The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has a group of staff who worked hard in order to improve the quality of care and met most of the requirements made at the last inspection. The assessment of needs and care plans are comprehensive with detailed information and staff interviewed were able to describe how care plans were implemented on a day-to-day basis.

What has improved since the last inspection?

The management and staff worked hard to implement the requirements, which were made at the last announced inspection. One requirement had not yet been fully met and a new date has been set for full compliance. Following the recommendation of the previous inspection report, the management has now installed a handrail on the wall to the staircase. Staff attended dementia care awareness training.

What the care home could do better:

Although two requirements were met in full since the last inspection, one remaining requirement is almost met. The Inspector was advised that this would be completely met within this month. The registered manager must ensure that she completes her NVQ level 4 qualification in management by the close of 2005. The management should regularly review staffing levels in order to meet service users assessed needs.

The home is situated in Cann Hall Road in Leytonstone, in the London Borough of Waltham Forest. The home is located in a residential area within easy access to community resources. There are two double bedrooms and 15 single bedrooms. The home provides five single bedrooms and two double bedrooms with en-suite facilities. The care hours are provided from 8am to 8pm with a minimum of three staff on duty and two waking night staff during the night. The home employs a full time cook, a handyman and the manager/proprietor arranges service users activity programmes. The unannounced inspection took place on a weekday morning on 9th of May 2005. The Inspector was able to speak to three members of staff including the deputy manager. The Inspector spoke to six service users and two of the family members during the inspection. They all expressed their satisfaction about the care provided in the home. The Inspector interviewed one of the proprietors who informed that they have applied for variations to provide dementia care for six people who would be accommodated in the ground floor of the home. Registration Inspectors from CSCI had already visited the home and made recommendations. The management is now working towards meeting these recommendations in order to provide such a service. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 The service ensures that all prospective service users assessments of needs are carried out prior to the admission and they are able to meet individual needs. EVIDENCE: From the examination of care files and discussion with staff it was clear that the proprietor/registered manager or the deputy manager undertakes a full and comprehensive assessment for all prospective service users to ensure they can meet individual assessed needs. For all individuals referred through care management arrangements the manager ensures she obtains a comprehensive assessment of care needs and a copy of care plan. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8 and 9 The service ensures that service users health needs are assessed and works in partnership with health professionals to meet these assessed needs. The requirements and recommendations made by the Pharmacist Inspector were almost met. EVIDENCE: Service users care plans suggest that staff identify the health needs of individual service users and how these would be met. Staff make all medical appointments and escort service users to these appointments. One service user with a pressure sore was provided with an air pressure mattress. The service has access and works together with health professionals to meet service users health needs. The deputy manager advised that they have met almost all requirements and recommendations made by the CSCI Pharmacist Inspector on 7/12/04. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13 The service provides opportunities for maintaining social activities in both home and in the community. The service welcomes visitors in the home. EVIDENCE: The service arranges social activities for service users. The activity programme recorded the following activities: bingo games, quizzes, puzzles and dominos. The home also arranges day trips to the seaside in the summer and visits to local parks, café and pubs. The service also provides a mobile sensory room for services users who have sensory impairments. Service users relatives spoken to informed that they felt welcomed by members of staff. Service users can meet visitors in private in their bedroom or in the office or in the conservatory, as the home does not have any separate visitors’ room. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Service users and their family members were aware of the complaint procedures and their complaints were dealt with. Service users have opportunity to access advocacy services. EVIDENCE: Service users family members spoken to were aware of the complaint procedures. They confirmed that management always listen to their complaints and try to resolve these as soon as possible. Family members were not aware of the CSCI role. However, the Inspector provided them with CSCI address and telephone number should they wish to make contact in future. Some of the service users exercised their right to participate in political process by voting in general elections. Service users and family members were given information how to access to advocacy services like Age Concern. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 22 The home is suitable for its sated purpose for its current service users. A handrail on the wall to the staircase is provided following recommendation made by professionals. EVIDENCE: St. Catherine is located within easy access of community resources, which includes local shops and public transport. There is a rolling programme for maintenance and a record is maintained of works carried out. The home is suitable for its stated purpose for the current service users accommodated in the home. Following the recommendation of the previous inspection report (dated 9/12/04) and the Health and Safety Manager of Waltham Forest a handrail on the wall to staircase is provided. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28 and 30 The deployment and current number of staff was sufficient to meet service users current needs. Adequate number of staff completed their NVQ level 2 qualifications in care. EVIDENCE: At the time of inspection three members of staff in addition to the deputy manager was on duty. The home currently employs 16 (full time and part time) care staff and 4 ancillary staff in order to support care staff in their work. Staff rota confirmed that two members of staff are on duty at night shift. The deputy manager advised that currently there are nine members of staff who completed the NVQ level 2 qualifications in care. The deputy manager is currently undertaking her NVQ level 4 qualifications in care and management and anticipating to complete these by the end of this year. The deputy manager advised that members of staff have attended Elder Abuse and dementia awareness training. The service is also planning to provide a 3 day course on dementia care for all members of staff as they have applied for variations to provide dementia care. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 and 38 There is an effective leadership, guidance and direction to staff to ensure service users assessed needs are met. The service ensures staff and service users health, safety and welfare. EVIDENCE: The registered manager is able to demonstrate leadership through her communication with members of staff to deliver an effective service. However, the registered manager should complete her NVQ level 4 qualification in management by 2005. In the absence of the registered manager there is a deputy house manager. The Deputy is currently undertaking NVQ level 4 qualifications in care and management. The management ensures service users and staff health, safety and welfare. Staff attended moving and handling, fire safety, first aid, food hygiene, elder abuse training. Regular checks are carried out on gas and electric appliances.
