What the care home does well

Not enough of the outcome areas were looked at to comment on what the service does well on this occasion.

What the care home could do better:

Privacy and dignity had not be respected for the three people being cared for in a room with two beds and a mattress on the floor. The room was designed to cater for two service users. No screening had been provided and a female service user was in the room visiting one of the three people in the room while the other two male service users were asleep in bed. This practice does not protect vulnerable people`s privacy and dignity. This also is being referred to the Regional Enforcement Team for consideration.Sleeping accommodation for the staff members covering night sleepers duties did not provide privacy for the staff member or give sufficient space for them to rest. Sleeping in a chair/small sofa in the lounge does not provide adequate accommodation for the staff members who are covering a night sleeper duty. This may compromise the staffs ability to work well with the service users because they are tired.

The building is situated in a residential area of Paignton, within walking distance of the sea front, shops and amenities. There is level access through a small front garden, with one step inside the lobby area to access the house. There is also a concreted area and lawn to the back of the home. The home has a lower ground, ground, first and second floor with stairs throughout. This may cause difficulties for Care Homes for Adults (18-65 years) Page 2 of 8 1 3 0 1 2 0 0 9 Brief description of the care home residents with mobility problems, as there are no bathing/shower facilities on the ground floor. An external fire escape connects all floors. On the lower ground floor there is a laundry, storeroom, bedroom, shower, bathroom, and lounge , taff sleep-in room. On the ground floor there are bedrooms, a kitchen, toilet, office, dining room and lounge. The first floor has a shower room, bathroom, and bedrooms, and on the second floor there are bedrooms and a toilet. Two of the bedrooms are doubles. The costs per week for residential care at St Andrews vary between roughly three hundred and fifty pounds and five hundred pounds. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: We arrived at St Andrews at 7:15 am on 29.06.09. We were greeted by service users who alerted a member of staff to our presence. The staff member in charge of the night shift asked to be able to call the owners which we agreed to and waited in the lounge. We asked to see the shared room at St Andrews he took us to the top floor of the home to Room 2. There were two people asleep in the two beds in the room. A mattress was on the floor between the two beds. One male service user was sitting on a chair opposite the door. Sitting next to him was a female service user. The staff member in charge said the female service user was just visiting the person (service user) sitting in the chair. There was very little room to walk around the room because of the mattress on the floor. The staff member in charge showed us the remaining areas of the home and provided information about who was in each room. We returned to the ground floor and asked the staff member to see the admissions book, medication records and care plans. The staff member in charge told us that the home had 19 rooms and had 21 people living there. One inspector took the admissions book and medication records to the dining room. The other stayed in the office with the staff member in charge and went through the names of the people on the care plans and the rooms they were occupying in at the home. This information showed that the number of service users living at St Andrews Lodge at the time of this visit was 22 people. The staff member in charge advised that the extra service user in room 2 had agreed to move out of room 8 on a temporary basis into the shared room as a third person to allow another service user who needed respite to use their room. This is being referred to the Regional Enforcement Team for consideration. The staff rota was seen, staff on duty during the night included, staff member in charge (awake), staff member (sleeper). The rota seen was for week ending 3 July 2009. The staff member completing the night sleeper duty told us there is usually a room staff used to sleep but the home was full so staff had to sleep on the sofa in the lounge. The sofa had two seats about 4 6 wide. The staff member was a tall person approximately 6 tall and so would not have been able to stretch out on the sofa. They said they had used the sofa in the lounge for the past two nights and had been off duty prior to this.
