Conditions of registration: Date of last inspection 6th September 2005 Brief Description of the Service: Sharnbrook house is a home owned by the Greensleeves Homes Trust, within a old Victorian house located in a small village with the same name. This is close to Bedford and positioned in 2.5 acres of land with a lake and swans at the bottom of its garden. The house is well maintained and tastefully furnished following its original style. The home offers a real village atmosphere in a very homely environment. In the small village all amenities are close and the home has developed community connections with local people and institutions. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 23rd February 2006. A senior Care assistant Rachel Damon was present throughout the inspection. Many of the areas within Sharnbrook House were visited and the inspector spent time with many of the residents’ in one of the lounge areas of the home. The care of two residents’ was examined in depth by looking at their records and interviewing the residents’ and staff who look after them. Observations of care practice and communication between the residents’ was also made at the inspection. The focus of this inspection was to look at the core standards not assessed at the inspection in September 2005 and to follow up on any previous requirements made at that inspection. What the service does well: What has improved since the last inspection?
A document known as a risk assessment is kept within the care records of each resident, this lets staff know any risk the resident might face and what they can do to reduce any risk and in turn protect them. Since the last inspection the home has made sure that these risk assessments have been signed and dated, this means that it is clearer to everyone when a review of this

Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 10 & 11 The systems and approach at the home in caring for residents at the time of their death is sensitive and supportive and continues to respect their wishes and feelings at this time. EVIDENCE: Risk assessments were examined within the care records of the residents. A previous requirement made at the inspection in September 2005 was noted to have been met as risk assessments had now been signed by the author and dated so a review date was clearer. Within the care records seen entries of the wishes of residents at the time of their death had been included. Information on their religion, who should be contacted and personal information was seen to be in place. A policy was examined to guide and direct staff in how they could support residents and their families at this time, guidance was noted to be clear and comprehensive. One resident complained that not all staff knocked on her bedroom door before entering and felt that she would like more privacy, other residents spoken with reported that staff did knock on their doors prior to entering. This was discussed with the senior care assistant at the time of inspection.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Dietary needs of residents are well catered for with a balanced and varied selection of food that meets resident’s tastes and choices. EVIDENCE: Menus were on display within the home; these showed that a varied and balanced diet including all the main food groups was on offer. Observations of lunch showed resident’s had been given a choice in their main meal, staff were seen to assist those who required help to eat their meals. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home has a satisfactory complaints system in place with some evidence that resident’s feel that their views are listened to and acted upon. EVIDENCE: The homes complaints procedure gave sufficient guidance to staff on the action that they should take on receiving a complaint. The procedure described the rights of residents and that all complaints must be responded to. Within the homes statement of purpose a summary of how to complain had been included for residents and their relatives. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 Overall the appearance and standard of décor in the home is good in this home creating a spacious and comfortable environment for those living and visiting there. EVIDENCE: Accommodation available to residents is across three floors. The furnishings, fittings and décor in these areas are of a very good standard. All residents who were seen spoke highly of their environment and felt that the home catered very well for their needs. A large rear garden provides pleasant views with spaces available to for sitting in the warmer months. Individual rooms contained personal items of the resident to assist in creating a homely atmosphere. All areas visited were noted to clean, tidy and free of odours. Staff were observed to wear suitable protective clothing when carrying out certain activities. Cleaning schedules were in place and clinical waste was disposed of in an appropriate manner.
