives. The inspector spent two and a half hours planning the areas to focus on at this inspection, based upon information gained from reviewing the homes service history, the last two inspection reports and information from the pre inspection data collection systems. The registered manager should seek guidance from the environmental health authority to make suitable arrangements for the safe disposal of continence waste products. The Laundry floor finish should be of an impermeable material and readily cleanable.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3&6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Prospective residents can be assured that the home can meet their needs and expectations. EVIDENCE: Prospective residents are invited to view the home and meet the residents and staff, copies of the homes statement of purpose and service users guides are provided, to enable them to make an informed choice about moving into the home. The resident’s care plans viewed included pre assessment documentation that had identified the prospective residents needs prior to them moving into the home and the care plans were signed and dated by the resident, and regularly reviewed.

Rose Cottage is a small care home, that provides a home for older people with very low dependency needs and residents that display anti social behaviour towards other residents and staff are not eligible to move into the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents can be assured that home can meet their health and personal care needs. EVIDENCE: Residents care plans were detailed and staff were knowledgeable of each residents individual needs. Residents spoke very highly of the care they received. Within the care plans there was records of residents being referred to their general practitioner as and when required. Domiciliary services visit the home to provide eye care, hearing, chiropody and dental services. The General Practitioners visit the home and residents also visit the local surgery from time to time. Residents are supported in attending healthcare appointments.
Rose Cottage DS0000012900.V303732.R01.S.doc Version 5.2 Page 12 Records of residents weights were retained within the care plans, however one care plan indicated that the resident had lost a significant amount of weight since moving into the home, on speaking with the staff and it was explained that the resident had chosen to lose some weight, this was not outlined within the care plan. All of the staff are trained in the administration of medication, and certificates were available to view, the dispensing pharmacy conducts regular inspections of the homes medication storage and administration practices. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 & 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home in general matches the social, cultural, recreational and occupational expectations of residents. EVIDENCE: The home endeavours to ensure that all residents have the opportunity to access local community groups, and pursue appropriate individual interests and hobbies, some residents attend a luncheon club during the week, and a neighbour pays a social visit to the home on a daily basis, residents said that they look forward to their visit. One resident said that they felt very safe living at the home, that it moving to Rose Cottage was the best decision that they had ever made. All the residents said that they enjoy each other’s company, that it was nice to live in a small home and that the opportunity to spend time alone is always available if they wished.
Rose Cottage DS0000012900.V303732.R01.S.doc Version 5.2 Page 14 The home employs an activity person 2-3 afternoons per week, which enables residents to take part in quiz and board games, such as who wants to be a millionaire, crosswords and other general knowledge quizzes. Staff said that they were busy preparing for the homes annual garden fete; on the residents notice board included a visit to Althorpe and forthcoming musical entertainment. The residents spoke highly of the outside entertainers, singers and musicians that visit the home and of the efforts made by staff to provide activities for them. Members from the local chapel visit the home on a monthly basis to sing hymns and conduct a small service of worship with the residents, residents said that they look forward to the visits but had no desire to attend the chapel services outside of the home. One resident said that their local vicar visits to perform an individual holy communion with them. The home conducts satisfaction surveys to seek the views of residents and relatives, to identify areas for improvements, feedback from a survey conducted with visitors during May 2006 were available to view, all the comments were very positive, such as ‘staff are excellent’, ‘staff are warm and friendly and helpful’, always greeted with a smile and a coffee’. The registered manager said that she plans to conduct a residents satisfaction survey during the coming months, on speaking with the residents they all said that they have regular meetings with the manager and see her on a daily basis and that if they had any dissatisfaction with the service provided they would not hesitate to raise them immediately. There are two small dogs Ruby and Sammy that live within the owners accommodation on the first floor of the home and are regular visitors to the home, the residents said that they enjoying the company of the animals and said that they enjoy seeing animals around the home. The dining area was pleasantly furnished having two circular tables that each seat four people, the meal on the evening of the inspection visit was Beef sandwiches and cheese and biscuits, one residents who was a vegetarian had an egg salad. Residents who are vegetarians said that they were pleased with the efforts made by the staff to provide them with suitable and sufficient meals. The kitchen was viewed and was clean and tidy, food safety standard records were available to view. The freezer contained a variety of foods and there was a selection of vegetarian foods available. On speaking with the residents about the foods

The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents can be assured that any complaints they may have will be taken seriously, and that they will be protected from abuse. EVIDENCE: The complaints procedure is available within the statement of purpose and there is also a copy available on the wall within the lobby of the home. There is a complaints book available, which outlines any concerns or complaints made about the home and outlines the action that the registered manager has taken to address them, however no complaints had been made at the home since the last inspection taking place and no complaints have been received by the Commission for Social care Inspection about the home. Staff training includes abuse awareness and the protocol for reporting any suspected or actual abuse. Should there be any incidents of an abusive nature, the home has a copy of the Northamptonshire Protection of Vulnerable Adult reporting procedures for guidance. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents live in a home that is safe, comfortable and clean and meets their needs and expectations. EVIDENCE: A limited tour of the building was conducted and the furnishings and decoration within the home was to a good standard. Resident’s bedrooms viewed contained personalised items of furniture and belongings and were clean and tidy. The bathrooms and toilets were all clean and hygienic, and contained hand sanitiser. Rose Cottage DS0000012900.V303732.R01.S.doc Version 5.2 Page 18 A minor fault with the bath chair had been reported by a staff member, the registered manager said that a replacement chair had been ordered from a local healthcare products supplier and was awaiting its the delivery. En-suites to bedrooms were personalised and homely, it was noted that within the en-suits that there was continence products available for residents use. When asked what arrangements were in place for the disposal of continence products, the registered manager said that there was no separate collection arrangement in place, the importance of having continence waste arrangements in place separate from household waste was stressed to the registered manager The laundry area was viewed, it was noted that the floor within the laundry was not of a washable surface; this was discussed with the registered manager as an area for future environmental improvement. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 & 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Resident’s needs are met by a staff team who are trained and competent to do their jobs. EVIDENCE: The dependency level of residents living at the home is very low; residents are encouraged to remain as independent as possible with minimum staff support The staffing levels are sufficient to meet the current needs of the residents living within the home. Staff recruitment records viewed indicated that the recruitment process is robust; there was evidence of criminal records bureau clearances and references being obtained prior to employment commencing. There is a strong commitment to staff training; all staff have achieved the National vocational Qualification 2 in care (NVQ2). And have all received statutory training, such as moving and handling, fire Safety, food hygiene, risk assessment, medication and protection of vulnerable adults training. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. The registered manager provides a clear vision for the home, and staff demonstrated an awareness of their roles and responsibilities. EVIDENCE: Rose Cottage is managed in accordance with its Statement of Purpose the registered manager is committed to the continuous development of the quality of service the home provides for its residents. The registered manager is experienced and holds the National Vocational Qualification level 4 (NVQ4) in Management and also the Registered Managers Award.

Regular resident and staff meetings take place, individual staff supervision takes place regularly and annual staff appraisals are carried out. The home has a small staff team and has a low staff turnover that ensures that residents have stability of care. Staff spoken with said that they liked working within a small team and that they enjoyed coming to work helping the residents, saying that there was plenty of training opportunities available and that they felt supported by the registered manager. The financial and accounting procedures within the home safeguard the resident’s interests, residents are encouraged not to bring expensive jewellery into the home, cash and valuables held on their behalf are stored securely. Records of accidents are maintained and individual risk assessments are in place, to ensure the health, safety and welfare of residents, and all records are stored securely.
