Conditions of registration: Date of last inspection 19th July 2005 Brief Description of the Service: 1 Station Road is registered to provide accommodation and care for up to six people with learning disabilities, of whom up to six may be over the age of sixty five.The registered provider is Macintyre Care of The Old School House, Bedford Street, Ampthill, Bedfordshire.MK45 2NB1 Station Road is a two storey, Victorian house, standing in large grounds in the village of Wheathampstead, close to local shops and amenities.The accommodation comprises six single bedrooms, one of which is ensuite, dining room, lounge, large kitchen, utility room, two bathrooms plus a shower room. The first floor is served by a passenger lift.The home has a riverside garden with seating area and has car parking to the rear. This is an overview of what the inspector found during the inspection. This unannounced inspection coincided with a visit by the area manager responsible for the home. It provided an opportunity to speak to her and the home’s relief manager about the challenges facing the service and to observe how they were dealing with the recent deterioration in the health of one of the residents, which had led to what will hopefully prove to be a temporary increase in dependency. The care outcomes for all residents remain good and there was a relaxed atmosphere in the home, together with a sense of anticipation ahead of the next days Christmas house party. This remains a well- focussed service, with management and staff committed to a person centred approach to providing care. The residents clearly regard this as their home, and the inspector was again made to feel very welcome. What the service does well: What has improved since the last inspection?
The problems previously experienced with the lift and water supply have been addressed, and both now operate satisfactorily. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8,9 The health care needs of residents in this home are well met. The home and its residents are well supported by community healthcare professionals and there is a robust system in place for the administration of any medication required. EVIDENCE: One of the home’s G.P.s visited during this inspection and there was recent evidence seen in care plans and daily records of the involvement of district nurses, occupational therapists and physiotherapists. Examples of the new care planning format were seen, together with some still in the existing format, both provide the required level of detail although the new version does so in a more accessible and user friendly format. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 Residents are encouraged to maintain contact with family and friends and to make use of local community facilities. EVIDENCE: Photographs are on display in the lounge, showing recent social events in the home attended by visitors and of visits made by residents to a variety of places outside of the home. Day care, art classes, clubs and church all form part of the activities available to and made use of by residents. One resident was due to go shopping in the afternoon of this visit, residents had invited several people to the home’s Christmas party to be held the next day and the visitor book contained evidence of a wide range of contacts with family, friends and the local community. EVIDENCE: Policies and procedures have been seen and records of complaints form part of the very full proprietor’s monthly visit reports, received by the CSCI. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 22 The home provides a safe and suitable environment for its residents and staff and provides the equipment required to meet the assessed care needs of its residents. EVIDENCE: Previous problems, experienced by the home with the lift and water supply have been satisfactorily dealt with. The home was seen to be comfortable and homely, providing a very domestic scale environment for the provision of care to its residents. One resident has recently been assessed as needing the help provided by a hoist and this has been serviced appropriately and staff have been adequately trained in its use. JUDGEMENT – we looked at outcomes for the following standard(s): 27 28 29 30 The numbers and skills of staff are adequate to meet the needs of the home’s residents. Macintyre Care have a robust and thorough recruitment process, which provides for the necessary checks to protect the best interests of the home’s residents. EVIDENCE: Staffing was seen to be at the previously agreed levels and appeared to be adequate for the needs of residents during this inspection. Staff confirmed that they feel well supported with training and supervision. JUDGEMENT – we looked at outcomes for the following standard(s): 32 38 Residents are well supported and protected by the management of the home. EVIDENCE: Observation on the day, contacts between visits and discussions with the Area Manager, the relief Manager, staff and residents provide confirmation that the process of management in the home delivers care to a high standard, with the health and well being of the home’s residents firmly at the heart of its operation. No. 1 2 Refer to Standard OP9 OP9 Good Practice Recommendations The manager should continue to monitor staff and medication records to ensure that any gaps are noted promptly and appropriate action taken. Medication storage temperatures should be monitored daily and recorded at least weekly to ensure that they remain at acceptable levels for the medication being stored.
