The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home is welcoming, well maintained and clean. Staff ensure service users needs and interests are identified to enable their needs to be met. Service users spoken to during the inspection said that daily routines are flexible, they can choose where they spend their day, have their meals, get up and go to bed. Service users say that they enjoy the meals available within the home. The homes policies and procedures protect vulnerable people. The homes quality assurance system audits practice within the home and ensures that the home is run in the interest of service users.

What has improved since the last inspection?

The Manager has taken advice given to improve laundry systems to minimise the risk of infection. Twelve staff have enrolled to undertake their National Vocational Level 2 Qualification in care.

What the care home could do better:

This home provides excellent care for its service users. More qualified care staff to meet the requirement of 50 % qualified care staff will only further enhance care that service users at Roxburgh receive.

Service user category SI remains until such time that the current service users placement is terminated. 18th October 2005 Date of last inspection Brief Description of the Service: Roxburgh is a large home providing accommodation for 47 residents who require personal care only. It is owned by Southern Cross Healthcare Limited who own a large number of homes throughout the UK. Situated on the border of Old Hill, it is easily accessible by public transport from Dudley, Halesowen and Birmingham. The local amenities include a library, community centre, post office, shops and public houses. The home has two large lounges situated to the left and right of the main hall with dining areas off each lounge. Throughout the home there are five double bedrooms and thirty-seven single rooms, one of the latter having en-suite facilities. Bedrooms are located on the ground and first floor and there is a lift available for residents to access all communal areas of the home. There are numerous toilets situated throughout the home and there are 6 assisted baths and 1 walk in shower room. A hairdressing room is also available. There is a small pleasant garden area that residents enjoy on fine sunny days. There is a car park situated to the front of the building that is quite reasonable in size. This was an unannounced inspection undertaken by one Inspector. The inspection was undertaken between 13.30 and 16.00hrs. The inspection followed a visit to the home in late December to agree to register the new Dementia Care unit. This visit included a review of records, talking to staff and service users. Care records were reviewed as part of the “case tracking” of three service users. Roxburgh is owned by Southern Cross Health Care Ltd. Mrs Tracey Mancini is the registered manager. One of the previous three requirements has been addressed, one requirement was made as a result of this inspection. The home has a new dementia care unit has recently been completed and will open shortly. What the service does well: What has improved since the last inspection? Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 Service users needs are identified and met. EVIDENCE: Service user’s all have detailed person centred care plans and care risk assessments. Risk assessments available include the risk of pressure sores, moving and lifting of residents, continence, nutrition and falls. Care plans and risk assessments are generally reviewed monthly. Service users and their representatives are involved in the planning and review of their care should they wish to be. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13,14 Services users are able to exercise choice and control over their lives and maintain contact with friends and family. EVIDENCE: Service users and visitors spoken to during the inspection and feedback from the previous inspection said that visitors are always made welcome by staff. Service users spoken to said that they are able to get up and go to bed when they wish, have a choice of meals and are able to choose whether they take part in the activities and social events. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The home safeguards service users from abuse. EVIDENCE: The home has a Southern Cross adult protection policy. Local authority adult protection policies are available but the home needs ensure that local policies are referenced and incorporated into their adult protection policy. All staff spoken to had good awareness of what is abuse and appropriate actions to be undertaken in response to any abuse allegations. Staff training in abuse awareness is ongoing for all staff. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The home is clean, hygienic and a pleasant place to live. EVIDENCE: Areas of the home seen during this inspection were found to be clean and homely with no offensive odour. The home has been completely refurbished within the last eighteen months and is decorated and furnished to a high standard. There is a large lounge and another lounge/ dining room, a separate smoking room is also available. A new and innovative dementia care unit has been created for ten older people with dementia. The dementia unit consists of a quiet lounge, a lounge and dining room. The unit also has a snoozelum that is used as a quiet and restful area. Snoozelums can calm distressed and agitated people with dementia with the use of soothing music, different lightening and aromatherapy and assorted items that can be touched. The home has appropriate procedures in place to minimise the risk of infection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,35 The home is run in the best interests of the service users. Service users financial interests are safeguarded. EVIDENCE: Southern Cross homes have an identified Quality plan for the home. Quality audits are undertaken six monthly with corrective actions identified, with a copy of the audit sent to both the Regional Manager and Regional Director. The home undertake monthly audits of pressure sores, service users weights, accident statistics, vacancies and recruitment, the kitchen and a review of all regulation 37 notifications that have been sent to the Commission for Social Care Inspection (CSCI). Weekly audits of care plans are also undertaken. A service user survey has been undertaken with results analysed and put into the service user guide and the homes newsletter. A new survey has recently been undertaken with plans in place to feedback the results to service users and all interested parties. The Manager and registered company proactively identify ways that requirements of Commission for Social Care Inspection will be met. The home displayed posters advertising the announced inspection which was undertaken in September. The majority of service users money is managed by their families, although service users can manage their own finances if they wish to. Secure facilities are available for the safe keeping of service users personal money and valuables. Written records are available for all transactions which detail the reason for the withdrawal and two signatures, receipts are available as proof of purchases. External audits of service users personal money are undertaken with an audit undertaken the week before the inspection.
