Care Homes for Older People Page 3 of 7 What we found: This inspection was triggered as the Commission had received some concerns regarding the service. All the relevant breaches regarding Regulations were looked into. Some of the concerns were substantiated. We discussed residents care plans, staffing level, staff personnel documents and we also checked medication records on one floor. Three care plans were sampled at random and it was noted that none of them have been reviewed recently. The care plans for resident A was last reviewed on 23/09/09 and the previous review date on some of the care plans were 15/06/09. The Waterlow risk assessment which need to be updated by the home on a monthly basis, was last reviewed on 23/09/09. Resident B care plans were last updated in June 2009. The Waterlow risk assessment has not reviewed since July 09 despite being graded as high risk. The resident has lost 6kg in weight and did have a dietician referral. However there was no evidence from staff about how the issue would be managed. Nothing about the provision of pressure relieving equipment etc was documented. Resident C was admitted in September 09 and the care plans have not reviewed since admission. The resident has lost 5kg since admission, and is now on PEG fed. There was no evidence about referral being made back to dietician or any action being taken to identified the weight loss. All our findings were discussed with the management of the service.
