Conditions of registration: NONE Date of last inspection Brief Description of the Service: Spring lodge is situated in the small village of Woolverstone about five miles from Ipswich. The home is a registered care home for 27 people.The accommodation is situated on two floors. The majority of the bedrooms are on the ground floor and provide single accommodation. Each bedroom has a wash hand basin and two rooms (one double and one single) have a WC en-suite. Bathrooms are situated on both floors. Communal areas are all on the ground floor and include a main lounge and three other areas. There is also a central dining room.The home is situated within its own grounds with parking for visitors. There is a large, attractive and enclosed garden at the rear of the property, which is well maintained and provides a variety of seating areas for service users. This is an overview of what the inspector found during the inspection. This was an announced inspection. The previous inspection which was unannounced took place on 15 December 2004. At this inspection, the owner, manager,and deputy manager were present for all or part of the inspection. The inspector also spoke to a number of staff either separately or in the course of their duties. Residents were also spoken to, and were very happy to talk about the home and themselves. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5. 6 is not relevant to this home. Prospective residents can be assured that there is sufficient information about the home together with visiting opportunities to enable them and their family to make an informed choice. No one is admitted without an assessment of their care needs to ensure that the home can provide for them. EVIDENCE: The home had updated its statement of purpose and service users guide to reflect that its registration category had been amended to allow it to care for up to 7 persons with a diagnosis of dementia. 2 recent admissions were tracked. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10,11 Residents’ health is assessed, reviewed regularly, and monitored to make sure that emerging as well as continuing needs are being met, both by the home and by health professionals. EVIDENCE: Care plans inspected were up-to-date and informative. A resident had recently been admitted to hospital with suspected scabies. However this had still not been confirmed. The home had consulted the appropriate specialists and was taking the advised precautions. One resident had developed a pressure area after a stay in hospital. This was being treated successfully by the district nurse. Drug administration procedures had been improved following the last inspection, to ensure that there was always a carer trained to undertake drug rounds on duty. These arrangements included the manager and deputy manager when necessary. A new drug room was being created from the previous staff room. This would improve security and bring a number of documents together. A medication review for all residents was carried out every 2 months with the GP. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15 A variety of communal areas give residents choice of where to go when they leave their rooms according to their preference for access to company. This also allows for activities to take place without disturbing those who do not wish to take part. Residents are able to choose where to take the meals of the day. The catering service is responsive to residents’ wishes, and provides a balanced diet. EVIDENCE: Care plans listed the preferences of residents for daily routines, including the time of getting up and going to bed, type of food, and activities. One of the senior carers had responsibility for the development of activities. There was currently a good variety but more were planned to cater for residents’ preferences, but also to stimulate them to try new things. The chef was trained and had a well-equipped kitchen. Although he was knowledgeable about residents’ likes and dislikes, he stated that he occasionally tried to tempt them with new dishes which they might not normally choose. JUDGEMENT – we looked at outcomes for standard(s) 16 Residents and relatives have any concerns dealt with at the time they are raised, which prevents complaints arising. EVIDENCE: There had only been one complaint received by the Commission in the last 12 months. Some aspects had been partly upheld and action had been taken to remedy the deficiencies. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23,24,25,26 The home provides a safe and comfortable individual and communal environment for residents. EVIDENCE: The replacement of the radiators required from the last inspection was in hand. The work had been delayed until the weather was warmer as the home would be without heating for part of the contract. The corridors would then be redecorated. Where necessary, devices had been fitted to the doors of residents’ rooms to ensure that they closed if the fire alarm sounded. The temperature of the hot water outlets was tested weekly. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30 Although residents were protected by the home’s commendable training and supervision programmes, there had been two occasions when recruitment practices had not been followed. This could have residents at risk. EVIDENCE: Two new staff had begun employment before the home had received the POVA check. In one case this was 6 days later, but in the other case it was 6 weeks later. It has been made a requirement that this must not happen again. Staffing numbers were assessed to be adequate for the needs of the residents. Those with dementia were only exhibiting mild signs of the condition at the moment. The regular reviews of care needs will pick up changing staffing requirements. No agency staff were used. A member of staff commented that “all the staff are dedicated, and all will help out to cover shifts”. The home had a commendable programme of training and supervision under the deputy manager. It was recommended that an annual forward programme of training should be prepared for budgeting and monitoring purposes. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,37,38 The home has a owner who visits the home weekly, and a strong management structure with regular reporting, to ensure that standards are maintained for the protection and confidence of the residents. EVIDENCE: The Home had an excellent training programme. Individual members of staff expressed their satisfaction with the availability of internal and external training opportunities. Evidence from the staff files confirmed this. The system of supervision was very comprehensive, covering care practice audits and training needs analysis with regular reviews of competence. The home had been complimented by the Fire Officer on the quality of their Fire Risk Assessment. The owner received weekly reports from the manager , the deputy manager and the senior carer on all aspects of the home.

The supervision system has been commended under Staffing. The ethos of the home was clear from the support of staff for their colleagues and the residents.
