This inspection was carried out on 16th June 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

All prospective residents` needs are assessed. They are able to visit the home prior to admission and the admission proceeds at a pace that suits the individual. All residents have a care plan that is designed using the information in the assessment. Residents are able to make choices and decisions about their lives and any potential risk us assessed and taken into account. Residents are offered a range of activities and are able to exercise choice over what they do. They spoke very positively about the activities. The food is described as very good. Residents receive personal and healthcare support that meets their needs; their medication is stored, administered and recorded appropriately. Residents are aware of how to complain and are confident enough to do so. They know their concerns will be listened to. The premises are of a suitable size and layout to meet residents` needs and are clean, tidy and in good decorative repair. A team of well trained and competent staff meets residents` needs.

The home continues to provide a very high standard of service as indicated in earlier reports.

A total of 22 service users from the age of 18 - 65 years in the category of LD may be accommodated in the home. 2 of these 22 may be named service users who are over 65 years. No one may be admitted to the home in the category of LD when there are already 22 service users in this category in the home. No further service users over 65 may be accommodated in the home. No service users other than the one named may be admitted in the category of MD The ground floor bedroom to be used only for existing service users who become ill and are unable to mount the stairs. In the unlikely event that the service user requires this room in excess of twenty eight (28) days the Commission for Social Care must be notified. Date of last inspection 20.12.04 Brief Description of the Service: Solden Hill house is situated in a rural location, just outside the village of Byfield to the southwest of Northamptonshire. The home has its own transport and service users can access the village shopping facilities, as well as the facilities of the nearby towns of Daventry and Banbury. The home is one of two owned by Solden Hill house Ltd and is managed by Mrs A. OHare. The home provides personal care and support for 21 service users with learning disabilities. The premises are set in their own grounds and split into two selfcontained houses, Solden and Apps house. Each house provides a communal lounge and separate dining room. Both houses have kitchen facilities. Solden Hill house has a workshop and Day Centre accommodation and the large communal dining area, used by all the service users attending the day centre facilities during weekdays. Solden Hill also contains office accommodation for the administrative section of the organisation. This was the first statutory unannounced inspection this year and took place over approximately three hours. Three residents’ were Case Tracked. The inspector looked at all three files and saw two of their rooms. The two residents gave the inspector a guided tour of the communal areas as well. Comments received from the residents were very positive. The home was described as ‘lovely’ and ‘very nice’. Staff were referred to as ‘very helpful’ and the food was described as ‘smashing’. Both residents stated that they have a choice of food and have been able to personalise their rooms. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 4 Residents’ needs are assessed and they are able to ‘test drive’ the home prior to admission ensuring that the home is the right one for them. EVIDENCE: All files tracked contained very useful assessments. Those residents who have recently moved to the home had community care assessments. The registered manager stated that she or her deputy always carry out an additional assessment at the point of referral. New residents are then introduced to the home at a pace that suits them. A visit is made by senior staff to the referral’s own home to discuss the admission process. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 9 Residents’ needs are reflected in their care plans, they are enabled to make choices and decisions and any risk involved is assessed. EVIDENCE: All files tracked contained very useful care plans. These were clear and concise and would enable new staff to quickly gain an awareness of residents’ needs. Residents who spoke with the inspector stated that they had contributed to their care plans and there were documents on file that they had signed. The care plans included headings such as social history, health, communication and behaviour. Each file contained a document, signed by the resident, that indicated that decision and choice making had been discussed with them. This was confirmed in conversation with residents who stated that they are able to make choices in many areas of daily life. All files tracked contained very useful risk assessments that were both generic and specific to activities such as horse riding. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 17 Residents are able to take part in suitable activities and feel that they are part of the community. They are able to maintain contact with families and friends and are offered a varied, nutritious diet. EVIDENCE: All files tracked contained a wealth of information about residents’ activities. All residents attend day activities of one sort or another. Those tracked do gym, horse riding and college. Two of the residents tracked are members of a local dance group and have performed at a number of events. The inspector was shown a video of the dancers in action as well as a gymnastics group she attends. Both residents who spoke with the inspector were very positive about their activities. All residents are enabled to be part of the local community through their various activities. All files tracked indicated that residents are able to maintain contact with family and friends; those that spoke with the inspector stated that they are able to receive visitors at any reasonable time and that they are able to visit in private. All files tracked contained records of residents’ dietary needs including any special requirements. Those who spoke with the inspector confirmed that the food is very good. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20 Residents receive personal and healthcare support that meets their needs and are protected by medication procedures. EVIDENCE: Residents’ personal and healthcare needs are recorded comprehensively in their files. They are all registered with a GP and any appointments are recorded. Those residents who spoke with the inspector stated that the staff look after them well. The inspector examined medication processes and found that it is stored, administered and recorded appropriately. Medication was also discussed with staff. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 Residents feel that their views are listened to and acted upon. They therefore have the confidence to complain. EVIDENCE: The home has a comprehensive complaints procedure. All residents who spoke with the inspector stated that they are aware of this and would be confident to speak with a senior member of staff should they ever need to complain. The inspector was shown a folder in which all complaints are recorded. A number of minor complaints had been recorded all of which had been dealt with within relevant timescales. JUDGEMENT – we looked at outcomes for standard(s) 24, 30 Residents live in a comfortable and homely environment that is kept clean and tidy. EVIDENCE: The premises are very well appointed. Residents’ bedrooms are pleasantly decorated and personalised to a high standard. Both residents who showed their rooms to the inspector were clearly proud of them. The rooms contained personal possessions and pictures and the residents confirmed that they had chosen the décor. Both houses also have comfortable lounge and dining rooms. The home was clean and tidy on the day of the inspection. JUDGEMENT – we looked at outcomes for standard(s) 35 Staff are well trained and are therefore better able to meet residents’ needs. EVIDENCE: The manager showed the inspector a staff training schedule that indicated a comprehensive programme of staff development. The inspector spoke with a member of staff and discussed medication training. Staff have also attended training regarding dementia, manual handling, food hygiene, epilepsy and abuse.
