Conditions of registration: 1. PD Minimum age 60 yrs Date of last inspection 18th September 2007 Brief Description of the Service: Rosevilla is situated on the Eccleshall road leading to the M6 (junction 14), in a residential area about one mile from Stafford town centre. Rosevilla is a registered care home offering 24 hour nursing care, which aim to offer a high standard of care and accommodation for thirty-five service users. The home has mostly single bedrooms, all with en-suite facilities. There are 3 double rooms for those who prefer to share, but these may be used as larger singles. The home started life as two large properties that have been extensively extended to provide the facilities it offers. Rosevilla consists of two floors, serviced by a lift in the original building, and a purpose built ground floor unit of single rooms with ensuite facilities. The service users have a choice of two lounges to choose from, one with views to the back of the property and one with views to the front. The opportunity to become involved with activities is also available with the homes activity organiser (Post is currently advertised) Home cooked food is provided and special dietary needs are catered for. The service users have access to all areas of the home including the gardens. There is ample parking space for visitors. This is an overview of what the inspector found during the inspection. The Quality rating for this service is 2 star. This means that the people who use this service experience good quality outcomes.
This unannounced inspection took place on 5th February 2008 by the lead inspector. The Manager was in charge of the home supported by her deputy manager, a trained nurse and 5 care assistants. The inspection included the following elements; • • • • • A tour of the building took place, including bedrooms, bathrooms, the kitchen, and all communal areas. Observation and inspection of records, which related to the provision of care, including care plans, risk assessments and medication. Discussions with several service users and relatives/visitors. Discussions with several of the staff members on duty, Observation and sampling of other services provided such as catering and laundry, and an inspection of the managerial aspects such as staffing issues, training, recruitment and health & safety. No complaints had been received in the home or by the Commission For Social Care Inspection. The Inspector was again made welcome in the home and assistance was given to gain the evidence required for the report. The home provider Mr Patel was in the home during the inspection and a discussion took place with regard to the improvements made in the home since the last inspection and the future plans for the establishment. What the service does well:
The staff at the home continues to assist the Service Users to live in a homely environment with some support to remain as independent as possible. The carers were motivated to provide a good service to the Service Users and were witnessed having conversations with service users and relatives in a caring and compassionate way. The home continues to have good working relationships with local doctors and nurse specialists who support the care offered in the home. Service Users spoken to at this visit told the inspector that they enjoyed the meals and could choose an alternative if they preferred. One family spoken to expressed their sincere appreciation for all the care their mother received in the home, and the warmth and generosity of the staff. They told the inspector that they are always offered a hot drink and recently visiting at lunch time they were offered sandwiches. What has improved since the last inspection?
Many improvements have been evidenced at this inspection. • • • • Care records are now well organised, personalised and updated. Accident records were available Staff files were in place (some to be completed) Environmentally all requirements were met, furniture and carpets (identified at the last key Inspection) had been replaced, and soft furnishings (curtains/ bedding) were due to be delivered in the next few days. The home was much cleaner and tidy in all areas. The kitchen was found in a hygienic state with the stores and cupboards in good order. The issues relating to the Statement of Purpose had been addressed and the Service User Guide was almost complete (just being finalised). The bathrooms were warm and inviting, blinds had been fitted, and ‘fish plaques’ had been put on the walls and the heating issue addressed. Storage units had been fitted in almost all the ensuites and the maintenance man planned to continue fitting them on a daily basis. Satisfaction surveys had been sent out to relatives and most had been returned with positive feedback. Three profile beds have been purchased and were in use. • • • • • • • What they could do better:
The manager requires the assistance of an administrator – the post is currently advertised. The improvements made (as above) will need to be self monitored by the management to ensure the standard is sustained and the manager is aware of ways to ensure this happens. A domestic vacancy is to be filled in the near future. A programme of routine renewal of the fabric and decoration of the premises must be implemented on completion of present improvements. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 3 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users do have the information they require to make an informed choice about the home. EVIDENCE: The Statement of Purpose and Service User Guide have now been addressed and are documents, which will be reviewed and updated as required. There was no doubt at the previous inspection that the Service Users and their relatives receive the information verbally and they do have an informed admission however the documentation did not support this. This is now in place. Visitors spoken to at this visit spoke with high regard for the home, the managers and the care that their loved ones received. They said that staff continually ensured they were fully informed of all events in the home regarding their relatives’ health and well being. They said the staff welcomed

The prospective Service Users and their relatives were encouraged to visit the home prior to admission at a time to suit themselves, no appointments were offered. There was evidence that relatives or the Service Users if possible signed the initial agreement regarding the care plan and this will be sustained through the implementation of a care plan check list at the front of the file and future care plan audits that are planned. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 7, 8, 9 and 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Care plans are in place for each service user that are up to date and in good order. Service users are fully met and this is now documented. Medication procedures were satisfactory. Issues relating to respect and privacy were identified as being highly important by the care staff. EVIDENCE: The inspector evidence that each service user had an individual plan of care, which includes elements of their health and personal care needs and some social care needs. The care plans seen by the inspector as part of case tracking within the home did reflect the care that the case tracked service users were receiving and the records were updated at least monthly. Information with regards to needs being met, progress or outcomes is now included.

Issues raised at the previous inspection had now been addressed • • • There was evidence that the service users were involved in the plan of care. Relatives were shown the individual plan of care and asked to sign the relevant risk assessments if Service Users are unable to do so. Service User photographs were evidenced in the care plans. Annual social service reviews were identified and most had now been completed. Much effort had been put into raising the standard of the care plans and this was evident. A new system had been introduced using dividers to keep the plan organised and accessible. Carers continue to assist where necessary with personal care needs; key workers ensured that service users toiletries were available and they were fullt aware of their responsibilities. Again Service Users spoken to at this visit and previous ones said, “They continued to feel that they were treated with dignity and respect”. They said, “The carers encourage them to be as independent as possible in a caring way”. Risk assessments were in place for service users and the Service Users themselves or their relatives signed most. Procedures to ensure consent signatures are gained immediately on admission are now in place. Medication administration in the home followed the homes policy and procedure. Medication stock appeared satisfactory for the size of the home and the service users medication sheets. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12 to 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service Users commented that they had continue to have sufficient activities to keep them entertained and their preferences were expressed. Families and friends visited as preferred by themselves and their relative. Service Users stated that they did have choices over their daily life supported by the carers. Service Users enjoy the food at the home and their preferences are recorded. EVIDENCE: There was evidence in the home of an activity programme, which were activities that were arranged over the week to attempt to meet the needs and capabilities of the Service Users. This was distributed to each individual Service Users in their bedrooms. The activity organiser has recently left the home and the post is currently being advertised, however the carers are supporting the service users needs and ensuring that planned entertainment continues. Photographs displayed in the hall confirmed most service users and some relatives enjoyed the activities arranged. Rose Villa Nursing Home DS0000061869.V358708.R01.S.doc Version 5.2 Page 14 It was identified at a previous visit that some more one-to-one sessions would benefit certain Service Users and also trips out in the community. Trips out had been arranged including pub lunches and trips to the garden centre and Christmas shopping trips. Each individual Service User continues to be encouraged to exercise ‘daily control’ in respect of his or her life style and visitors were welcomed at any time. There were several visitors in the home throughout the visit by the Inspector and those spoken to again… expressed their sincere praise for the staff in the home and the care and compassion given to the relatives and themselves. The menus for the week were displayed for all Service Users to observe. The records seen by the inspector offered choice, good variety and a well balanced diet. The meal for the day was observed being prepared by the cook in a clean, well-organised kitchen. Stores and storage of food was found correctly arranged and of good quality. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 16 and 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service Users felt confident that if they had any concerns they would be dealt with. Their legal rights are supported and Service Users are protected from abuse. EVIDENCE: The inspector evidenced the complaints process and procedure displayed in the entrance hall. Each of the Service Users had been provided with a copy of the homes complaint procedure in the event of having any concerns or a complaint. There was evidence and knowledge from the staff that training and supervision covers issues of abuse, its identification and types of abuse. The Whistle Blowing policy is also in place to support staff in reporting of any untoward incidents. The home or the Commission For Social Care Inspection had received no complaints since the last inspection. Relatives spoken to at the visit were confident that all their requests are met or considered and they had no issues with regard to speaking to any of the staff if they had any concerns. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users live in a safe and well-maintained home that is clean, pleasant and hygienic. EVIDENCE: The inspector inspected the kitchen and issues raised at a previous key inspection had all been addressed. The Environmental Health Officer had visited the home in February 2007 and at that visit no requirements were made. Areas of the home identified as requiring attention had been addressed. The laundry rooms and linen rooms were clean and tidy. Several bedrooms (in the old house) had been redecorated, recarpeted and the furniture replaced. The transformation was to be completed with the matching
Rose Villa Nursing Home DS0000061869.V358708.R01.S.doc Version 5.2 Page 17 curtains and bedding which was on order and due to arrive in the next few days. The seating in the lounge areas is dated and institutionalised, replacement chairs, which are more comfortable and homely, are being considered as part of the refurbishment. The bathrooms, which were previously uninviting and cold, have now been decorated with ceramic fish on the walls and blinds fitted to the windows. The heating has also been adjusted to make the bathing experience a warm and pleasurable one for all service users at any time of the day. Most of the ensuites had had cupboards fitted to avoid storage of any items on the toilet cisterns or on the ensuite floor and it was an infection control hazard. Toilet and bathrooms were evidenced to be clean and no malodours were apparent at this visit. Some Service Users are using bed wedges rather than cot sides to assist their security whilst in bed. Specialist Profile beds to aid the Service Users comfort have replaced 3 beds. Redecoration and replacement of soft furnishings is to continue as required on the homes rolling maintenance plan. The outside area of the home is to be developed to assist Service Users to sit outside in fine weather. One member of the domestic staff had recently left their employment and the post is currently advertised. Relatives confirmed that the home was clean and tidy and the bedrooms warm and personalised. This judgement has been made using available evidence including a visit to this service. Service Users needs were being met by sufficient and capable care staff, the staff told the inspector that they had been trained in the home, and training records were available for the inspector. The recruitment policy and procedure is in place. EVIDENCE: Service users and relatives have confidence in the staff that cares for them at the home. Rotas show the home to be staffed efficiently, with particular attention given to busy times of the day and changing needs of the people who use the service. The manager is confident that the staff is skilled in their role and are consistently able to meet their needs. There is consistently enough staff available to meet the needs of the people using the service, with more staff being available at peak times of activity. The staffing structure is based around delivering outcomes for the individual service users and not led by staff requirements. All staff receives relevant mandatory training that is focussed on delivering improved outcomes for service users.

This procedure is followed in practice with the home recognising the importance of effective recruitment procedures in the delivery of good quality services and for the protection of individuals. Staff recruited confirm that the home was clear about what was involved at all stages and was robust in following its procedure. There are clear contingency plans for cover for vacancies and sickness and there is little use of any agency or temporary staff. Staff meetings take place regularly and notes were evidenced. Supervision sessions are performed regularly and staff find them helpful with a focus on improving outcomes for people using the service. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31, 33, 35 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users do live in a home, which is run in the best interests of the service users. Financial procedures in the home are well managed and the health, welfare and safety of service users and staff are protected and promoted. EVIDENCE: The manager has a clear understanding of the key principles and focus of the service, based on her own values and priorities. She works to continuously improve the service. The home provides an increased quality of life for service users with a strong focus on equality and diversity issues and promoting human rights, especially in the areas of dignity and respect.

The manager demonstrates a strong ethos of being open and transparent in all areas of running of the home. She leads and supports a strong staff team who have been recruited and trained to a good standard. The manager is aware of current developments both nationally and by CSCI and plans the future of the home accordingly. The issues raised at the last inspection were addressed. Some were fully completed and other areas were commenced and were to be sustained. The home works to a clear health and safety policy. All staff is fully aware of the policy and is trained to put theory into practice. Regular random checks will now take place to ensure they are working to it. Safeguarding is given high priority and the home provides a range of policies and guidance to underpin good practice. Maintenance records for the home were of a good standard and are routinely completed. All necessary checks were being made. Service users are aware of safety arrangements and have confidence in the safe working practices of staff. The manager ensures risk assessments involve the service users or their relatives in their production and that they are fully completed and taken into account in planning the care and routines of the home. The registered person has the skills and ability to deliver good business planning, and effective financial controls, they provide a quality assurance and monitoring process to ensure efficient running of the home, which gives value for money and delivers effective outcomes for the people who use the service. Service users are supported to manage their own money where possible. Where this is not possible there is a clear reason why. Individuals have access to their records whenever they wish. The home has access to professional business and financial advice and has all the necessary insurance cover to enable it to fulfil any loss or legal liabilities.
