Conditions of registration: There are none Date of last inspection 22 December 2004 Brief Description of the Service: 1 Station Road is registered to provide accommodation and care for up to six people with learning disabilities, of whom up to six may be over the age of sixty five. The registered provider is Macintyre Care of The Old School House, Bedford Street, Ampthill, Bedfordshire.MK45 2NB 1 Station Road is a two storey, victorian house, standing in large grounds in the village of Wheathampstead, close to local shops and amenities. The accommodation comprises six single bedrooms, one of which is ensuite, dining room, lounge, large kitchen, utility room, two bathrooms plus a shower room. The first floor is served by a passenger lift. The home has a riverside garden with seating area and has car parking to the rear. This is an overview of what the inspector found during the inspection. This unannounced inspection began as two of the residents were preparing to leave for their day centre and continued through to lunch, which the inspector was invited to share with the residents and staff. The registered manager was away from the home and the acting manager was engaged with NVQ activities, the staff present provided excellent support to the inspection and enabled access to the basic records required. The main focus of this inspection was to talk with and observe the interaction of the residents. It was possible to accompany three residents for a short walk to feed the ducks and the inspector was given a picture painted by one resident to brighten up the office. It was a pleasure to share lunch with the residents and the inspector would like to thank them for again welcoming him to their home. The good standard of record keeping has been maintained and there have been some improvements to the premises. There were no requirements made following the previous inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3 & 6 Assessments, Service User’s Guide, care reviews and contracts are provided for all residents in a format that is clear and comprehensible for them and their representatives. EVIDENCE: The documentation seen has been imaginatively produced, including pictures and diagrams, to assist residents understand and access the contents. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 & 10 There are comprehensive individual records of residents’ health and social care needs and how these are met. Residents are treated respectfully and are appropriately involved in decisions about their care. EVIDENCE: Care plan documentation was seen during this inspection to be very full and detailed and included a good level of resident’s signatures to help demonstrate their involvement in the care planning process. Throughout the inspection the interaction and relationship between staff and residents was seen to be relaxed, respectful and appropriate. Medication records were checked and found to be in order. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,14 & 15 Residents are encouraged to exercise choice and control over all areas of their lives and Person Centred Planning is in place and embedded within the ethos of the home to ensure this is a reality not just an aspiration. EVIDENCE: Residents’ care plan documentation contains considerable evidence of their involvement in social and community activities. Residents were keen to discuss their forthcoming holidays and trips and arrangements were being made during the inspection to book a holiday for two residents specifically tailored to meet the criteria they had established. Service users’ legal rights are protected. Service users are protected from abuse. JUDGEMENT – we looked at outcomes for standard(s) 18 Residents are protected by a system of staff training and policies and procedures, which should enable actual or potential abuse to be identified and appropriately dealt with. EVIDENCE: Policies and procedures have been seen dealing with the protection of vulnerable adults. Staff training includes awareness of abuse and what action to take if it is suspected. The residents have access to external agencies and individuals who could assist them to express any concerns they might have independently of the home. Financial records for residents were spot checked and found to be robust and properly supported by receipts and counter signatures. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,21 & 26 With the exception of current problems being experienced with the home’s water storage, the home provides a safe and suitable environment for its residents and staff. EVIDENCE: The lift was recently out of action as a result of water seepage into its mechanism. During the inspection taps had to be turned on at various points in the home to deal with overflows. The home was clean and tidy and provided a pleasant environment for residents, however not all toilets had soap and towels provided. Liquid soap and paper towels should be available to ensure good hygiene and infection control procedures. An upgraded shower room, which the residents particularly like, has been provided since the previous inspection.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 & 30 The numbers and skills of staff are adequate to meet the needs of the home’s residents. EVIDENCE: Staffing was seen to be at the previously agreed levels and appeared to be adequate for the needs of residents during this inspection. Staff confirmed that they feel well supported with training and supervision. Training and supervision records were available to confirm frequency and content. One member of staff present is currently undertaking the Certificate for Working with People with Learning Disabilities, and it was possible to see her workbook. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35,36 & 37 Residents are well supported and protected by the management of the home. EVIDENCE: Records seen and practice observed suggest that resident’s needs and interests are genuinely at the centre of the home’s activity. A spot check of residents’ accounts was satisfactory. Some clarification of the requirement to report certain incidents to the Commission for Social Care Inspection (CSCI) was discussed and is the subject of a requirement as care plan documentation seen contains details of some incidents which, whilst very fully recorded, were not notified to the CSCI under this provision.
