Conditions of registration: Date of last inspection 2nd November 2004 Brief Description of the Service: St Stephens Nursing Home is registered with the Commission for Social Care Inspection accommodate 31 people with dementia. The home is a converted 3storey house with a purpose built 2-storey extension to the rear. The home is situated in the North shore area of Blackpool, close to the seafront, and approximately 10 minutes by car to the town centre. There are an appropriate level of aids and adaptations in place to meet the needs of residents who are frail and immobile. The home has 27 single rooms, 14 with on suite facilities, 2 double rooms. This is an overview of what the inspector found during the inspection. This was an announced inspection. It took place on the 31st May 2005 and was conducted for approximately 5 hours. Discussion took place with the registered manager, four interviews and informal discussion with staff members. There was formal discussion with two visitors; and general discussion and observation of residents. This process helped to gather information, which is contained in the main body of this report. Twelve visitor/relative comments were received. They were generally favourable and praised the staff team. It is important to ensure relatives have access to the most recent inspection report. In addition questionnaires were sent to a number of social workers and GP surgeries to comment on the standard of care provided by the home. At the time of writing the report one GP had returned the comment card, which was favourable to the level of care provided by the home. The examination of documentation formed the basis of the visit. Staff recruitment procedures are now in place, and all fitness checks are undertaken prior to staff commencing employment at the home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,5 People are admitted to the home, only when assessments have been carried out; by social workers and the registered manager of the home, this ensures a persons needs can be met by the home. Relatives or advocates are encouraged to visit the home prior to making a decision to agree a placement. EVIDENCE: The admission procedure is planned and ensures residents needs are properly assessed and planned for. One visitor spoken to said “we had a good look round before making our decision, but this home felt just right”. There is a need to ensure that the current inspection report is made available to all users of the service. Individual records seen were complete and up to date, they contained information relating to the needs of that person. Staff spoken to are fully informed of the needs of people living in the home. A full social work assessment is made available to the home as part of the admission process as well as the registered manager undertaking assessment of the prospective resident prior to admission, to ensure the placement is appropriate and the can meet the residents needs.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The health care needs of people living at the home are clearly identified, so that staff are able to deliver the level of care necessary for residents. EVIDENCE: Individual care plans identify all aspects of a persons; health, social and personal needs. Plans seen were up to date and showed reviews are held regularly, ensuring peoples individual needs are met. Staff spoken to were aware of the need to ensure residents privacy and dignity is upheld. One member of staff commented “I treat people in a way I would expect to be treated, and sometimes that just means making sure a door is closed”. There was evidence of risk assessments being carried out for all residents, for personal and environmental risk. This makes sure residents and staff are protected. Staff knowledge and understanding of residents healthcare needs ensures they are being met. Records and health plans were viewed for three residents. Plans clearly describe health needs and how they would be met. Three residents had received health checks, and ongoing appointments for specialist treatment were being kept with outcomes recorded.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,14,15 Social activities are in place but could be further developed to meet the specialist needs of residents living at the home. Meals are well managed, balanced and provide a daily variation and interest for people living in the home. EVIDENCE: There is an activity programme, including a monthly entertainer visiting the home, some reminiscence and recall therapy, as well as staff encouraging residents to join in singing and dancing. The home would benefit from researching other forms of activity specifically designed for people suffering from dementia. Residents have various limits on their ability to make choices, however staff were seen to help and gently guide them in all areas of their daily lives. Staff spoken to said “we get to know residents individually, and can recognise what they like and need from watching their actions, and following the care plans”. The food and menus are varied and offered daily choice. Mealtime arrangements are flexible enough to accommodate individual preferences. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Complaints are handled objectively and responded to appropriately, which is good practice. There is a vulnerable adults procedures, which is used in induction and training to ensure residents are safeguarded from abuse. EVIDENCE: There have been three complaints investigated by CSCS since June 2004, they were not upheld. Visitors spoken to knew of the complaints procedures and how to use it. Staff spoken to were confident in the homes complaints system, one staff member commented “ complaints usually result in changing things for the better”. Four staff members spoken to had training in the area of adult protection and whistle blowing, therefore are equipped to identify abuse and act upon it appropriately. Suitable systems have been introduced to report all significant matters that occur. There were no outstanding complaints or issues on going at the time of inspection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29,30 The procedures for the recruitment of staff are robust thereby providing protection for the people living in the home. Staff have access to training, but need to attend updated courses, in order to be kept up to date with changing practices. EVIDENCE: Three staff files seen show recruitment is thorough, ensuring staff have received all checks before starting employment at the home. One staff member commented on how they had been recruited, and said “ how thorough it had been”. Staff training is focused upon highly, as the home wants its staff team to be trained to deliver a good level of care. There is a need for staff to update training in areas where there are time limitations including First Aid and Food Hygiene. There should be more training in areas for caring for people suffering from dementia, in order to use up to date practices for people suffering from the condition. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,36 The home is well managed and run efficiently providing a safe and stable environment for people living there. Staff are supported and feel confident in the way the home is managed. EVIDENCE: The management structure makes sure there is shared responsibility in the operation of the home. Staff spoken to commented on how supported they felt. One staff member said; “I know I can always rely on the manager to get things done”. The home has been awarded with Investors in People, which helps staff develop in training and improves the quality of service they provide in their work. Staff spoken to commented on “how supervision helps, develop personal needs”. The registered manager is supported by the registered providers, who work jointly in the decision making process, in order to ensure all residents and staff needs are met.
