Conditions of registration: Date of last inspection 22nd August 2005 Brief Description of the Service: The Bridgings, Woodland Road, Middlesbrough, was opened in 1989 to provide a home for 10 younger adults with a learning difficulty. The home aims to provide care tailored to individual need and the emphasis is on meeting, holistically, the daily living needs of all residents. With the exception of one double room, all rooms are single, and rooms can be personalised by residents according to their preference and taste. The home is situated in central Middlesbrough close to the University of Teesside, and within easy walking distance to shops, cafes and restaurants. This is an overview of what the inspector found during the inspection. This Inspection took place over three visits to the home and in the main concentrated on ensuring all remaining Key Standards had been audited. In addition, time was spent talking with residents who were at home at the time. Members of care staff were also spoken with. The internal maintenance and decoration of the home was also a major factor assessed at this Inspection. What the service does well: What has improved since the last inspection? Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Prospective care needs and aspirations are assessed prior to any admission to the home. EVIDENCE: All residents currently living at Middlesbrough Bridgings have been there for some considerable time, and as such there have been no recent admissions to the home. However, discussion with the Manager confirmed that a thorough and detailed, multi-disciplinary assessments would be obtained prior to any new admission taking place. The assessment will include details of all care needs, along with preferences and aspirations of the person concerned. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): These Standards were not assessed on this occasion, as the key standards were audited at the Inspection held in August 2005. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17 Discussion and observation confirmed that the rights are respected, and responsibilities recognised of those living at the home. Staff work towards ensuring a healthy diet is offered at all times. EVIDENCE: The manager said that all living at Middlesbrough Bridgings take responsibility for their own bedrooms, with support from care staff where necessary. In addition, residents assist with light cleaning, and domestic tasks. Those who wish to, enjoy helping to plan and prepare meals. Three residents also undertake paid work in the community on a regular basis. In respect of diet and food preparation, a member of care staff has been nominated to take responsibility for menu preparation. This takes place in consultation with residents, who contribute both to menu planning and shopping. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 A policy for the administration of medication is in place and staff are appropriately trained in this respect. EVIDENCE: Currently, no service users wish to retain responsibility for their own medication and all medication is administered in accordance with the policy of the home. Three members of care staff are in the process of completing the Safe Handling of Medicines course at Middlesbrough College. The home recently received a visit from the pharmacy advisor for the Primary Care Trust, who apparently commented on the fact that the system for the administration of medication was sound. In order to ensure both compliance and accuracy of the procedure, the manager carries out regular audits, which are usually ‘spot on’. Medication is safely and securely stored within the home. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Service users definitely feel listened to and their views are acted upon where appropriate. Staff are aware of the need to protect vulnerable adults. EVIDENCE: In addition to regular residents’ meetings, those living at the home are able to chat to staff on an individual basis whenever the need arises. As a result of this, service users know that they are listened to, and that their wishes are acted upon where appropriate. Observation of interactions between residents and staff confirmed that there appears to be an easy, trusting relationship between them. During the Inspection it was observed that all residents were relaxed and appeared very much at home. Staff at the home have completed the Teeswide Adult Protection training level 1, using the ‘No Secrets’ workbook. The manager is aware of the reporting procedure for any concerns, and staff are aware of the importance of the need to protect residents from abuse, and the need to flag-up, with senior staff, any concerns they might have. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 The lack of prompt action on issues relating to the décor and maintenance of the home continues to cause concern. EVIDENCE: On the first day of the Inspection, it was disappointing to find that work required to upgrade the environment, which was highlighted at the last Inspection, had not been addressed. In addition, it was seen that some areas of the home were in need of general upgrading from the point of view of décor. However, at the final Inspection visit, much of the outstanding work was in the process of being carried out and agreement was reached that the environment will be inspected again at the end of February 2006. JUDGEMENT – we looked at outcomes for the following standard(s): 34 A policy and procedure is in place in respect of recruitment, which works towards ensuring the protection of residents. EVIDENCE: Discussion with the manager and inspection of staff files confirmed that two references, along with a verbal check, are obtained for prospective staff. In addition, both CRB (Criminal Records Bureau) and Pova (Protection of Vulnerable Adults) checks are carried out prior to employment commencing. JUDGEMENT – we looked at outcomes for the following standard(s): 39 Discussion with staff and service users confirmed that their views are taken into account and acted upon where appropriate. EVIDENCE: Observation during the Inspection confirmed that those living at Middlesbrough Bridgings feel able to freely voice their wishes and preferences – none of those present during the Inspection appeared reluctant to air their views. In respect of a recent holiday abroad, those wishing to go on the holiday reached a consensus about their preferred destination. Again, observation confirmed that residents are treated with respect, and their wishes and preferences are taken into account and acted upon where possible and appropriate.
