Care Homes for Older People Page 3 of 8 What we found:
We had received information which included concerns about the care of a person who lives in this home. As a result of this, we decided to carry out an unannounced Random Inspection at the home in order to assess whether the needs of this person are being met at the home. We spoke with the Registered Manager and Responsible Individual about the care of this person. They told us about the persons individual plan of care. We looked at this plan of care including documentation relating to the intake of fluids and diet. It was identified that the person is receiving regular fluids and diet and where the diet is poor the person is offered dietary supplements which have been prescribed by her doctor. There is also a dietary assessment tool in place in the care plan which is current and up to date. Regular changes of position take place and the person is now sitting out of bed for short spells. Pressure area care is good and the person is nursed on a special mattress and cushion. On 21/10/08 it was noted that there was a gap in the recording of care of 5 hours and this was discussed with the manager at the time. The manager explained that the person has a problem with her swallowing reflex sometimes and has been assessed by the Speech And Language Therapist (SALT). There is no record of this assessment in the care plan and there is no plan of care in place relating directly to this problem. This was discussed with the manager at the time of the inspection visit. It was also identified that there is little evidence of reviews taking place with the persons representative present. This was also discussed with the manager at the time. Also the monthly evaluations of the care plan are not very detailed and do not give us much information. These evaluations/reviews would benefit from being made more detailed. This was also discussed with the manager. We visited the person who is living in the home and discussed her care with her. She is generally happy with the care provided by the staff at the home and appears comfortable and well cared for. She did say that she had not had her teeth cleaned that day and was waiting for this. We asked the manager to address this at the time of the visit. The person receives visits from her GP as necessary. The persons next of kin (representative) is documented as her brother. The manager and responsible individual identified that one visitor had been requested to ring the home and only visit with prior consent and that the homes Solicitor has sent a letter to this visitor with details of visiting arrangements.
