Conditions of registration: 1. 2. 17 Service Users who are over 65 years of age and fall within the category of old age. One named person under the age of 65 years of age at the time of admission can be accommodated and cared for in this home. 6th October 2005 Date of last inspection Brief Description of the Service: St Anthony’s residential home offers residential care for eighteen older people. The home is a large detached two storey property which has been extensively renovated, extended and decorated to a high standard. The rooms are all suitably furnished and equipped to meet the needs of the occupants. The decor throughout is homely .St Anthony’s provides a total of fifteen bedrooms consisting of twelve single and three double bedrooms, which are situated on both the ground and first floors. There is an attractive dining room, two lounges and a dedicated smoking area. All bedrooms and communal areas have access to a call system. Whilst none of the bedrooms have en suite facilities there are ample assisted bathing and toilet facilities to meet the current needs of the residents. The home has vertical lift access to the first floor. The home is situated close to local amenities and public transport. There is limited parking at the front of the home. This is an overview of what the inspector found during the inspection. This unannounced key inspection took place over one day by one Regulation Inspector. The Registered Manager and the staff assisted the Inspector throughout the inspection. Information was gathered from speaking with all 18 residents, 7 in depth, visitors, staff, a visiting professional District Nurse, observing the care staff performing their duties, and from examining care records and other documentation. A full tour of the premises was undertaken. 26 comment cards were received from service users and relatives, all of which were very positive in nature. What the service does well: What has improved since the last inspection?

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5 The information available, and pre-admission assessment process ensures that prospective service users have enough information to make an informed choice about living at the home, and know that the home can meet their needs. EVIDENCE: Service users are given all the information needed to make an informed choice about the home. Comprehensive pre-admission assessments are carried out prior to admission, this enables an holistic picture of the service user to be gained. This information is carried over into the service users care plan to enable the staff to meet the service users individual needs, wishes and preferences to the full. Service users, their relatives and friends are given the opportunity to visit the home prior to admission to assess the facilities and care provided. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Service users health and personal needs are very well met with the additional support of visiting Health Care professionals. Written documentation is good. Medicines are administered safely and medication records are well maintained. EVIDENCE: The service users care plans seen were very good. They contained very thorough information giving an holistic picture of the service user. Assessments are detailed with likes, dislikes and personal preferences. Monthly evaluations take place with service user involvement covering physical health, moving and handling, falls risk, nutritional status, personal risks, mental health and behavioural status and individualised daily routines Service user health care needs are fully met, the home has a very good relationship with the GP’s surgery and seeks professional advice when necessary. A district nurse visiting on the day of inspection stated “ This is one of the best homes I come to, the care and attention given to service users is above and beyond. I can guarantee that the home will carry out any care instructions I leave, and that the service users are well cared for”.

Service users stated: “ The care and attention given by the staff is 1st class” “Nothing is too much trouble for them” “I love living here, I am very happy, all my needs are met”. Medication procedures have improved. The amount of medication received by the home is documented, and returns are stamped by the homes pharmacy. Medication administration records (MAR charts) are complete, there are no missing signatures. Photographs of the residents were present on all MAR charts. Creams and lotions are dated on opening and discarded within 28 days. All staff have received training in medication administration procedures. Excellent interaction was observed between the service users and staff. Staff know the service users very well, and vice versa. Comments from service users and relatives included: “ Each service user is treated and cared for with the utmost respect” “ We are delighted with the love and care given to our relative, all members of staff are welcoming and helpful” “ The staff do an excellent job, they are friendly, co-operative and could not possibly do any more” “ Everyone is most kind” St Anthonys Residential Home DS0000016780.V289083.R01.S.doc Version 5.1 Page 12 Daily Life and Social Activities
The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Service users are able to exercise choice over their daily lives. Social contact and the activities they choose to participate in, which promotes their individuality and independence. Service users receive a wholesome and varied diet that meets any special dietary requirements. EVIDENCE: Service users take part in a wide range of activities such as trips out, keep fit, pampering afternoons, coffee mornings, board games and video evenings. Service users who are more independent take themselves out to the local shops and attend day centres. Contact with family and friends is encouraged by the home. There is an open visiting policy and visitors are made very welcome. Visitors are invited to stay for meals, and always receive tea and biscuits! Family and friends join in with the activities and outings. Comments include:

Friends and relatives are always made welcome. The staff are very kind and caring” “ The home is excellent with a very caring staff” Service users are able to exercise choice and control over all aspects of their life at the home. Meals are well balanced, nutritious and looked very appetising. Mealtimes are very relaxed. The dining room is attractively decorated, and adds to the relaxed atmosphere. Staff assist residents discreetly. Daily food records and monthly weights are recorded and nutrition risk assessments are carried out. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 The complaints procedure is comprehensive and is accessible to service users and their representatives should they need to make a complaint. The home has robust systems in place to protect service users from abuse. EVIDENCE: The complaints policies and procedures are thorough and are accessible to the service users and their representatives. No complaints had been received since the last inspection. Comments included: “ The standards are very high, my mother has been a resident for 3 years and we are very happy with the care she receives” “ The Manager and staff are very good to all of us, they listen to what we want” “ We never have any cause to complain, everything is marvellous” The home adheres to the Birmingham Multi-Agency Guidelines with regards to the Protection of Vulnerable Adults. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26 St Anthony’s provides a clean, homely and comfortable environment to live, where the service users are safe, relaxed and secure. EVIDENCE: A tour of the premises was undertaken. The home was found to be very clean, pleasant smelling and hygienic. All of the environmental requirements from the last inspection have been met. The bathroom and shower room were clean, warm and well decorated. Hygienic hand washing facilities with the provision of liquid soap dispensers were available in all areas. A new washing machine and tumble dryer have been installed in the laundry room. All infection control procedures have been met. All of the service users bedrooms were seen, and all were very comfortable and personalised with the service users own ornaments and photographs. Bedside lamps are provided for those service users who require them. All of the service users spoken to said the beds were very comfortable. St Anthonys Residential Home DS0000016780.V289083.R01.S.doc Version 5.1 Page 16 The décor in the communal areas is of a high standard, lounges were very comfortable and homely, and new televisions have been purchased. The home has put in a lot of effort to make the environment as homely and comfortable as possible. New flooring in the kitchen, new carpets in the bedrooms and new furniture in the dining room reflect this. Comments included: “ St Anthony’s is a very good home, its always clean and tidy” “ The standards are very high and the home is always clean” “ I am very happy here, its very comfortable” “ The care, cleanliness and attitude of the home to both service users and visitors is excellent. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 The home maintains a high level of core staff to offer good consistent standards of care to meet the needs of the service users. Staff undertake regular training to further develop and improve their skills. EVIDENCE: The home has excellent staffing levels which help to maintain the high standards of service provided. Staff are well trained and competent at carrying out their duties which adds to the “person-centred” ethos of the home. Comments included: “ The staff are very good” “Nothing is too much trouble for them” Staff recruitment files were examined and found to be in good order, with references and CRB checks in place. Staff undergo a thorough induction, receive regular training to develop their skills further, and have formal documented supervision sessions to discuss any issues that may arise. Staff have received training in Adult Protection, Infection Control, Administration of Medication and Palliative Care. On the day of inspection a number of staff were attending fire training. Future training for staff includes Food Hygiene, 1ST Aid and an update in Moving & Handling.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35,36,37,38 The Registered Manager ensures a high standard of service is provided at the home. Residents and staff are regularly consulted on the service provided and how it could be further improved. The health, safety and welfare of service users and staff is promoted and protected. EVIDENCE: The Registered Manager is very experienced in caring for the older person. She holds an NVQ in Management, and is an NVQ Assessor. She is due to complete the Registered Managers Award at the end of the month. High standards are maintained in all areas of the home by the Manager. Quality assurance is monitored by the Manager and the Provider, (who fully supports the Manager), in the form of meetings and discussions with staff, relatives, other professionals and service users. On the day of inspection a

The home has been awarded the Birmingham Quality Assurance Premium Award. The system for staff supervision and appraisals is up to date. Health & Safety maintenance checks have been carried out on all equipment within the home including emergency lighting, gas safety, fire alarms and electrical wiring. Accident records were reviewed, and all accidents had been documented, and CSCI informed. Staff receive regular training in Health & Safety issues to ensure service users are safe.
