This inspection was carried out on 28th June 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

Service users benefit from having a stable staff team some of whom have known them for several years. There is the opportunity for a varied and busy day care programme and they are encouraged to participate in all aspects of life in the home. Individual life plans are drawn up with service users to identify personal goals that staff can support them in achieving.

What has improved since the last inspection?

Since the last inspection the medication cabinet has been moved to a more suitable location and staff have received training and been assessed on the safe handling of medication. All staff have now completed their induction programme and other core training is scheduled to take place. The Adult abuse policy has been amended as advised in the last report. Service users benefit from the recent purchase of new lounge furniture.

What the care home could do better:

A review of the systems in place to monitor progress in achieving personal goals of service users would be more useful in assessing outcomes. A review of the facilities in place for the security and confidentiality of service user information is urgently required. This issue has been raised in previous reports by the CSCI, by the providers Quality assurance manager and Home manager.

The premises consists of a spacious cottage situated in the rural village of Mersham. The local pubs and village shop are a short walk from the home. There is a public bus service to the larger towns of Ashford, Hythe and Folkestone. The home has a large secluded garden and access to country walks. The scale of charges as at 27/06/06 range between £1202.05 and £1436.95 per week, there are additional charges for Horse riding, rock climbing, trampoline sessions, ten pin bowling and hairdressing. This inspection was unannounced and was of approximately four hours duration. The completed pre-inspection questionnaire was received by the CSCI prior to the inspection date. Also returned were four surveys from service users. A tour of the premises was conducted and a variety of records and documents were examined. The inspector spoke with two service users and three members of staff and observed the interactions and practices within the home. This judgement has been made using available evidence including a visit to this service. Service users have the information they need to make an informed choice on whether the home is able to meet their needs. EVIDENCE: Care plans viewed showed that a full assessment of individual aspirations and needs had been carried out. A service user guide is available in pictorial format to enable understanding of those individuals with communication difficulties. Each service user has an individual contract detailing terms and conditions of the home. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9, 10 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Service users personal goals are not adequately monitored for progress. Confidentiality of written and verbal information is compromised. EVIDENCE: Support plans viewed contain a lot of information relating to individual routines, likes and dislikes, and general risk assessments for activities. Many of the personal goals recorded are repetitive and agreed some time ago. A discussion with the person in charge followed and it was agreed that individual goals need updating and monitoring against progress in achieving. One service user was observed doing cooking with support from a member of staff. Service users spoken with appeared happy with their lifestyles. The office area is open plan and people are constantly walking through to other parts of the home. Care plans and financial information are displayed on open shelves and therefore not secure from unauthorised individuals, likewise telephone conversations can be overheard by anyone passing through the area. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13,14, 15, 16, 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users have the opportunity to participate in a range of social, educational and leisure activities and enjoy meals that are varied and of their choice. EVIDENCE: Activity records examined showed that the majority of day-care is based around the Martello day centre where a range of activities are provided. At the time of the visit 2 service users were out and one was engaged in a cooking session supported by a member of staff. Service users have the opportunity to attend social events at the Martello, Special Olympics, and local public house three evenings per week. The person in charge states that additional staff are available if required to facilitate activities at weekends. Service users have recently had a holiday at a Butlins holiday camp and they spoke well of the experience. Menus viewed looked varied and balanced and service users spoken with said they enjoyed their food. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users physical and emotional needs are met and they are protected by the homes policies and procedures for dealing with medication. EVIDENCE: Support plans viewed contain a lot of information relating to individual routines, likes and dislikes, and general risk assessments for activities. As required by a previous inspection the medication cabinet has been moved to a more suitable location. The staff team has received training on the safe handling of medication evidenced on the staff training matrix. The homes manager states that he assesses each member of staff for competence on medication issues and this is recorded in their files. Service users know their concerns will be listened to and acted on, and they are protected from abuse by the homes policies and procedures. EVIDENCE: The homes Adult abuse policy has been amended as advised in the last inspection. All staff have now attended training on adult protection issues. Members of staff spoken with were clear in their understanding of reporting procedures when allegations arise. Service users spoken to stated that they would approach staff members with any concerns they might have. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. This judgement has been made using available evidence including a visit to this service. Service users benefit from living in a homely, safe environment with adequate personal and communal space for their needs. EVIDENCE: The home has recently purchased new lounge furniture as required by the previous inspection. A tour of the building indicated a clean and safe environment with no obvious health and safety hazards. Individual bedrooms reflected the interests and lifestyles of service users and where furnishings are sparse documented risk assessments explain the reasons. Service users’ benefit from the use of very large and well-maintained gardens. Service users benefit from having a stable staff team in the home who are clear about their roles and responsibilities and are well supported and supervised. EVIDENCE: All staff have completed their induction package to ensure clarity of roles and responsibilities. Training matrix viewed shows relevant staff training is ongoing and core training requirements are scheduled to take place where required. Two members of staff have NVQ care awards and two others are currently undergoing training. Robust recruitment practices are in place for the protection of service users evidenced by individual CRB, POVA and reference checks on employees. A rota for staff supervisions is displayed and the senior states that seniors supervise support workers and the manager supervises the seniors. This judgement has been made using available evidence including a visit to this service. Service users and staff benefit from the leadership and management approach of the home and their views are sought in the running of the home. EVIDENCE: Staff members spoken with demonstrated a commitment to providing a good service and the homes manager and senior were observed to provide leadership and an open approach to management. Service users are encouraged to express their views on a daily basis and have regular meetings with their key worker. The company quality assurance manager arrived during the visit to conduct a Reg.26 visit and was observed to speak with service users. Surveys received back from service users were generally positive and responses from those spoken with support this view. A tour of the premises did not identify any obvious health and safety hazards and various charts monitoring health hazards were examined.
