The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home provides comfortable and homely accommodation to residents. The premises are generally well maintained and bedrooms are personalised to the individual. The registered manager and her staff demonstrate a full knowledge of residents needs and were observed to interact positively with individuals on the day of inspection. Relatives and friends of residents are welcomed and encouraged to visit the home.

What has improved since the last inspection?

The physical environment presented well during this inspection and all areas of the home were found to clean and free from offensive odour. Thermostatic valves have been fitted to all hot water outlets thereby further ensuring the safety of residents. Fire Safety records are additionally now fully maintained. Further policies and procedures have been put in place and risk assessment documentation has been implemented as required.

What the care home could do better:

Improvements must be made with regard to the care planning systems in place. These must be re-organised and a consistent format used that fully addresses the needs of individual residents. The carpet in the main hallways of the home presents badly in comparison to other areas. The replacement of the lounge carpet should also be considered in the short-term. The training provision at the home requires improvement. It is essential that all care staff attend training with regard to abuse awareness, dementia, food hygiene and Health & Safety. The registered provider must ensure that appropriate resources are made available to ensure these courses are provided.

The home is privately owned with a manager in day to day charge. The service is situated in a residential area of Wimbledon, near the town centre. The location is close to shops, leisure centres, churches of different denominations and good public transport links. Accommodation is provided over two floors serviced by a lift. The home is staffed twenty four hours a day with additional dedicated catering and domestic staff provided. This was an unannounced inspection that took place over one day. The inspection consisted of the examination of records, a tour of the premises and individual conversation with residents, staff and the registered manager. The inspector had the opportunity to speak with five residents and two members of staff. The term ‘residents’ is used throughout completed sections of this report instead of ‘service users’. This was agreed as the preferred terminology to be used at the time of inspection. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3 and 5. The homes Statement of Purpose and Service User Guide require further review to ensure sufficient and accurate information is provided. Written statements of terms and conditions are provided within the Service User Guide for the home. The fees charged are not consistently specified within this document. The needs of residents are assessed prior to moving into the home. Prospective residents are given an opportunity to visit the home in order to assess the quality, facilities and suitability of the home. EVIDENCE: The Statement of Purpose in place requires review to ensure the accuracy of the information provided. The document examined stated that staff were all qualified in their own field of work with NVQ Level Two. This was not the case at the time of this inspection visit. Further amendments were seen to be required in order to specify that telephones ‘can’ be provided in individual South Park Residential Home G54-G04 S27229 South Park V222986 250405 Stage 4.doc Version 1.30 Page 9 bedrooms and the replacement of references to ‘mental health problems’ with the term dementia. The Service User Guide additionally requires update to reference where a copy of the last inspection report can be accessed. Two instances were noted where the Service User Guide supplied to individual residents did not have the fee payable specified. This figure must be inserted at the time of issue. Both documents should be updated to ensure the CSCI is referenced accurately throughout. Assessments were in place for three residents whose care documentation was examined. A prospective service user was seen to be making a day visit to the home on the day of inspection. The registered manager was observed to additionally supply good information to a visitor to the home with regard to the service provided. This included a tour of the premises. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 9, 10 and 11. The care plan documentation in place requires full review and re-organisation in order to utilise these as effective working tools. Medication records are well maintained and kept up to date. Items of medication are securely and appropriately stored. The organisational procedures in place require further review. Information concerning individual wishes following death are not consistently obtained and recorded. EVIDENCE: Care plans were observed to be in place for three residents whose documentation was examined. All were seen to be subject to a monthly review process but the inspector noted that differing formats were used for care planning at the home. The presentation of information was poor with documents stored in individual box files with no indexing for reference purposes. Assessments were observed to be in place regarding areas such as nutrition and pressure areas. These were not consistently evidenced within all the care files and some were observed not to be dated. Medication administration records were well maintained at the time of inspection. Individual items of medication were observed to be securely and appropriately stored. The organisational procedures in place were seen to require review to ensure they are consistent with guidelines issued from the Royal Pharmaceutical Society. Feedback obtained from residents was generally positive regarding the quality of care provided. One newly admitted resident stated that it was ‘a lovely place’ and a visiting relative of a resident reported that the home fully involved him in decisions concerning care. Individual issues raised included the availability of telephones and accessing library facilities were raised with the registered manager on the day of inspection. The inspector was satisfied that these had been addressed by the home and that appropriate liaison had taken place with relatives of individual residents. Information concerning individual wishes following death was not consistently recorded within the care plans examined. The home must ensure that information is obtained and recorded – where a resident declines to discuss these details, this should be documented as such. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 15. The activity provision at the home is adequate. Further work is required to ensure that individual needs are fully addressed within care plan documentation and is then subject to an ongoing evaluation process. Residents are able to receive visitors and are assisted to maintain contact with family / friends / representatives by the home. The dietary needs of residents are well catered for with a balanced and varied menu available. EVIDENCE: Activities are provided both in the morning and in the afternoon. This provision was seen to include cards, puzzles and memory exercises on the day of inspection. Television, video and DVD equipment is provided in the main lounge area along with books and videos. Minutes of two recent resident meetings both contained requests by residents for a larger television in the lounge area. This has been included as a Requirement within this report. One member of staff reported that they escorted two residents out to the park during the last week and that a trip had been arranged to visit a country pub within the next week. The record in place confirmed that the majority of residents received very regular visits from family or friends. One relative of a resident was spoken to and they reported that they visited every day and felt able to drop in at any time without prior notice. A main meal is served for lunch every day and this is displayed on a whiteboard within the dining area of the home. A four-week revolving menu is operated at the home and this has been revised since the previous inspection visit. The menu does not include a list of alternatives but an alternative meal can be provided according to individual wishes. A meal of liver, bacon, potatoes and other vegetables was served on the day of inspection. Tea and biscuits were also seen to be provided at around 2pm. Feedback from residents was positive regarding the food provided. Comments included ‘good’ and ‘very nice’. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18. A satisfactory complaints procedure is in place and is included within the issued Service User Guide. Systems for the recording and monitoring of complaints are in place. EVIDENCE: A record of complaints was seen to be maintained at the time of inspection. The registered manager stated that no complaints had been received by the home since the last inspection visit. A visiting relative of a resident stated that they felt able to discuss any concerns with the registered manager as required. Organisational abuse procedures were in place at the time of inspection. Training is still required for all care staff working at the service and the Requirement made at the previous inspection has been re-stated. The home should ensure that an up to date version of the Local Authority Protection of Vulnerable Adults (POVA) procedure is obtained for reference purposes. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 25 and 26. The home provides comfortable and homely accommodation to residents. Further replacement of communal carpeting must take place to further enhance the appearance of the home. The premises are maintained to a good standard of cleanliness and kept free from offensive odour. Bathroom facilities are sufficient to meet the needs of residents currently accommodated. Resident bedrooms were seen to be well maintained and personalised to the individual. EVIDENCE: The home was seen to be kept in a good decorative state on the day of inspection. The carpeting in the ground floor hallway, stairwell and first floor hallway requires replacement as it presents very badly compared to other South Park Residential Home G54-G04 S27229 South Park V222986 250405 Stage 4.doc Version 1.30 Page 16 areas of the home. The registered persons should additionally consider replacing the carpet in the lounge / dining area in the short-term. The main lounge / dining room provides comfortable and accessible communal space for residents. The lounge seating has been replaced since the previous inspection. Individual bedrooms present very well and provide comfortable homely accommodation for residents. All were observed to be kept very clean and free from offensive odour. The bathroom / toilet facilities were observed to be in good decorative order and are sufficient in number to meet current resident needs. Aids and adaptations provided include bath chair equipment – the registered manager stated that no other equipment was currently required but would be obtained as necessary. Two minor shortfalls were noted with regard to the premises. A conservatory area is provided that is utilised as the main laundry for the service. The roof was seen to require further attention in order to provide adequate shading for staff or residents on warmer days. A damaged electrical socket located in the first floor hallway also requires replacement. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30. Staffing numbers are adequate to meet the needs of the residents currently accommodated. The training provision for care staff must be further developed and improved. EVIDENCE: Staffing levels remain consistent with those in place at the previous inspection visit. There are always two members of care staff on duty at the home. These levels were assessed as being sufficient to meet the current needs of residents and two members of staff spoken to confirmed this. Feedback from residents included ‘the staff are alright’ and ‘lovely’. The 2005 training plan for the home was seen to include Health & Safety, abuse awareness and dementia. The registered provider must ensure that resources are now provided to deliver this programme. The registered manager stated that medication and Food Hygiene training had been provided for staff and records examined confirmed this. The home must however ensure that any in-house training provided is supported by documentation as to the content of individual sessions. This was not in evidence with regard to the medication training given. The registered manager stated that four staff were currently studying for their Level Two qualification with a further individual staff member working towards the Level Three award. The registered manager reported that she was studying to attain the NVQ level Four qualification. The induction and foundation training materials for new care staff require review to ensure that these are consistent with the national Skills for Care standards. All new staff must be checked against the POVA list prior to commencing employment at the home. This had not taken place for one member of staff as identified by the registered manager. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 33, 34, 36 and 38. Further equipment / resources should be supplied to the manager in order to further improve administrative systems and training availability. The systems in place for consultation with service users and their representatives require further development. Accurate records of residents money must be consistently maintained with suitable secure storage provided. This was not fully evidenced at the time of this inspection. An adequate system for regular 1-1 supervision with individual staff members is in place at the home. Fire Safety records are now suitably maintained. Fuller records are required with regard to hot water temperatures and electrical safety. Care staff spoken to reported that they felt ‘looked after’ and ‘supported’ by the manager. The registered provider should consider the provision of computer and printer equipment for the home in order to enhance current administrative systems. Further resources for training must also be provided. Resident meetings are held on a monthly basis and minutes were seen to include discussion regarding food, activities and equipment. A quality assurance system must be fully implemented at the home to include annual questionnaires for residents, their relatives / representatives and other stakeholders. The information obtained should then be used to inform the annual development plan for the home. Monthly unannounced visits by the registered provider are taking place. These reports must now be supplied to the CSCI. A system for staff supervision is in place at the home. Two members of staff spoken to stated that they received regular 1-1 supervision with their line manager. The home holds personal monies for four residents. Records of transactions were seen to be maintained but one of these was observed to be inaccurate at the time of inspection. The registered persons must additionally review the security arrangements due to the cash amounts held for individuals. Fire Safety records were observed to be consistently maintained. The hot water temperature checks are carried out on a weekly basis – mixer valves have now been fitted to all outlets to regulate temperature. The record maintained must be amended to include a specific temperature reading especially with regard to bath and shower outlets. Portable electrical appliances have been tested. Documentation was not available with regard to the five yearly test of the fixed electrical installation at the home. A cupboard containing cleaning materials was observed to be left open on the first floor of the home. The home must ensure that all potentially hazardous substances such as bleach are kept securely at all times.
