This inspection was carried out on 14th February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This was a very positive inspection with evidence of the staff working very hard to maintain high standards of care and services. Staff engaged in conversation were very enthusiastic about their workplace and reported ` This is a very homely environment, we all help each other out` `we give each other lifts to work and support each other`. One stated `this is the best place I have ever worked in`. When asked if they had any negative comments to make about the home, no one could think of anything to say. The Home is very well managed and staff felt very supported by the managers and it was evident that the Home is run in an open and inclusive manner. Residents were very happy with the care they received and the visitors spoken to reiterated this. Staff work hard to ensure that dignity and respect is upheld and maintained, sometimes in difficult circumstances and this was commendable. Residents are treated as individuals and their personal preferences and requirements are upheld wherever possible. The meals are interesting, wholesome and varied with special themed meals served and kitchen staff willing to go that bit further to accommodate residents.The programme of activities and entertainment is extensive and co-ordinators work hard to ensure that this is delivered and tailored to meet individual needs. The Home is very clean well presented and well maintained and the records relating to this are effectively organised, ensuring that staff work and residents live in a safe environment.

What has improved since the last inspection?

New carpets had been provided in the main lounge and throughout the corridor areas, thus improving the appearance of the Home. Two new kettles, one toaster and a new supply of cutlery have been purchased for the kitchenette. The Managers were now in the process of undertaking the Registered Managers Award.

What the care home could do better:

There were no requirements made as a result of this inspection. Recommendations included deep cleaning or renewal of carpets in the mentally frail unit, regular checking of all wheelchairs and updating of the Statement of Purpose. When residents were asked what would improve their life in the Home they could not think of anything. One resident commented that he would rather be at home but as this was not possible this Home was "the next best thing".

This includes personal care of service users with a mental disorder over 65 years of age and care of elderly service users who have physiacal disabilities.The home is owned by a company – Southern Cross Healthcare Services Limited.The home is located within walking distance to the Staffordshire town of Newcastle-Under-Lyme with its array of amenities. The home is relatively new having been purpose built in recent years and consists of two floors served by a passenger lift. There is ample car-parking facility at the entrance car park. Gardens are accessible around the home to service users including wheelchair users.The accommodation provides for single bedrooms, all of which have en-suite facilities. There is ample provision of communal and seating areas throughout the home. There are three units within the home. On the ground floor there are two units – one which accommodates service users needing personal care and the other accommodating service users with personal care including mental health care needs. The second floor accommodates service users requiring nursing care. All units are staffed and managed separately. This unannounced inspection was carried out over four hours by two inspectors. During the course of the inspection inspectors spoke with residents, staff and visitors. Records and documentation were also examined and a tour of the home was undertaken. Not all of the standards were assessed during this visit but those not assessed last time and those not fully met were examined. Both registered managers were off duty at the time of the inspection but Rachel Simpson came in to meet the inspectors. The regional manager had also arrived to carry out an unannounced inspection at the home. Verbal feedback was given at the end o the inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3 and 4 All residents undergo a thorough pre admission assessment and are given the information in order to make an informed choice before moving in. Residents can be assured that this home will be able to meet their assessed needs. EVIDENCE: The home provides a Statement of Purpose and Service User Guide. Copies of these were seen kept in the visitors’ lounge in the main entrance. These were informative and included copy of the “Southern Cross Times” which is a magazine produced regularly by the Company highlighting events and achievements at different homes. The Statement of Purpose will need to be updated with the details of the second registered manager. The inspector was shown a sample contract for residents who are self funding at the home. All residents accommodated are given a Statement of Terms and Conditions when moving into the home. Rowan Court Nursing & Residential Home DS0000026963.V284163.R01.S.doc Version 5.1 Page 9 Discussion with the deputy manager on the nursing unit revealed that, upon referral, prior to admission, a suitably qualified member of staff carries out an assessment of need to ensure the home can meet the individual needs of the resident and meet the registration categories of the home. Inspection of records that were sampled demonstrated residents received a care management assessment that detailed the support required. Through direct observation, discussions with residents, relatives and staff and examination of records it was confirmed that the home was meeting the assessed needs of the residents on a continuous basis. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Residents in this home received good standards of personal and nursing care and this care was delivered in a professional and dignified manner. EVIDENCE: Inspection of records demonstrated each individual had a plan of care based upon the initial assessment. The plan of care contained a photograph, personal details, previous history and support and care required, with activities associated with daily living. Where appropriate monitoring charts were completed and risk assessments were in place for nutrition, moving and handling, falls and skin integrity. Each individual had a primary nurse and a key worker allocated to them who completed a daily dairy and personal hygiene records. All plans were reviewed monthly to ensure the continued appropriateness of the plans and where possible residents or a representative are encouraged to be involved in this process. Where appropriate nursing staff met the identified health needs of the residents and specialist health care professionals attended the home. Each resident is registered with a local General Practitioner. The deputy manager reported staff escort residents to attend medical appointments if there are no relatives available. Nutritional screening forms part of the initial assessment and where a risk was identified this was incorporated into the plan of care and reviewed monthly. A record of health care appointments and details of all outcomes was recorded. Observed practice on the day of inspection was appropriate and showed respect for the residents. Contact with significant others is encouraged by the home and facilitated for where possible. Visitors are able to meet residents in their bedrooms or the day/dining rooms offer privacy when not being used. Residents were not all able to hold meaningful conversation to some confused states but those who could offer an opinion were satisfied with their residency. A relative spoken to during the inspection confirmed that staff treated her mother with respect and she felt that her mother’s privacy and dignity was maintained at all times. The residents on the Nursing Unit looked very well cared for by a determined and committed care team. Observation of the medication administration round on the Residential (mental frailty) Unit confirmed that this process was effective and carried out as required. Medication Administration charts had been completed as required. The manner in which the residents were addressed by the staff on the residential units was commendable. Residents were treated with dignity and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Residents are helped to exercise choice and control over their lives wherever possible throughout all the routines of the home. EVIDENCE: There is an extensive and effective programme of therapeutic activities in the home. The programme is tailored to meet individual needs and capabilities. Entertainers regularly visit the home and residents enjoy trips out during the summer months. The home employs dedicated activity co-ordinators to oversee this programme. On the day of the inspection a Valentine’s dinner and dance evening had been arranged with a special themed dinner to accompany. Discussions with the kitchen staff identified that they regularly hold themed evenings and dinners, these had included a Mexican and Chinese night. Residents on the residential units were observed eating their lunches. This was seen as an enjoyable experience with staff helping to create a pleasant and unhurried atmosphere. The meals served appeared wholesome and nutritious and there was an alternative available at every mealtime. Rowan Court Nursing & Residential Home DS0000026963.V284163.R01.S.doc Version 5.1 Page 13 The staff on the mentally frail unit worked very hard to maintain dignity for the residents over lunchtime, which can be especially difficult with residents who have challenging behaviour needs. Residents can expect to receive their mail unopened and private telephone facilities are readily available. Bedrooms are appropriately personalised and residents are able to take their personal possessions into the home. Where residents are capable of managing their own affairs the home attempts to support this. Visiting is welcomed at any reasonable time and residents are able to receive their visitors in the privacy of their own rooms if they so wish. The Statement of Purpose highlights the open visiting policy of the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 Residents could be assured that their legal rights would be protected in the home. EVIDENCE: Discussion with staff indicated that there was great awareness with regards to promoting choice. Staff reported that there are up to 8 residents able to vote and one that manages her financial affairs independently. Advocacy services are sought where necessary; a resident had been accessing this service up to recently and this had worked out very successfully. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Observation and discussion confirmed that residents are living in a safe and well-maintained environment. This included easy access both internally and externally. The home had adequate washing and toilet facilities, including comfortable and personalised bedrooms, all clean, and free from odours. EVIDENCE: The home is purpose built to meet the National Minimum Standards. All areas of the home were decorated and furnished to a high standard. Individuals are able to all access all areas of the home and there is a choice of communal areas and dining rooms. To the rear of the home is a patio area with seating. An appropriate fire system was installed at registration and the home has maintained this system and equipment. Staff reported they have fire training and take part in fire evacuations. The deputy manager reported where equipment or furnishings need replacing the home has an adequate budget to ensure the high standards are maintained. There were no CCTV cameras installed in any areas of the building On the first floor there is one unassisted bathroom and two assisted bathing facilities. There were also three separate shower rooms. There were two sluice areas, which housed mechanical disinfecting/cleaning machines. Hand washing facilities were provided in all residents’ bedrooms. There were suitable toilet facilities located near all residents’ areas The home has a range of adaptations and equipment to ensure the needs of the residents are met. On the top floor, there were four mobile hoists and two assisted bathrooms. Corridors have handrails and doors are an appropriate width to provide wheelchair access. There are even surfaces to gain entrance to the building and there is a patio area at ground level. The home has limited storage facilities but manages to accommodate all equipment. There is a passenger lift to the first floor. Most equipment has been serviced and the staff reported they have received training to use all equipment safely. However, the staff reported that some of the wheelchairs had not been serviced, as they did not belong to the home and the residents who had previously owned them had since passed away. Three residents had their own electrical wheelchairs, which were serviced independently. There is a nurse call system fitted in each room to enable staff and residents to summon for help. Care staff confirmed that this was fully operational. There are two double rooms and all other bedrooms are single and all have ensuite facilities. Each room that was seen on the inspection had sufficient space to ensure care needs can be met and contained the recommended furnishings. There were adequate electrical sockets, television aerial point and facilities to allow residents to have a private telephone line. Individuals are able to bring in their own furniture and rooms were personalised with photographs, pictures and belongings. The double rooms have interlocking doors and the staff reported that double rooms are occupied only when there has been a positive choice to share. Residents are able to have a key to their room and the locks have an over-ride device to enable access in emergencies. All rooms are doubleglazed and are able to be opened to naturally ventilate the room. First floor windows were restricted. Thermostatic valves are fitted to water outlets and a random selection of hot water outlets found the water to be satisfactory. The home has domestic style lighting and emergency lighting is provided in throughout the home. The home was found to be clean, hygienic and free from offensive odours. All staff were provided with appropriate protective equipment and all areas were clean and appropriately maintained. The inspector was informed that the laundry room provided professional washing and drying equipment to meet disinfectant standards. Soiled linen was transported appropriately with foul linen in red acetate disposable bags. The laundry room was inspected and found to be clean and well organised. The kitchen was inspected and found to be hygienic and well organised, some changes to storage facilities had been made since the last inspection and this had been advantageous. There was an ongoing programme of redecoration and refurbishment at the home and the inspector was provided with details of this. It is recommended that the carpets in the lounge and corridor areas in the residential mentally frail unit are deep cleaned or replaced as these are becoming badly stained. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Residents could expect continuity of care provided by sufficient levels of welltrained and motivated staff. EVIDENCE: On the nursing unit there are usually two trained nurses on duty on each of the three shifts per day, additionally, on the: • • • Early shift (8.00 – 2.00) there are 7 care staff (There is also a hostess on this shift to deal with catering aspects on the nursing floor) Late shift (2.00 – 8.00) there are 5 care staff Night shift (8.00 – 8.00) there are two care staff There were 38 residents on the nursing unit at the time of the inspection including one in hospital with four vacancies. On the day of the inspection it was determined that there were appropriate staffing levels and skill mix on duty. The staff complement and their deployment were discussed with some of the care staff and each said that they considered that there were generally sufficient levels available to meet their needs. A team of domestic staff caters for all the requirements in the home between them. The level of cleanliness seen in the home was very good and a credit to the team. Rowan Court Nursing & Residential Home DS0000026963.V284163.R01.S.doc Version 5.1 Page 19 On the Residential Unit there were 15 residents accommodated and two care assistants on each shift throughout the day, one being a senior carer. There was one care assistant on duty throughout the night but if assistance was required this was available from one of the other units. On the mentally frail residential unit there were 16 residents accommodated and there were three care staff on duty all day, one of which was a senior carer and then two carers at night. There were sufficient numbers of ancillary staff. The acting administrator had now been made permanent. The maintenance person was employed full time and had worked at the home for many years. There were two activity co-ordinators employed. The manager stated that there was a good staff team with no current vacancies. Staff moral was noted to be high with a dedicated, caring staff team. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35 and 38 The Home is very well managed and the health, safety and welfare of residents and staff are promoted al all times. EVIDENCE: There was a Manager appointed to each of the three Units throughout the home and two Registered Managers who job shared the role. Each of the Managers had been in position for a considerable length of time and this had a positive overall effect on the Management of the Home. There had been consistency of the management of the Home and this was reflected in the high standards of care and services delivered. Very positive comments were received from residents, visitors and staff in relation to the management. Comments indicated that management was open and inclusive and that staff felt supported. The Operational Manager who was visiting the Home at the same time as the inspection supports the managers and staff in the home. She offers support and advice on a continuous basis and sends in a regulation 26 quality assurance report each month to the CSCI. There was no cause for concern raised in respect of financial viability of the Company and so confirmation of Company financial status was not requested at this inspection. There was a valid insurance certificate displayed in the entrance to the home, which included Public Liability Insurance. The records in relation to the maintenance of individual finances and pocket monies was examined and found to be in order. Maintenance records were examined. The documentation had been changed since the last report. The records had been very well maintained and the home benefited from having good maintenance support.
