The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

St Giles has a very good management team running the home. The general care provided is of a high standard and this is reflected in the detailed documentation in the home. The home provides a warm, safe and relaxed environment for the residents. Menu`s are varied and offer good choice. Staff interaction with the residents is very good. The training and supervision given to staff ensures that staff are competent and well trained enabling them to provide individualised person centred care to the residents.

What has improved since the last inspection?

Care plans are much improved and now provide a full and detailed picture of the resident and their individual needs, wishes and preferences. Medication practices are now very good, staff training, internal audits and an external audit from the local pharmacist has resulted in improved practice. The quality assurance system takes into account everyone`s views and this is reflected in the quality audit report produced by the home.

What the care home could do better:

The general cleanliness within the home is excellent but the bath hoists require a thorough clean particularly underneath to maintain good hygiene standards.

That the home is registered to accommodate 48 adults over the age of 65 who are in need of nursing care for reasons of old age and will include one named individual, who is 48 years of age and is in need of nursing care whilst alternative private housing is arranged. One named individual who was 48 years of age at the time of admission and in need of nursing care can be accommodated and cared for. Up to five service users between the ages of 55 and 65 years who are in need of nursing care can also be accommodated and care for in this home. 27th January 2006 2. 3. Date of last inspection Brief Description of the Service: St Giles is registered for 48 older people, who require nursing care. The home is also registered to accommodate up to five people between the ages of 55-65 who are in need of nursing care. St Giles is situated in Garrets Green, close to public transport links and has facility for off road parking. The home is purpose built and the accommodation is spread over two floors. All bathrooms are on suite with toilet and washbasin. Bathing facilities are shared, with a mixture of assisted baths and floor draining showers on both floors of the building. Dining facilities and lounges are communal and the home also has a hairdressing salon. Kitchen and laundry facilities are based on site. The home is a non-smoking environment and a shelter is provided outside for this purpose. St Giles employs an activities coordinator and a wide range of activities are on offer. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 4 The quality outcome for this area is excellent. The judgement is based on available evidence including a visit to the service. All new residents receive a full comprehensive needs assessment before admission. Assessments are carried out by staff with skill and sensitivity taking into account the individuals cultural, religious and social preferences. Staff are well briefed on the needs of new residents, and significant time and effort is spent making admissions to the home personal and well-managed. EVIDENCE: Prior to prospective residents being admitted to St Giles, a thorough and comprehensive pre-admission assessment is completed. This enables the home to ensure they can fully meet the needs of the resident, and the resident has all the relevant information about the home prior to admission. The assessment tool used by St Giles is excellent. The information gained helps to inform the individuals care plan and their risk assessments. St Giles Private Nursing Home DS0000024893.V293330.R01.S.doc Version 5.1 Page 8 Area’s looked at in the assessment include personal details, past medical history, current medications, allergies, general appearance, communication needs, safety, skin integrity, diet, personal care, hobbies and interests, daily routines, spiritual needs, mental health, sleep patterns, death & dignity, equipment, aids and adaptations needed. Once this assessment is complete it provides the home with an excellent picture of the resident and allows them to start planning for admission, installing necessary equipment and planning staffing rota’s. Residents and their families have all the necessary information needed to allow them to make an informed choice about living at St Giles. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 The quality outcome for this area is excellent. The judgement is based on available evidence including a visit to the service. Health care provision and general care delivery is very good. Care plans are comprehensive, thorough and detailed enabling all staff to provide individualised “person-centred” care. Medication practices are thorough and safe. Staff are well trained in medication administration procedures. Residents are cared for in a respectful manner ensuring that their dignity and self-esteem are maintained EVIDENCE: Care plans examined during the inspection were of a very high standard. They were comprehensive, thorough, detailed and reflected the current needs of the residents. Specific care plans are in place for acute medical conditions and these are updated when changes occur. Care plans give a full holistic picture of each individual resident covering everything from personal hygiene needs to skin integrity, sleep patterns, individuality and cognitive functions. The care plans are well written and enable all staff to provide individual “person-centred” care. Residents seen looked well, were comfortably dressed and happy. The atmosphere in the home was relaxed and happy. Good interaction was observed between staff and residents, and residents confirmed they are always treated with respect and dignity. Medication practices are much improved. Medications are stored appropriately and internal audits take place regularly to ensure policies and procedures are followed. Staff have received training in medication administration, and this is reflected in the improved practice. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The quality outcome for this area is good. The judgement is based on evidence available including a visit to the service. The activities on offer meet the expectations, interests and abilities of the residents. Residents are supported to maintain contacts with their families and friends. Residents are given the choice and freedom to make decisions about their daily lives and this ensures their independence and individuality is maintained. The choice of wholesome and well-presented meals meet any special dietary needs of residents for reasons of health or cultural/religious beliefs. EVIDENCE: The home has a dedicated Activity Co-ordinator who provides an activity programme for all the residents, taking into account their individual needs and preferences. Activities include progressive mobility sessions, church services, games, karaoke, quizzes and arts & crafts. Residents birthdays are celebrated with parties, and family and friends are welcomed to join in. The home has an open visiting policy, and contact with family and friends is encouraged and supported by staff. St Giles Private Nursing Home DS0000024893.V293330.R01.S.doc Version 5.1 Page 12 Residents are offered choice and control, and encouraged to maintain their independence by personalising their bedrooms, choosing what to do each day with activities, what clothes to wear and meal choices. A mealtime was observed during the inspection. Dining rooms are spacious, the tables are nicely laid and staff assist discreetly where necessary. Menu’s are varied and offer choices from bread type to sandwich filling to hot or cold meals to vegetarian options. A choice of hot/cold drinks are available throughout the day. The complaints procedure is accessible to residents and their visitors should they need to make a complaint. Complaints are investigated in a timely and appropriate manner. The home has adult protection policies and procedures in place, and staff have a good understanding of adult protection and receive regular training. EVIDENCE: The home has a comprehensive complaints procedure. The procedure is accessible to residents and their visitors should they wish to complain. Since 31/10/05 nine complaints have been received on various issues from laundry problems to staffing. All complaints were responded to quickly, investigations and outcomes are documented, and all complainants were satisfied with the responses received. Adult protection policies and procedures are in place, and staff have a good understanding of adult protection issues. The manager has received formal training in adult protection and has cascaded the information down to the staff. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25 and 26 The quality outcome for this area is good. The judgement is based on available evidence including a visit to the service. Residents live in a homely, comfortable, clean environment. There are appropriate aids and adaptations available for residents use and this ensures that their comfort and independence is maintained. EVIDENCE: A full tour of the home was undertaken during the inspection. The home is well decorated, carpets are in good order and the communal areas such as the lounges and dining rooms are spacious and comfortable. Bedrooms are personalised to the individuals taste and all have call bells to summon assistance if required. Bathrooms and shower rooms were in good order although the bath hoists require general cleaning underneath to ensure infection control practices are maintained. Infection control audits are regularly carried out by the manager. Liquid soap and paper towels are in place where required. The home has a number of aids and adaptations for residents use including hoists, wheelchairs, air mattresses, and pressure relieving equipment. All these were found to be in good order and well maintained. In general the home is well maintained, in good decorative order and odour free. The judgement is based on available evidence including a visit to the service. There is a stable permanent workforce and this ensures that residents receive continuity of care. Staff recruitment procedures are thorough and robust. Supervision takes place regularly and is well documented. Staff training ensures that staff acquire the appropriate knowledge to support residents in a competent and safe manner. EVIDENCE: The home is well staffed with a stable permanent workforce and this ensures that residents receive continuity of care. Staff receive regular supervision sessions, attend staff meetings and receive regular training to ensure that they are competent and skilled, and that residents are safe and well cared for. Recruitment policies are robust, and staff files examined contained all the relevant information required such as references, CRB checks and work histories. All staff carry out an induction at the start of their employment which covers all aspects of care, mandatory training, social and cultural needs, diet and nutrition, housekeeping, communication and residents rights. Staff receive regular training, topics covered this year include tissue viability, palliative care, diabetes care, CPR and 1st aid. Annual appraisals are carried out on all staff to evaluate their performance, competence, skills and to identify future training and development needs. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 35, 36, 37 and 38 The quality outcome for this area is good. The judgement is based on available evidence including a visit to the service. This is a well-managed home and systems for resident consultation are good ensuring that they are able to put their views forward about the services provided at the home. Staff are trained in health & safety issues in order to safeguard residents. All equipment used at the home is regularly checked and maintained, and appropriate records are kept. EVIDENCE: The home is well-managed, and the manager has recently completed her Registered Managers Award. Management meetings regularly take place with the homes owners who are very supportive, and the manager is very proactive and interacts well with all staff and residents. Regular audits take place by the owners, external pharmacy audits and infection control audits. Residents and their representatives are regularly consulted about the service they receive via questionnaires, face to face meetings and informal discussions. Residents finances are well-managed, documented and regularly audited. Staff are regularly supervised and issues raised are followed through. Training and development needs are identified and planned for. All records and documents in the home are well maintained, in good order, regularly reviewed and updated. The homes health & safety practices are excellent. Generic risk assessments are in place, there is good accident recording and analysis of trends, all aids and equipment are in good order, regularly checked and maintained.
