The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Service users again reported that staff are kind, attentive and helpful, staff welcome visitors to the Home and service users liked the food provided to them. Staff purchase Christmas presents for service users from their own wages. The last Inspection Report is available to service users and visitors in the hallway. The Registered Manager keeps up to date Care Plans for service users, which are reviewed on a monthly basis. Staff understand how to act if abuse occurs or is suspected. If service users complain the Registered Manager has taken up issues and resolved them. Facilities are kept in a generally clean and tidy condition. The majority of staff have completed the National Vocational Qualification level 2 course to increase their knowledge of the job.

What has improved since the last inspection?

Some staff have received training in relevant service users conditions e.g. diabetes and stroke care. Facilities have improved in relation to a bathroom used as a storeroom is now locked to protect the Health and Safety of service users, a second floor bathroom has a hoist waiting to be installed to assist service users on this floor so they do not have to go to other floors to bathe, two rotten window frames have been replaced and tiling installed to a bathroom.

The location is convenient for transport links.There are four lounge areas and one dining room on the ground floor.A large number of bedrooms are well over the National Minimum Standards size requirements.There are a majority of single bedrooms. Service users have a ramped way to the rear garden and a patio where they can sit. The focus of the inspections undertaken by the Commission for Social Care Inspection is upon outcomes for service users and their views of the service provided. The primary method of inspection used was ‘case tracking’ which involved selecting two service users and tracking the care they received through looking at their records, discussion, where possible, with them and care staff and observation of care practices. This was an unannounced Inspection. The Home’s Registered Manager was on duty, who assisted with the inspection process. Planning for the Inspection included assessing notifications of significant events sent to the Commission for Social Care Inspection by the Registered Manager and correspondence between the Registered Provider and the Commission for Social Care Inspection. There have been no complaints regarding the service since the last inspection. The Inspection took place between 9.35 and 16.30 and included a tour of the building, inspection of records and indirect observation of care practices. The Inspector spoke with twelve residents and two members of staff. What they could do better: A number of Requirements from the last Inspection Report have again not yet been actioned as there has been a failure to deal with maintenance issues for some time in a number of areas – heavily stained carpets to the lounges and dining room still need to be replaced, a number of areas of the home are still in need of repainting – e.g. chipped skirting and paintwork still needs to be attended to on skirting, pipes and doorways where required, and WC F, where there are signs of damp that need to be eradicated. Three bedrooms were odouress, and needed their carpets intensively cleaned. It is recommended that appropriate equipment is purchased that can effectively deal with such carpets. Some service users suggested having a menu displayed and having more green vegetables. It was also noted that the Registered Manager still does not have a maintenance budget – it is again strongly recommended that this be set up, as then facility issues should then receive swift attention. For reasons of staff recruitment it is strongly recommended that new staff do not have to pay for their own Criminal Records Bureau checks. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 The individual needs and choices of clients living in the home are generally well met, staff are seen as efficient and caring and medication systems are generally robust. EVIDENCE: A service user said her Care Plan had discussed with her. The Care Plan covers all aspects of the national standard regarding health, personal and social care needs e.g. behaviour, skin care, speech, hygiene, medication, medical checks, food preferences, Risk Assessment s on Health and Safety issues etc. There were some sections that were not detailed, e.g. last dental appointment, the frequency of chiropody visits etc. The Registered Manager said that they would be reviewed and this information added. Plans are reviewed monthly. St Bennetts Care Home DS0000006381.V273872.R01.S.doc Version 5.0 Page 11 Records of medication administration were generally detailed and up to date – only senior staff issue medication and they have all received proper training. It is the home’s policy to promote self-medication where possible and there is a risk assessment form to evaluate this. Three service users self medicate – other service users were satisfied staff gave them their medication. Service users said if they were ill then the GP would be called. This was also evident from service users files. All service users asked said that staff were very good: ‘They could not be kinder’. ‘Staff are wonderful. They help me when I need it’. ‘This home is built around the staff. They make it a place worth living in’. ‘I was worried before I came here but the staff have been so nice I don’t worry now’. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Service users have a good lifestyle. EVIDENCE: Service users and staff said that there were no rules apart from smoking confined to a designated room. If someone wanted to have a lie in or go to bed late then this would be their choice. There is an opt out choice if service users do not wish to join in activities and some service users stay in their bedrooms all day if they wish. The Registered Manager stated that if extra bathing is wanted then this would be arranged. There was a comment regarding the infrequency of activities. The Registered Manager will look into carrying out a service users survey regarding this and then arrange extra activities as requested. Service users said that they were generally satisfied with the food with only suggestions to display the day’s menu and have more green vegetables. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Service users are confident their concerns will be acted upon and staff know how to protect them from abuse. EVIDENCE: Service users said that they thought that if they had any concerns then the Registered Manager would follow these up with proper action taken. Staff were asked what they would do in case of abuse and were aware of the proper steps to take regarding reporting this to outside agencies if not properly handled by the Management. A staff member has recently followed the whistle blowing procedure and this is being investigated to ensure that the service always protects service users from any form of abuse. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,26 Facilities are beginning to look dated and worn and some required improvements have not been made, with odour still a problem in three bedrooms. EVIDENCE: St Bennetts Care Home DS0000006381.V273872.R01.S.doc Version 5.0 Page 15 A number of facility issues have been attended to - rotten window frames to some bedrooms have been replaced, a bathrooms has been retiled, two dormer windows have received repairs and a bathroom, used as a storeroom, has been locked to protect the Health and Safety of service users. However it was disappointing to note that there continue to be a number of maintenance issues which have not been attended to by the Registered Provider, e.g. communal carpets in the lounge/dining room and ground floor corridors were still heavily stained in parts and have been for some considerable time. The Registered Provider has previously given written reassurances that this would be rectified but, to date, this has not occurred, although the Registered Manager stated that quotes have been presently obtained for carpet replacement. A number of areas of the home are in still in need of repainting – chipped skirting, pipes and doorways, and WC F where there are also signs of damp that need to be eradicated. Windows in room 11 do not open and need to be freed to allow fresh air into the room. Three bedrooms were slightly odouress and needed their carpets cleaned. It was also again noted that the Registered Manager still does not have a maintenance budget – it is still strongly recommended that this be set up, as then issues should then receive swift attention. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30 Clients benefit from a dedicated staff group, and staff checks are generally in place to protect service users welfare. EVIDENCE: This inspection has taken into consideration the number of staff/hours recommended by the Department of Health as detailed above. The home complies with the previous registration authority’s staffing requirement of care hours. The Registered Manager said that there are a minimum of three Care Assistants and a Senior Care Assistant on each shift with two waking staff at night. The rota evidenced this. The Registered Manager tries to ensure that staff have two written references and these are received before employment commences, criminal records bureau checks applications are completed as soon as possible - this was reflected in the Criminal Records Bureau checks on file, though a Protection of Vulnerable Adults check for an overseas worker employed by the Registered Provider was missing. The Registered Manager said this would be chased up. Training has been organised on a number of issues since the last inspection and nine staff are in the process of completing their National Vocational Qualification level 2 courses. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35 Service users on the whole, benefit well from the management of the home. EVIDENCE: The Registered Manager was commended by service users and staff for her efficiency and openness to new ideas – this situation is commended. Service user monies records were found to be properly kept with running balances, two signatures and receipt. Two service users amounts of monies were checked and found to be in order.
