What the care home does well

The premises are of a very high standard. The residents have plenty of space to sit with friends and visitors. The conservatory and gardens are much enjoyed by the residents, as is the unique elephant water feature in the garden. The home continues to provide a high standard of care with a caring staff group led by an efficient and competent manager. All work well as a team to ensure that the needs of the residents are met. The commitment to staff training and service satisfaction by the manager and provider is commendable.

What has improved since the last inspection?

The premises were not inspected but the new wooden flooring in the corridors and hallways is worth mention, as it looks lovely. It is much appreciated by residents as they find it easier to manoeuvre their wheelchairs and frames. The percentage of staff with National Vocational Qualification (NVQ) level 2 or above is now at 50%.

The home comprises of two storeys with lift access. The home is registered for 29 service users but currently accommodates 28 in single rooms, 11 of which have en-suite facilities. The home has large wellmaintained gardens with a summerhouse, water features and patio areas. There is a large, heated conservatory that offers more than adequate communal space for residents. The home is close to the local facilities in Kelvedon and accessible by train and other forms of transport. This was an unannounced inspection that took place on the 19th December 2005, lasting 3.5 hours. This was the second statutory inspection of the year and focussed on the remaining key standards not inspected at the last inspection, as well as a review of the requirements and recommendations from the last inspection. The inspection process included: discussions with the manager, registered provider, care staff, residents; a visiting district nurse and the information gathered from a pre-inspection questionnaire sent to the manager. This inspection covered seven key standards, two of which exceeded the standard by demonstrating commendable practice and actions. The home was clean and well maintained. It was beautifully decorated for Christmas. The staff and residents had done this task together by having a “decorating party”. The staff were caring and much respected by the residents. A visiting district nurse told the inspector that “there is a good response to health needs by staff and so referrals were few.” The manager approached the inspection in a positive and cooperative manner that was focused on achieving best practice to meet the needs of the residents. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 The manager and staff are caring and positive in their contact with residents. EVIDENCE: During the inspection the majority of residents were busy doing activities. On this occasion they were joining in with armchair aerobics followed by a memory game. The residents were thoroughly enjoying themselves. The laughter and interaction with the staff was lovely to observe. The staff clearly enjoyed the residents company as experiences and stories were being shared and told. There was lots of eye contact as residents were listened to and genuinely made to feel that they had something to share. The staff and residents demonstrated mutual respect for each other. The inspector was impressed by the staffs’ professional yet friendly approach with the resident’s throughout the activities and inspection. They were observed to walk in and out of communal rooms and bedrooms doing domestic and care tasks, always respecting residents’ privacy and making time to talk to them. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Residents’ choice is respected in many aspects of their life in the home. EVIDENCE: The residents are supported and encouraged to exercise choice in many aspects of their life in the home. Residents are represented on the quality assurance group and all are invited to regular meetings. Residents are supported to manage their own monies. They are also supported and encouraged to bring in personal possessions. The notice board in the hallway has information for residents about local advocacy service and access to legal advice is provided, if requested. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 Staff have a positive attitude towards the care of residents and the training provided. Staff are well trained to do their job in a competent manner. EVIDENCE: The commitment to NVQ and other training by the manager with support from the provider is commendable. Half of the staff team have NVQ level 2 or above and other staff are enrolled on courses. The manager had attended a “Skills for Care” training day and gathered the required information regarding the new common induction standards. She had then taken the core standards and produced a detailed training package for new staff to assist them in understanding and completing the induction. The managers’ approach is well organised and constructive in addressing the training requirements of new and established staff. Staff training records were well-organised and contained good evidence of attendance and certificates by staff on various training courses relevant to the residents’ needs. These included moving and handling, dementia awareness, continence promotion and health care. The information provided by the manager for the inspection states that additional training, on a variety of other topics, is planned for the New Year. Staff spoken to at the inspection told the inspector that they enjoyed the training opportunities, especially the NVQ courses. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35 and 38 The manager is well supported by the owner and staff in providing a well managed home that ensures good care for the residents. Practices and procedures ensure that the health and safety of residents are protected. EVIDENCE: The manager, as part of the quality assurance process, had undertaken a survey of residents, relatives and stakeholders. She had collated the responses and had written to individual relatives about any issues raised as part of the process. All information from the surveys is held in a quality assurance file accessible to residents, visitors and the Commission. St Dominics Residential Home DS0000017937.V281165.R01.S.doc Version 5.1 Page 17 A quality assurance group meets regularly, with resident, volunteer and staff representatives, to look at issues raised from the surveys as well as other health and safety concerns and training needs for the home. The quality group sets an action plan and review. It was a very efficient and effective exercise. Comments were received from health professionals, as part of the survey. One form a district nurse stated, “professional and friendly approach to both clients and district nurse team… quality of care comes across at a very high standard and this is reflected in how happy, comfortable the clients are that we visit”. Some residents manage their own monies and have secure facilities in their rooms. Records of monies held for other residents were well kept. A sample of residents’ monies was examined and well recorded with clear receipts and information on individual files. The manager is not an appointed agent for any resident. Health and safety records were well organised. The required checks with regard to electrics, hoists, lift, gas and water temperatures are maintained and monitored. Staff training with regard to manual handling, food hygiene and infection control was recorded and monitored for refresher training. The manager had done night staff checks with regard to fire drills so that all fire records and checks were completed.
