Conditions of registration: Date of last inspection 8th December 2005 Brief Description of the Service: Roselea is an adapted period property situated discreetly in a residential area. The home is registered to accept up to 9 residents who have a learning disability, several of whom may also have behaviours, which challenge. The accommodation is over two floors, but there is no lift so unless a ground floor bedroom is available any potential resident would need to have good levels of mobility. The centre of West Bridgford is around a ten to fifteen minute walk for those with good mobility. It has good access top a range of shops, cafes, restaurants, parks, the post office, banks and public houses. The home has its own minibus for the use of the residents. This is an overview of what the inspector found during the inspection. This inspection was carried out unannounced over 5hours to assess the key standards as recorded within this report. There was a range of information used to inform this report including 8 completed and returned surveys of the resident’s views. The previous compliance with meeting the National Minimum Standards was considered and used to determine what areas required further checking at this inspection. The main method of inspection was to case track, this method requires the inspector to read the care plans of a resident and look at how the care is provided and if the person’s needs are being met. The manager and senior care staff were spoken with along with a visiting college tutor. Three residents were spoken with and their individual rooms inspected along with the facilities they access within the home. The Social Services are presently undertaking an adult protection investigation which is not concluded, one are of this concern was discussed with the manager and requirements set as a result. What the service does well:
The manager has the required qualifications and experience and is competent to run the home. She is resident focused and leads and supports a stable staff team of higher than average numbers. Residents and external professional views show that the staff team are skilled and are allowed time for activities and community involvement for residents. There is a strong commitment to enabling residents to achieve their potential in all areas of their life. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Residents are only admitted to the home following a comprehensive assessment of their needs to determine if the home is suitable and able to meet the needs of the person. EVIDENCE: The home has a sustained track record of meeting the standard around obtaining appropriate information and assessment of any prospective resident to the home. This ensures that the home is able to meet the needs of any prospective resident prior to admission. The homes statement of purpose was examined and the document describes the process of admission to the home. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6-7-9 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Residents in the home know they have their needs assessed and that their personal goals are reflected in a care plan. Residents are enabled to make decisions about their lives and assistance is given wherever necessary to manage any risks to their safety. EVIDENCE: Standard 6 The home has a sustained track record of meeting the standard during 2005/6. 2 care plans examined showed that the home provides a care plan for each area of personal and social care support that the resident requires. There are positive, planned interventions with good emphasis on assisting with communication to ensure the residents are given choice and are listened to.

Through observation and discussion with the residents it was clear that they were able to and supported to make decisions about their own lives. Residents are supported to take risks as part of an independent lifestyle. Care plans contained clear evidence that any potential hazards to the person’s safety and well being were considered and strategies were in place to manage these risks. Service users are offered a healthy diet and enjoy their meals and mealtimes. JUDGEMENT – we looked at outcomes for the following standard(s): 12-13-15-16-17 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Residents are supported to integrate into the community and benefit from a varied and fulfilling social life. Residents are encouraged to be independent and to maintain family links and friendships. Healthy food choices are in place and residents benefit from a balanced nutritious diet that respects their individual preferences. EVIDENCE: Residents are provided with opportunities inside and outside of the home to further their education. The tutor from the local college was attending the home during this inspection to conduct various activities with the residents.
Roselea Care Home DS0000008800.V294291.R01.S.doc Version 5.2 Page 12 The tutor spoke of the excellent participation by the home in supporting residents with activities and showing commitment to improving their lives. Three residents discussed their daily lives and the range of things on offer, which they found very enjoyable and fulfilling. The manager and staff have developed many links within the community to support the residents and make use of local facilities such as the pubs, restaurants and swimming pool. Residents spoke of being able to go home and visit their relatives and friends and that staff were able to take them if required. One resident spoke of not wishing to participate in group activities but was being supported to plan and work in the gardens at the home that was a great source of enjoyment for her. Residents spoke of how they plan the menu for the week on Wednesday’s and then assist with the shopping if they wish. Mealtimes were flexible and residents were happy with the food provided. The manager spoke of healthy eating options that are made available. Residents have keys to their rooms and staff do not enter without invitation. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18-19-20 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. The personal and healthcare support the residents receive is based on their individual needs. Residents are protected by the homes policies and practice in the management of medicines. EVIDENCE: Each resident has a care plan that describes in detail the wishes of the resident and their preferences in how they wish to be supported by staff. Residents were dressed nicely and had individual appropriate appearance that they chose and that reflected their personality. Care plans demonstrated that there was good support from external professionals. Care plans required more assessment of the residents sexual health and the required precautions necessary to safeguard them and to ensure they were supported with their consent to have relationships as they choose. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22-23 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Resident’s views are listened to and acted upon, robust procedures are in place to safeguard the residents. EVIDENCE: There are robust procedures in place to ensure staff know how to deal with any suspicion or allegation of abuse. Staff receive training in adult protection and physical and verbal aggression to ensure these are dealt with appropriately. There is one adult protection investigation ongoing at present. This investigation is almost concluded and will determine any action that needs to be taken to safeguard the residents at the home. The complaints procedure is in a suitable format for the residents. There is a sustained track record of meeting the minimum standard. Responses received from residents show they know who to speak to if they have any concerns. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24-30 Quality in this outcome area is good. This Judgement is made using available evidence including a visit to this service. Residents live in a homely, comfortable and safe environment EVIDENCE: Standard 24 has a sustained track record of compliance with the minimum standards. The premises are safe, comfortable, bright, airy and clean. The home is not suitable for people with mobility problems as there is no lift to the first floor bedrooms. The last Environmental Health and Fire Inspection reports show the home is compliant with the requirements of both departments. The laundry facilities have been upgraded since the last inspection with new equipment and the replacement of the old flooring with a non-permeable flooring to ensure infection control standards are met.

This Judgement is made using available evidence including a visit to this service. Residents are protected by the homes recruitment practice and needs are met by a trained staff team in sufficient numbers to meet the individual needs. EVIDENCE: The recruitment of staff is appropriately managed with a track record of ensuring residents are protected from people who may be unsuitable to work with vulnerable adults. The records of individual staff training show the manager has an appropriate development programme in place, which ensures staff are trained to undertake their role. Staff receive any training needed to address specific problems and needs of the residents. The majority of staff are qualified in NVQ or working towards certification and this is linked with Learning Disability Award Framework, which is accredited training.

The management of the home is based on openness and respect, has an effective quality assurance system in place. The health safety and wellbeing of the residents is protected. EVIDENCE: The manager of the home is competent and experienced. She is registered with the Commission for Social Care Inspection and regularly updates her knowledge and skills to ensure best practice. There is a quality assurance monitoring system in place and residents, relatives and external providers are asked for their views on the service provided. Feedback on questionnaires is provided. The manager ensures that staff are trained in all areas of health and safety and that they receive appropriate updates as required. There was excellent attention to the provision of suitable fire safety procedures with regular fire drills and training. All staff are trained in first aid and food hygiene. Training and procedures are in place to ensure staff are able to move people safely to avoid any injury. Evidence was seen of Hot water safety and compliance with Legionella precautions. Electrical equipment is checked annually. Present refurbishment and replacement of windows is being undertaken safely. Any accident or incident is recorded.
