The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has been successful in maintaining a permanent staff group and a limited number of regular Agency staff. This is beneficial to the service users as it ensures continuity. It was confirmed by the Registered Manager, that of the 21 permanent care and senior staff members, 16 have obtained NVQ level 2 training, four are currently receiving training and two are scheduled to commence training later this year. The records indicated that apart from the Topps Induction Foundation course, a programme of regular in-house training is being delivered to the care staff at the home. Staff members receive regular supervision and team meetings are held on a monthly basis. All documentation and records seen at the time of the inspection were satisfactorily kept and up-to-date. The home was organised, well run and staff members were observed to respond sensitively and appropriately to the needs of the service users.

What the care home could do better:

Detailed financial records relating to individual service users were not accessible at the time of the Inspection. The Registered Manager said that these were logged onto the computer and could only be accessed by the Administrator who was not present during the inspection.

It is owned and managed by the London Borough of Hounslow. It is situated in a residential area of Bedfont and there are local shops and public transport within half a mile. The home is a two-storey residential home which is divided into four units, Swallows and the Intermediate Care unit on the ground floor, and Lapwings and Magpies on the first floor. These are pleasantly furnished in a domestic style. In each of the units there are kitchen facilities, a lounge, dining area, a bathroom and toilets. A large communal lounge on the ground floor is used for entertainment, functions and meetings. There is a coffee room adjacent to the lounge for visitors, residents and staff to use. A large well-furnished conservatory is next to the lounge. This is used as the smoking area. The Rehabilitation unit provides a short-term service for seven clients who require rehabilitation either before returning home from hospital or to remain at home. There is a maximum six week stay. This unit has its own manager and team of staff, which includes a physiotherapist and occupational therapist. The home has an extensive garden with a pond and seating. A corridor links the home with a day centre, which has its own separate entrance and facilities. There is adequate parking for staff and visitors, both in front of the home and in the nearby road. This Unannounced Inspection was carried out between 10.15 am and 3.25 pm on 13th August 2005. The Registered Manager, two senior staff members and six care staff were present throughout the inspection. A selection of documentation was examined and a tour of the building and its surrounding gardens was undertaken. The Inspector spoke to six service users all of whom reported positively about the care they received at the home. Discussions were held with a total of six staff members selected from the early and later shifts. They expressed satisfaction with the working conditions at the home, the training delivered and the support provided by the Registered Manager who was described as being approachable and inclusive. Overall, the home was found to be hygienically clean, suitably furnished and well maintained. The atmosphere was calm and homely. The service users appeared healthy, appropriately dressed and comfortable within their environment. The Registered Manager was co-operative and provided appropriate assistance throughout the inspection. There were no requirements or recommendations arising from this report. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5,6. The home’s policies and procedures regarding the admission of new service users within their permanent and rehabilitation units were in place and satisfactory. There were four permanent vacancies at the time of the inspection. EVIDENCE: The Registered Manager reported that prospective new service users were referred to the home by Social Workers from Hounslow Locality teams after initial assessments had been undertaken. This was followed by the home’s own assessment process which required prospective service users to visit the home. Interested family members, relatives, friends and relevant medical professionals – such as General Practitioners, Occupational Therapists, Physio Therapists and District Nurses were involved, where appropriate. New service users were initially placed on a six-week trial basis after which a review would be held to determine the suitability of the placement. All new service users were required to sign a written contract/statement of terms and conditions. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9,10 and 11 Care plans containing the individual medical needs of the service users are in place and satisfactory. The records indicate that specific health care needs such as Chiropodist, G.P. and District Nurses are accessed when required. Service users also receive regular dental and eye checks. EVIDENCE: A sample of service users’ care plans was examined during the inspection. These were found to be fully detailed. Service users individual needs including health, diet, personal care and social interests had been assessed and aims and action were clearly stated. All care plans and related risk assessments were regularly reviewed. Medication was appropriately secured and start dates were recorded on bottles. Administered medication had been properly documented, signed and witnessed/counter signed by a senior staff member. Medication training was delivered to all care staff. The Registered Manager confirmed that none of the service users were currently capable of self administering their medication. Service users were treated with respect and assistance with personal care was carried out in the privacy of closed bedrooms or bathrooms. Specific requests regarding the death of a service user were established and documented during the initial assessment process. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 It was indicated on individual care plans seen that service users receive choice regarding meals, what to wear, social activities and interests. Service users also received weekly allowances and were able to purchase items of choice. EVIDENCE: Service users were observed being involved in both separate and group activities. Some watched television in their bedrooms, other service users read or sat in a group or separately in an adjoining lounge. The Registered Manager reported that three service users attended a Church Service on Sundays. A Catholic Priest delivered weekly Holy Communion at the home and a Church of England Service was held each month. Indoor activities included bingo, arts and crafts, gardening and cooking. Occasions such as birthdays, Halloween, Mother’s Day and Valentine’s Day were celebrated. The home operated an open policy regarding visitors and relatives and friends were invited to all activities held at the home. Two cooks and one kitchen assistant are employed at the home and it was indicated on the menu that service users were able to choose from a variety of nutritious meals. Snacks and drinks were also readily available. Care staff reported that although service received assistance with personal care they are offered choice regarding what to wear, make up or hair styling. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 An updated manual on Hounslow’s policies and procedures regarding the Protection of Vulnerable Adults was in place. The Complaints Book was satisfactorily documented. EVIDENCE: The home’s complaints procedure was clearly detailed in a question and answer format within the service users guide. There were leaflets and information regarding complaints was also pinned on notice boards. No complaints had been made subsequent to the last inspection. All care staff received training on abuse and the protection of vulnerable adults. Care staff who spoke to the Inspector confirmed their knowledge of the policy relating to Whistle Blowing. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25 and 26 The home was clean, well maintained and suitable for meeting the communal needs of the service users. Four domestic staff members and a handyman are employed at the home. EVIDENCE: A tour of the home and surrounding gardens was conducted at the time of the inspection. The home was clean, comfortably furnished, bright and well ventilated. The communal areas were designed for joint activities as well as for individual use. The Gardens were well maintained. The service users’ bedrooms were tidy, suitably fitted and reflected individual tastes and choices. The bathroom and toilet facilities were hygienically clean and appropriate for meeting the personal needs of the service users. The overall environment within the home was pleasant, comfortable and homely. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30 Apart from the Registered Manager, two Assistant Managers, three Senior Staff and eighteen care staff are employed. Six care staff and two seniors are on duty during the morning and afternoon/evening shifts. There are two waking care staff during the night. The staff group at the home are in the majority permanent and are experienced, fully trained and competent in meeting the needs of the service users. EVIDENCE: The Inspector viewed a sample of personnel files and found that they contained all the required documents. Training programmes and certificates seen indicated that care staff received appropriate training for ensuring competency in meeting the needs of the service users. An adequate number of staff was on duty at the time of the inspection. They were observed to respond sensitively and appropriately to the needs of the service users who related to care staff in friendly manner. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 35, 36, 37 and 38. The home is organised and well run and service users health, safety and welfare are being protected. EVIDENCE: The Registered Manager has been in post for one year and is currently a participant on the Management Award Course. Staff members who spoke to the Inspector, confirmed that they received regular supervision and that there were yearly appraisals. The Registered Manager reported that the Civic Centre held overall financial responsibility in relation to the service users. However, they handled weekly allowances used for hair dressing expenses, toiletries and various personal shopping needs. Although service users signatures had been obtained in relation to monies issued, the Registered Manager confirmed that detailed breakdown of Sandbanks Care Home G61-G10 s32618 Sandbanks v238463 120805 Stage 4.doc Version 1.40 Page 16 individual expenditure was kept on the computer, which was not able to be accessed at this time. All health and safety checks including the fire alarm system, water temperature, emergency lighting, fire drills and tests for legionella, were upto-date. The home’s policies and procedures for safeguarding the welfare of the service users had been satisfactorily updated.
