It is a small family style home. The lounge is situated at the front of the house and dining room overlooks the garden. Toilet and bathroom facilities are on both floors. The home offers six single bedrooms and one double, which has been separated to ensure privacy. Rooms vary in size, but none have ensuite facilites and some would be too small for wheelchair use. A shaft lift provides access to both floors where residents accomodation is provided. St Michaels Care Home is close to local amenities, including transport facilites. There is limited parking on site, but additional parking is

Staffing Complement. When arriving for the Random Inspection it was noted that only one staff member was on duty. The second staff member had left the home to go and get some shopping needed for lunch. It was explained to both staff members that the health and safety of the residents and themselves is paramount and one staff member should not be left alone on duty. The staffing rota for the week was requested and this was provided. The rota was correct on the day of the inspection, and on questioning staff they were able to confirm the days they had previously worked. On previous inspections it has been noted that only one staffing rota is kept in the Managers office; so it is difficult to establish work completed by staff on previous weeks. A second rota was available on this inspection, dated the 19th to the 25th but did not have a month recorded on it and the date it did not coincide with the present rota. The Deputy Manager was advised to have more than one staffing rota, so a pattern of work can be identified on future inspections. The staffing rota viewed showed that the home presently has six staff who cover both day and night shifts and mostly do a 8.00am to 8.00 pm shift. The Deputy Manager advised that the Proprietor is presently interviewing more staff to ensure there is sufficient cover for any sickness or annual leave. Qualifications. It was confirmed that two staff had completed their NVQ 2 training and one is presently doing their training. The Deputy Manager is presently doing their NVQ 4 and hopes to complete this soon. Recruitment. No new staff had been recruited since the last Key Inspection, so it was not possible to view documentation to support that the homes recruitment procedure had been followed. As previously stated the Proprietor is in the process of recruiting more staff, so this Standard will be fully inspected at the next Key Inspection. At previous inspections it has been established that all staff have separate files and these contain personal data, training and supervision documents. Staff Training. Due to no new staff having been employed, it was not possible to review induction documentation. At the last Key Inspection it was established that all new staff would complete a three-day induction course in London, but documentation was not available to support this. The Deputy Manager had recently produced a staff induction matrix, which provided information when the present staff had completed their inductions. The Deputy Manager had also produced a training matrix, which provided details on when staff had completed training and when updates were required. On viewing this it was
Care Homes for Older People Page 4 of 12 clear the staff have completed training on moving and handling, health and safety, safeguarding vulnerable adults, infection control, food hygiene, fire safety, first aid and medication. From the dates recorded on this document it was noted that some updates are now required. Staff had also completed training on dementia activities, pressure care, nutrition in the elderly and dementia awareness. Day to Day Operations. At the last Key Inspection is was established that the Proprietor planned to make Deputy Manager the Registered Manager of the home. At this inspection it was confirmed that the previous Manager of the home had now changed her position and does not have the responsibility of the day-to-day running of the home. She is still employed by the home, by working nights and the occasional day shift. The Deputy Manager who has been present on the last two inspections, is now in charge and is in the process of applying for a visa to enable her to apply to be the Registered Manager. The CQC had not received anything in writing from the Proprietor with regard to the recent day-to-day changes to management the home. St Michaels Care Home still does not have a Registered Manager and an application is required as soon as possible. The Inspector was advised that the Proprietor is now spending less time at the home, due to other work commitments. The Deputy Manager explained that he still oversees the running of the home and the finances, but she is in day to day control. Ethos. Two random inspections have been completed over the last 6 months to assess the improvements made to the home and to check that these have been sustained. During this visit it was clear that further improvements had been made and the recommendations made at the last random inspection in February 2010 have been acted upon . The staff at the home work well together and they were observed offering choice and encouraging residents to do things for themselves and were aware of the residents care needs. Quality Assurance. It was established at a previous Key Inspection that resident meetings are bi-annual. The next residents meeting has been arranged for 8th May 2010. The Deputy Manager stated that they are in the process of completing their annual quality assurance on the home and it is hoped that the report will be available on the next Key Inspection. Financial Procedures. This standard was not fully inspected, but it was noted that the home has insurance cover and this is valid until the 19th April 2011. Service Users Money. The home has a system in place for the financial interests of the residents. Three residents monies were requested and checked. It was noted that the home had provided receipts and there was also a financial form to record any transactions on. The form did not have a column to record any monies received and this had been added under monies
Care Homes for Older People Page 5 of 12 spent; which was confusing when auditing the contents. All three residents had recorded that they had paid for the service of a chiropodist, but when checking their monies they were all £12.00 over the recorded balance. This was brought to the Deputy Managers attentions and advised that entries should only be made when a financial transaction has taken place. They were also advised to gain 2 signatures when taking monies out, which would assist in any financial audit completed by the home. Staff Supervision. A staff meeting had been arranged for the 1st May 2010, but this had been cancelled and rearranged for after the residents meeting on the 8th May. The Deputy Manager had produced a supervision matrix, which showed the dates staff had been seen. This showed that staff had been supervised over the last 12 months, but the frequency differed for each staff member; but had improved since the last Inspection. This will be inspected further at the next Key Inspection. Record Keeping. Individual records are now available for both staff and residents. There have been ongoing improvements on record keeping over the past 12 months and the Deputy Manager has introduced systems to ensure that there is documentation available to evidence practices and procedures within the home. The Home is now able to supply written documentation to show that the National Minimum Standards are being met. Discussion took place with the Deputy Manager on how she can ensure that the relevant documents is available on future inspection visits, especially when she is not on duty. Safe Working Practices. This standard was not fully inspected at this inspection. What the care home does well: What they could do better:
Improvements have been made to the home since the last Key inspection and written documentation seen during the random inspections shows that this has continued. Most of the areas highlighted in the last random inspection have been actioned, and the home continues to develop activities organised for the residents. Some training updates are required for staff. The next annual quality assurance on the home is now due and it is hoped that the report will be available on the next Key Inspection. The form used for recording residents monies needs to be updated to include a record of any money received. The process of recording monies spent needs to be reviewed as the records were not always correct. Entries should only be made when a financial transaction has taken place. Also gain 2 staff signatures when taking monies out, as this would assist in any financial audit completed by the home. The home is still looking tired and in need of redecoration and new furnishings. There have been no new admissions to the home, although the rating has improved. The CQC should be made aware of any changes to the day-to-day management of the home. The home is still without a registered manager. This is an area that needs to addressed, especially as it is an outstanding requirement from September 2009. This is to ensure that residents live in a home that is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully.
