Conditions of registration: There are no additional conditions of registration. Date of last inspection 24/11/04 Brief Description of the Service: St Michaels provides residential care for 39 older people. The home is situated on the sea front at Clacton on Sea and is close to local shops and amenities. The building is a large property spread over three floors. There is a religious community adjacent to the home and a Chapel that is shared by the home, Convent and the local community. There are gardens and terrace areas around the property with views over the sea. There is a small car park for visitors with ample roadside parking nearby. This is an overview of what the inspector found during the inspection. This routine unannounced inspection took place on 11th August 2005 over 5 ½ hours. 17 of the 38 National Minimum Standards were assessed at this inspection and all were met. During the inspection process the inspector spoke with 12 residents, 5 family members, the manager, deputy manager, building manager and 9 staff members. A physical tour of the premises was undertaken. The level of assistance and participation from all concerned in the inspection process on the day was greatly appreciated by the inspector. Overall the standard of care at St Michael’s was good as evidenced by the findings of this inspection and the positive feedback from residents on the day of the inspection. What the service does well: What has improved since the last inspection?
The previous inspection did not result in any requirements or recommendations. The registered manager and staff had supported the residents to benefit from a charitable project enabling them to undertake training in the use of computers. Residents were provided with a computer, scanner and digital camera and were offered tutoring and mentoring in the skills to operate the equipment. 16 of the 34 residents had embraced this opportunity and exhibited excitement and enthusiasm towards the project. Participants spoke of the thrill of accessing memories via the Internet and the stimulation this activity provided.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Residents move into St. Michael’s assured that their needs are assessed and will be met. EVIDENCE: Prospective residents to St. Michael’s had their needs assessed by people trained to do so before entering the home. The assessments were undertaken by the Care Manager and were comprehensive encompassing the physical, spiritual and emotional needs of the individual. One resident recently admitted to the home spoke of the assessment process and of the family consultation and involvement. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10 Residents were treated with respect and their right to privacy was upheld. EVIDENCE: The arrangements for health and personal care ensured that residents’ privacy and dignity were respected at all times. All residents consulted during this inspection process confirmed that they felt valued, respected and protected by St Michael’s management, staff and ethos. Residents had access to telephones at any time; calls could be made from the privacy of the manager’s office if required. Many residents had private telephones in their own rooms. Staff members were observed to use individual forms of address preferred by each resident. All staff members received instruction on how to treat residents with dignity at all times as part of the home’s robust induction process. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15 Residents’ experiences within the home matched their expectations and preferences whilst satisfying their social, cultural, religious and recreational interests and needs. Residents were encouraged to maintain contact with family, friends and the local community as they wished. Residents were encouraged to exercise choice and control over their lives. Residents received a wholesome appealing balanced diet in pleasing surroundings. EVIDENCE: The routines of daily living at St Michael’s were observed to be flexible and varied to suit residents’ expectations and capacities. A social activity programme and chatty newsletter were published monthly and distributed to all residents informing them of events in the locality as well as in the home. Residents had the opportunity to attend daily Mass in the Chapel, members of the local community joined with residents.

Residents stated they chose when to go to bed or rise, how they personalised their rooms, what activities they participated in and they had a choice of meals offered. Residents were encouraged to maintain contact with family and friends. Visitors were made welcome at any time and residents had the choice of receiving them in their own rooms or in a bright cheerful parlour. Overall, residents spoken with confirmed they found the food provided at St Michael’s to be wholesome and appetising. One resident said of the food, “you couldn’t better it”. The majority of the residents took breakfast in bed daily whilst other meals were served in the dining room. The home employed dedicated catering staff to prepare food and wait on tables. Menus demonstrated that the residents had a choice of two main meals daily and that menus were rotated every 6 weeks. The chef consulted with residents regarding their dietary requirements individually every 2 weeks and attended residents meetings. Some residents required assistance at mealtimes; this was delivered in a quiet and dignified manner. On the day of the inspection residents were observed enjoying their midday meal in the pleasant dining room in a relaxed and social manner. The residents have access to a daily ‘trolley shop’ where they may purchase personal items such as toiletries, birthday cards and sweets. The residents have decided to purchase a plasma screen television for their communal space; this will be funded by a ‘Strawberry Tea’ fundraising event undertaken this summer at the home. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Residents, their families and friends were confident that complaints would be listened to, taken seriously and acted upon. Residents were protected from abuse. EVIDENCE: St Michael’s operated a simple, clear and accessible complaints procedure including the stages and timescale for the process. The home had not received any complaints since the previous inspection. The manager stated that the management team operated an ‘open door’ policy enabling residents and families to address any areas of concern before they develop into complaints. All residents and family members spoken with during the course of this inspection confirmed that if there were any areas of concern they would have no hesitation in discussing these with the manager and/or the deputy and they were confident their opinions and feelings would be valued. The manager and deputy had received training in the Protection of Vulnerable Adults from abuse. This training had been cascaded trough the remaining staff team including the catering, domestic and laundry staff. All new staff members undertook an enhanced Criminal Record Bureau disclosure and were checked against the PoVA referral list. During discussion the manager demonstrated a good understanding of PoVA procedures. Some ‘pocket monies’ were held by the home on behalf of some residents with complex needs. Two signatures were present for all transactions, receipts were cross-referenced and individuals’ funds were maintained separately.

Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 and 26 Residents lived in a safe well-maintained environment. Residents had access to safe and comfortable indoor and outdoor communal facilities. The home appeared clean, pleasant and hygienic. EVIDENCE: The location and layout of the home was suitable for its stated purpose; it was accessible, safe and well maintained; the home met residents’ individual and collective needs in a comfortable and homely way. The home employed a building manager responsible for routine maintenance, decoration and refurbishment. There were plans already agreed by the Trustees to install a ‘wet room’ specifically with highly dependent residents needs in mind. At the time of the inspection equipment such as shower chairs was being assessed for suitability. The residents at St Michael’s benefited from spacious well-decorated communal areas. The gardens were attractive, well kept, safe and accessible

Lighting and furniture throughout communal areas was domestic in character and of good quality. On the day of the inspection the home appeared clean, fresh, welcoming and friendly with no offensive odours present. There was a dedicated team of house keeping staff to maintain the home to this standard. Laundry facilities were sited away from food preparation areas. The home employed dedicated laundry staff and all residents spoken with praised the laundry service at St Michael’s. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29 and 30 Residents’ needs were met by appropriate numbers of skilled staff competent to do their jobs. Residents were safe at all times and were supported and protected by the home’s robust recruitment practices. EVIDENCE: Staffing rotas and discussions with management demonstrated that the residents’ needs were met by an experienced staff team including care staff, laundry staff, domestic staff, and catering staff. No care staff below the age of 21 were employed by the home. The staff at St Michael’s were committed to the NVQ programme. At the point of this inspection 80 of care staff had achieved NVQ level 2 or above. The home did employ agency staff when the need arose; they had a good relationship with the agency and ensured continuity of care by the provision of regular staff to the home. The registered person operated a thorough recruitment procedure in order to ensure the protection of the residents. One volunteer attended St Michael’s regularly and the manager had obtained a Criminal Record Bureau disclosure for this person. All staff received induction training relevant to the area of work they were to perform. All staff received training in the mandatory areas required to meet

Service user group specific training had been provided for care staff as a result of liaison with the local NHS Trust. JUDGEMENT – we looked at outcomes for standard(s) 32 and 35 Residents’ benefit from the ethos, leadership and management of the home. Residents’ financial interests were safeguarded. EVIDENCE: The people residing at St Michael’s Care Home were supported and protected by the open positive and inclusive management style of the registered manager and deputy; this was evidenced through consultation with residents, visitors and staff members. All 12 residents consulted stated that if they felt they had any concerns regarding any aspect of life at St Michael’s they would be confident in approaching the manager or deputy and that their opinions and feelings would be respected. ‘Pocket monies’ were held by the home on behalf of some residents with complex needs.
