No service users other than the one named may be admitted in the category of MD The ground floor bedroom to be used only for existing service users who become ill and are unable to mount the stairs. In the unlikely event that the service user requires this room in excess of twenty eight (28) days the Commission for Social Care must be notified. 16/06/05 Date of last inspection Brief Description of the Service: Solden Hill house is situated in a rural location, just outside the village of Byfield to the southwest of Northamptonshire. The Village is accessible by public transport, although visitors may find the journey difficult, as public transport is infrequent. The home has its own transport, and service users can access the village facilities, as well as the facilities of the nearby towns of Daventry and Banbury. The home is one of two owned by Solden Hill house Ltd and is managed by Mrs A. OHare. The home provides personal care and support for 21 residents with learning disabilities. The premises are set in their own grounds and split into two self-contained houses, Solden and Apps house. Each house provides a communal lounge and a separate dining room. Both houses have kitchen facilities. Solden Hill house has a workshop and day centre accommodation including a large communal area, used as a dining area for all the residents attending the day centre facilities during weekdays, and for other activities during the evening and at weekends. There are extensive well-maintained and attractive grounds that are fully accessible to residents. Solden Hill also contains office accommodation for the administrative section of

This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission of Social Care Inspection is upon the outcomes for Residents, and upon their views of the service provided. The primary method of inspection used was ‘case tracking’ which involved selecting 2 residents and tracking the care they receive through review of their records, discussions with them, and with the care staff, and observations of care practices. The inspection took place during a weekday over a period of 4 hours and was carried out on an unannounced basis. Communal areas, and some bedrooms were visited. A selection of care records, and essential records of the home were reviewed. A number of the residents were spoken to as part of the inspection process. The Registered Manager Ms Averil O’Hare, was available throughout the inspection. The Commission had received a number of comment cards from relatives prior to the inspection. The comments recorded were very positive and praised the home and the staff for the way in which the care is provided. What the service does well:
The home offers a friendly warm environment in which the residents and the staff are very friendly and welcoming. Residents are encouraged to be as independent as possible and to maintain links with relatives, friends and the community. The home was decorated to a reasonable standard and the residents’ bedrooms that were inspected were personalised containing, personal items. There is a good staff team who work well together. They are committed to undertake regular training and continue to reflect on the systems of the home to ensure that things work well and are in the best interest of the residents. Staff undertake specific responsibilities within the home, and spoke knowledgably and enthusiastically about the tasks undertaken.

Residents have appetising and healthy food; the mid-day meal on the day of the inspection was enjoyed by all, and eaten with the staff as a family meal. All prospective residents are assessed by the Registered Manager, and the admission process is taken at a pace suitable for the resident and their families. The care planning system gives a comprehensive picture of each person. A wide range of activities are provided in the home and in the day centre and residents choose what activities they wish to participate in. The programme of activities is flexible and can be altered to suit resident’s choice. What has improved since the last inspection? What they could do better:
Evidence should be provided to demonstrate that care plans are updated at monthly reviews, and agreed with the residents. The present quality assurance system should be developed to include relatives and other stakeholders’ views, and linking the results with other monitoring systems to the annual development plan. This home continues to provide a service where the priority is to ensure that residents lead an independent life as possible, and reach their full potential. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,4,5. The homes information including the Statement of Purpose / Service User guide is well written and tailored to meet the needs of residents/ families/ friends, and health care professionals. EVIDENCE: Considerable effort has been made to make the homes Statement of Purpose, and Service User Guide accessible to people with a range of abilities. The Registered Manager undertakes the admission process, visiting the prospective resident at home to begin the assessment of need. There was evidence that residents who are placed by Care Managers are also assessed and a copy of that assessment is received by the home. The Registered Manager also contacts other professionals to contribute their assessments to the process. All of the residents interviewed expressed great satisfaction and confidence that the home can meet their needs and that the staff support them to meet their ambitions. New residents are introduced to the home at a pace that suits them, this would include visits for meals and overnight stays. A contract is in place for each resident. This agreement clearly sets out the terms and conditions of occupancy, and had been signed by the resident. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7,9. Resident’s rights are respected and they are encouraged to be involved in decisions about the home, and in their own lives within the home. EVIDENCE: Particularly comprehensive care plans are developed for each resident. Each plan examined was seen to be individually constructed and to include a wide range of daily living activity related issues. A running record of significant events is maintained in a daily notes. Each plan was seen to contain detailed notes of instruction to staff to guide interventions and to support the residents to attain their goals. The Registered Manager said that regular reviews of the care plan take place, and that parents are invited to the annual review. Advice was given that records are maintained of the review process, detailing dates, signatures of the worker and where possible evidence that the resident has been involved. The Registered Manager said that an advocate is not used in the home currently but that if an advocate was required assistance would be given to a resident to access an advocate service. It was evident from speaking with a resident that he was able to make decisions about his live and that support is available to help him.

Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,15,16,17. Opportunities for personal development are provided through a broad range of community based leisure and educational activities. EVIDENCE: The Registered Manager confirmed that the home has a positive relationship with the local community. Local amenities are available and used by residents, such as the local supermarket, restaurants, and pubs. The staff are able to give equal support to residents both within and outside of the home, by working to a flexible staff rota, which revolves around residents social needs. There was evidence of daily programmes for each resident involving college placements gym, swimming, horse riding or craft activities provided in the day centre next to the home. During the morning of the inspection, residents were seen to be involved in weaving, painting, or talking over a cup of coffee, whilst others were at college. Items made in the craft centre were displayed around the home. There is one large community room in the home, with a curtained area for a stage, which can be used as a games room, or for musical presentations. Residents spoken to were very positive about the variety and choice of activities provided by the home.

Residents confirmed that they go home for weekend stays on a regular basis. The Registered Manager demonstrated her awareness of resident’s rights to intimate relationships, protection and specialist information in this area. There was evidence from the menu submitted to the Commission of Social Care Inspection that residents are provided with healthy nutritious food. Residents were positive about the quality and choice of food. The menu is decided following consultation with the residents, and on the day of the inspection the midday meal consisted of cauliflower cheese, potatoes and vegetables followed by fresh fruit. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20. The health needs of residents are well met with evidence of good multi disciplinary working taking place. EVIDENCE: The care plans contain detailed information about the health needs of each individual. There is evidence that advice and input is sought from relevant specialists and these have included psychiatry, speech and occupational therapists, chiropodist, dentist and community psychiatric nurse input. The staff responsible for medication described the medication policy and procedures, which were found to be satisfactory. The residents have a weekly cassette dispensed by the local pharmacy. Medication is checked into the home and recorded by two members of staff. Medication is stored safely in a locked cupboard. Staff administering the medication have received appropriate training, and were aware of the homes policy if medication was inappropriately administered. The Administration records were generally in good order although advice was given that staff record an appropriate code, if prescribed medication was not given. A disposal book was used to record all medication returned to the pharmacist.

The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 Staff have knowledge and understanding of adult protection issues, which protects residents from harm. EVIDENCE: The home has a Protection of Vulnerable Adults (POVA) policy and the Registered Manager was aware of the Northamptonshire inter agency procedures. The commission of social care inspection have received appropriate notification of any incidents that have occurred. Staff spoken to during this visit were able to describe the appropriate actions they would take if suspecting any sort of abuse had occurred. The Registered Manager said that all staff have received protection of vulnerable adult training. Staff do not carry out unsupervised tasks with residents until they receive their Criminal Records Bureau clearance however the home was able to demonstrate that POVA first checks had been obtained. All staff have been checked against the official list of people not able to work with vulnerable adults, including volunteers from Germany working in the home. The Registered Manager explained and demonstrated the homes policies and practices regarding resident’s personal finances, which were found to be satisfactory. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 The standard of the décor within the home is reasonably good, and the standard of hygiene was high, so that the residents have a homely place in which to live. EVIDENCE: All communal areas and one bedroom were viewed and all were maintained and decorated to a reasonable standard. There are a range of communal areas that are used by residents, all with comfortable seating and good quality furniture. Examples of residents work, completed in the craft areas are displayed around the home. One of the residents was pleased to show his room during the inspection, where personal possessions were observed. The room was found to be pleasantly decorated and it was observed that there was a high standard of cleanliness. Locks can be fitted onto bedroom doors, and keys issued based on individual choice and risk assessments. There is a large enclosed garden that is safe and accessible to residents. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34 The arrangements for the recruitment and induction of staff are satisfactory with staff demonstrating a clear understanding of their roles. EVIDENCE: The recruitment process was discussed with the Registered Manager. There is a robust system in place to ensure that appointments are made on the basis of equal opportunity legislation, and satisfactory checks are completed prior to appointment. A staff file was examined and seen to evidence a thorough recruitment process with appropriate references and clearances. There was a comprehensive induction plan for new staff. The Registered Manager demonstrates a commitment to providing appropriate training for the staff team. Training facilitated, since the last inspection includes: Infection control, Basic food hygiene, Care of Medicines, Health and Safety, Fire Awareness, Breakaway techniques and First Aid. Staff, of both genders, are employed to meet residents personal preferences in the provision of personal care. The arrangements for on-call support and management oversight of the home are appropriate, staff clearly knew what was expected of them while on duty,

Staff commented that staffing levels were adequate. This was further supported from relative’s comments within pre-inspection comment cards. The two staff spoken to outlined the training they have received since joining the home, and this supported the documented details passed to the Commission for Social Care Inspection by the Registered Manager. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42. There is sound leadership; guidance and direction to staff from the Registered Manager that ensures the residents receive consistent quality care. EVIDENCE: The Registered Manager has a wealth of experience in caring for people with learning disabilities, and discussions with staff indicated that the manager was visible and very much a part of the team, offering support and guidance. The Registered Manager is in the process of taking the Registered Managers award; every effort should now be made to complete this award. There was evidence that questionnaires had been completed with the residents, however the results of this survey had not been published or indeed used to influence the planning of services provided in the home. Relatives, friends, or stakeholders have not yet been approached for their views of the service. Solden Hill House DS0000012913.V277146.R01.S.doc Version 5.1 Page 23 The fire logbook demonstrated that fire detectors were tested weekly, and that fire drills were held on a regular basis. There was evidence of in-house fire training for staff. Records of tests to fire safety equipment were in good order and health and safety issues were well documented. The accident book was satisfactory. There was evidence that the hot water temperatures are monitored and recorded weekly. The Registered Manager supplied evidence that Gas, heating, and electrical equipment received regular maintenance checks. Staff spoken to were aware of their responsibilities in relation to Health and Safety, and said that their training was regularly updated.
