This inspection was carried out on 15th February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

One service user said that "we are just one big family here" and that appears to be the sentiments of all service users. Service users said that they were happy and well looked after. The manager ensures that they are fully involved in family life and are part of the local community. The registered person continues to be the main carer and can demonstrate that she has a very good understanding of service users individual needs. There is a homely and welcoming atmosphere and the ground floor accommodation, which is used by service users, is very well maintained. Records are kept to a satisfactory standard.

What has improved since the last inspection?

The home continues to maintain a good level of care. There were no requirements or recommendations arising from the last report.

What the care home could do better:

Portable appliances need to be tested on an annual basis. Record keeping for service user`s monies needs to be amended to show the reasons for withdrawals.

The home is registered for older people and for those with mental health needs and is more suitable for people with low dependency needs. The house is detached with good sized and accessible gardens. Each service user has a single bedroom, which are comfortably furnished, these all have patio doors leading out into the rear garden. Residents are appropriately supported by the local community health team and the small staff group who provide day-to-day care. Residents make good use of local facilities and day services and are thus encouraged to retain links with the local community. Residents are also encouraged to retain positive links with family members. This was an unannounced inspection of the home taking place over a two-hour period. The inspector spoke with the registered person and all service users. Records were examined and some areas of the home were visited. The inspector would like to thank service users and staff for their hospitality and assistance given during this inspection visit. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): These standards were addressed in a satisfactory manner at the last inspection. EVIDENCE: There has been no changes to the service user group since the last inspection therefore these standards were not inspected. Service users are settled and get on well with one another. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Personal and healthcare needs are met in a satisfactory manner. EVIDENCE: All care plan records were examined. These were up to date having been reviewed regularly. Service users continue to receive support from the community mental health team and other healthcare professionals. Service users had been to visit the dentist recently. One said he preferred not to go. One service user said that he was awaiting admission to hospital and was on the waiting list. Another said that he had started using a Zimmer frame and was now able to walk much further and this had helped him “lose a bit of weight”. Regular weight checks were recorded in individual care plans. The records of receipt, administration and disposal of medication were satisfactory. Medication is stored safely in the registered person’s accommodation on the first floor. One service user was seen to have had recently had a review of his medication on his personal record. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15 Good arrangements are in place for service users to pursue their own interests. EVIDENCE: Two service users had just returned from a lunch out before the inspection visit. The other had been for lunch but had taken himself out for a walk. Examination of his daily notes showed that he does this regularly to maintain his fitness. Service users said that they go out regularly for meals and go to a local resource centre twice a week where they join in a variety of activities. Service users said that they have regular contact with their families. From discussion with service users it was heard that they are able to decide how they would like to spend their day. One likes to go to church now and again. Another likes to watch sport on his own television. A lot of photographs of holidays and other outings showed that service users are involved and that they enjoy outdoor activities. Service users said that they were looking forward to using it for a holiday but were undecided where to go. Service users said that the food was good. A record was seen to be kept of the chosen daily menu. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 There are satisfactory arrangements in place for dealing with complaints and adult abuse although this would be strengthened when staff have received training on adult abuse and protection. EVIDENCE: A complaints policy is in place. There has been no complaint since the last inspection visit. Service users said that they had nothing to complain about as they had a good life. The manager and the support worker were awaiting a date to attend the local authority adult abuse training at the last inspection. The manager said they are still waiting for a new date. The manager is aware of needing to inform relevant parties should an allegation of abuse be made. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 24 & 26 The home is well maintained, clean and comfortable and is to the liking of the service users living there. EVIDENCE: Most areas of the home were visited and were found to be clean, tidy and well maintained. No changes had been made to the home since the last visit as most areas had been recently redecorated then. One service user showed the inspector his new furniture and television. He said that the provider made sure his bedroom always looked nice. He was pleased that he could go out of his room via French doors into the garden. Service users were seen to have many personal possessions in their room reflecting that their interests and choice was taken into consideration. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 & 30 Satisfactory staffing arrangements are in place to ensure the safety and wellbeing of service users. EVIDENCE: The manager said that there has been no change to the staff team since the last inspection. Both she and one staff member are nearing completion of their infection control training. This had been delayed as the trainer had left but the manager has been told that a new trainer has been appointed and she hopes that this course ends soon. No staff apart from the provider was on duty during this visit. Staff personnel and training records were not examined as these were seen to be satisfactory at the last inspection. The manager said that she is supporting her member of care staff through NVQ Level 2. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 37 & 38 Health and safety arrangements are generally good although portable appliance testing needs to be updated. Record keeping is good and will be further enhanced by clarifying those for service user’s monies to fully safeguard service users and staff. The home is well managed with the full involvement of service users who are satisfied with the service provided to them. EVIDENCE: The manager has completed the Registered Managers Award. Although there are no formal arrangements to meet with service users it was observed, and service users confirmed in discussion, that they are consulted about life in the home and how it affects them. There is a very family Springfields DS0000006252.V283972.R01.S.doc Version 5.1 Page 15 orientated feel to the home. The manager knows service users very well since they live together. The records for service user’s finances were seen. The provider and service users were able to explain fully why withdrawals were made from their own account for personal spending but from examining records this needs to be more clearly accounted for. One service user said that he pays his own way and has a savings account. The provider keeps copies of all bank statements for individual service users. The records for the fridge, freezer, food and water temperatures were well maintained. Gas appliances were last serviced in April 2005. Smoke alarms are tested regularly and a record made of the outcome. The manager said that she carries out a routine health and safety check on a monthly basis. The portable appliance test was out of date. The manager said that she would ensure this was dealt with as soon as possible.
