Date of last inspection 12th April 2005 Brief Description of the Service: 36 Roseberry Gardens is a care home situated in Haringey, close to the facilities of Green Lanes and within a short bus journey to Wood Green Shopping Centre. There are three single bedrooms, a galley kitchen and a dining room combined with a lounge. One of the bedrooms is on the ground floor and the other two are on the first floor. There are small front and rear gardens. The home has a stated purpose of providing a high quality 24-hour residential care for you whilst supporting you and maintaining your independence and choosing your choices. This is an overview of what the inspector found during the inspection. The inspection was conducted over a period of 3 hours, commencing at 11:00 am and concluding at approximately 2.00 pm. Mrs Angelina Espino, the registered manager, was present throughout the inspection. The inspection activity undertaken included a tour of the building, the examination of service user and staff files including care records, the examination of health and safety records, and the viewing of staff rotas. The person who lives at the home was spoken to in private. What the service does well: What has improved since the last inspection? What they could do better:
Three requirements and a recommendation are made. The registered person must review assessments and care plans. Service users and, as appropriate, their representatives and social workers must be involved in the review of assessments and care plans. It is required that all staff working at the home have current CRB checks and that POVAFirst must be applied for while staff are waiting for their CRB certificates. The home’s quality assurance system needs to be fully implemented. The registered person must collate the feedback obtained as a result of the quality assurance exercise and put in place an action plan. More effort is required from the registered person to enable a service user to locate their relatives and to re-establish contact with them. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: These Standards were assessed at the last inspection. No new service users have been admitted since the last inspection. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, and 9 The systems for assessments and care plans are poor. Service users are not sure if and when their needs are assessed and if appropriate care plans are put in place. Service users have benefited from the system of encouraging them to do things for themselves by taking responsible risks. EVIDENCE: The service user who lives at the home was spoken to. They said they have not met their social worker for a long time. The service user said they would wish to see their social worker to discuss their needs. The registered person said she had contacted the social worker and a review meeting had been planned for the following week. This was a surprise for the service user who said that they did not know the social worker had been contacted. There was also no written evidence to confirm that the meeting had been arranged. The service user explained that they were encouraged to travel to shops and to go out for walks in the park. They have front door and bedroom keys. The service user said they preferred their money to be cashed from the post office by the registered person. There are no bedtime restrictions. The service user was observed making hot drinks.

Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 15, and 17 The home has made good progress in encouraging, supporting and providing opportunities for service users to engage themselves at the home and in the community. The food provided is satisfactory with the evidence that service users can have a choice of what and when to eat. The registered person has made a good effort to locate relatives of a service user to establish contacts. More work needs to be done by the registered person to ensure that the wishes of the service user are realised and contacts are re-established. EVIDENCE: The person who lives at the home said they have family members but they do not know their addresses. They said they would like to keep in touch with their families. The registered person stated that she had tried unsuccessfully to locate the service user’s families. She said she would continue trying by using various options including the Internet to try to locate the service user’s families. The service user confirmed that they attend a day centre where they meet with friends, do different activities and get paid some money. They said they are happy with their day arrangements. When they do not go to the day centre,

At the last inspection a requirement was made for the registered person to consult a service user and provide them with appropriate social and leisure activities. The service user referred to is no more a resident at the home. The service user said they are happy with the food provided at the home. They said they feel their needs are met regarding food. The service user stated that they do not practice their religion and do not mind eating any kind of food prepared by the registered person. The service user can make hot drinks and is involved in making decisions about the food they eat. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, and 20 Good progress has been in respect of meeting service users’ personal and medical care. A service user whose needs could not be met at the home has moved to an alternative accommodation. The systems for administration and recording of medication are satisfactory. EVIDENCE: At the last inspection requirements were made for the registered person to arrange for an occupational therapist assessment for a service user and provide them with appropriate equipment and facilities that meet their needs. The registered person confirmed that following an occupational therapist assessment and consultation with the placing authority, a decision was made to move the service user to a more suitable accommodation. The registered person said this has happened about six months ago. Currently, only one person lives at the home. The service user said they are independent in carrying out self-care tasks. Medication is kept in a locked cabinet. The registered person and the staff who administer medication have undergone medication administration training. The medicines and medication administration record sheets were correct on the day of the inspection. JUDGEMENT – we looked at outcomes for the following standard(s): 22 The home’s complaints processes are adequate to ensure that all complaints are appropriately investigated and resolved. EVIDENCE: A previous complaint about the home has been investigated and resolved. The person who lives at the home stated that they know how to complain and they can talk to the registered person. It was stated above that the service user does not know the address of their relatives. The home has a complaints procedure which explains the process of investigation, the date by which a response would be made to the complainant, and the local office address of the commission for Social Care Inspection. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, and 30 The Standard of the home is good providing service users with a safe and comfortable environment to live in. EVIDENCE: The home is located in a residential street close to shops and cafés of the Green Lanes Road half way between the Turnpike Lane and Manor House Underground Stations. There is a visitor’s sitting room and a lounge. The lounge has been refurbished with new floor tiles and new double-glazing doors. The service user said they are happy with their room and the facilities of the home. They said they have keys to their bedroom and the front door. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 33, and 34 There is a gap in the home’s recruitment process with the evidence that care staff have been allowed to work without current CRB check. This potentially puts service users at risk. The home has staff team in sufficient number, knowledge, skills and experience to meet the needs of service users. EVIDENCE: The registered person and her husband, who is also one of the registered providers, are working at the home. They are supported by three part time care staff. The registered person confirmed that a minimum of one member of staff is at the home at all times. As stated elsewhere, currently there is only one service user living at the home. Documents at the home indicated that the staff have attended a range of training programmes including medication administration, health and safety, basic food hygiene and first aid. None of the care staff have a care qualification equivalent to NVQ level 2. However, one member of the staff is undertaking a nursing qualification. The registered person was working alone at the time of the visit. The service user said the staff are good and they are happy with the home. The staff files, which were assessed, showed that the staff have been interviewed and two written references have been taken. Evidence was also available to indicate that all the staff have undergone CRB checks. An examination of the CRB’s revealed that the staff CRB’s were obtained as part of their previous employment or study at another organisation and not specifically carried out as part of their current job.

Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 39, and 42 EVIDENCE: At the last inspection the registered person was required to collate feedback received through quality assurance questionnaires and devise an action plan, which addresses any shortfalls in the quality of the service. It was also required that service users, relatives and professionals are informed of the outcome of the quality assurance system.These are yet to be complied with. The home was clean and tidy on the day of the inspection. It was evident from the records that the gas boiler was serviced on 8/4/05 and the fire alarms have been tested regularly. There are smoke/heat detectors and fire fighting equipment in the home. The home is not registered to accommodate service users with a mobility difficulty. No incidents accidents have been recorded.
