What the care home does well

The manager and staff at the home are good at providing a friendly atmosphere that places residents at the centre of the care provided. One pictorial survey received contained the comment ` I just like living here`. Comments in relative surveys included ` they totally understand my relatives and their needs` and ` I am totally satisfied with everything they do`. There are opportunities for personal development, social and leisure activities for residents. Staff are committed to providing care that is individual to each residents needs. Residents are encouraged and enabled to have choice and control in their lives.

What has improved since the last inspection?

What the care home could do better:

Redecoration and refurbishment should continue to improve the environment for the comfort and benefit of residents. The annual survey should be developed further, and results published in a format that is usable by residents and relatives. Hot water temperatures should be recorded to confirm compliance with safe working practices.

Situated in Blenheim Terrace, a residential area of Redcar, the house is a three storey Victorian terrace house, which is in keeping with other properties in the road. Accommodation is provided for ten residents in eight single rooms and one double room. Communal space consists of an upstairs lounge, with a dining room downstairs. There is a garden at the front of the house and at the rear an enclosed yard. Residents are able to personalise their rooms according to preference and taste. The home currently charges fees from £398.27 to £442.64 per week. The unannounced inspection took place over two days. During the visits the inspector spoke to six residents, two staff members and the manager. The inspector walked around the building and looked at documentation including resident files and staff records. The home submitted a pre inspection questionnaire. The inspector received responses to the pictorial survey from all ten residents. One relative also returned a survey. The visits to the home lasted a total of five hours. What the service does well: What has improved since the last inspection? There are on going improvements in the décor and maintenance of the home for example some residents rooms have been decorated and radiator covers have been installed as required at the last inspection. The home has also reviewed the complaints procedure and documentation used in the annual quality review to make them more user friendly. The home has recently introduced competency checks in relation to the administration of medication. This judgement has been made using available evidence including a visit to this service. People who may use the service and their representatives have the information needed to choose a home which will meet their needs. EVIDENCE: This standard has been met by the home during previous inspections. The manager told the inspector that there have been no admissions to the home since the last inspection. She described to the inspector the process of information gathering that would take place prior to any admission. She also said that any prospective resident and relatives would be encouraged to visit the home for periods of time before making any decisions. The home ensures people receive a brochure and service user guide. Documentation in current residents files seen confirmed assessments are carried out and are reviewed on a three monthly basis. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. JUDGEMENT – we looked at outcomes for the following standard(s): Outcomes for Standards 6, 7, 8, & 9 were looked at. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Individuals are involved in decisions about their lives and play an active role in planning the care and support they receive. EVIDENCE: During the inspection three residents files were examined. These were found to be well organised. Files were found to contain an action plan detailing care needs, including health, emotional, social, leisure, and educational needs. Records also included details of what residents can do for themselves, their likes and dislikes, any risks and how needs are to be met. One risk assessment in relation to chocking was seen to involve input from the speech and language therapist. Action plans are reviewed as required or at least three monthly. All staff who spoke to the inspector had a good knowledge of individual residents needs, preferences, choices and how these are met. The home uses a key worker system that promotes individual care of residents. Staff who spoke to the inspector talked of residents having the right to be involved in decisions including those around taking risks. Residents who spoke to the inspector and feedback in surveys received confirmed they are supported to make decisions and choices about their lives. For example they can decide how they would like to spend their day, the activities they take part in and contact with family and friends. Residents are encouraged to participate in the daily routine of the home, tidying their own rooms and communal areas. There are regular resident meetings, residents told the inspector that in future they are to lead these meetings, and record their discussions. Comments received included ‘ I like living here’, ‘ its nice here’ and ‘staff are all lovely’. The inspector observed a good interaction between residents and staff. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. This judgement has been made using available evidence including a visit to this service. People who use services are able to make choices about their lifestyle, and are supported to develop their life skills. Social, educational, cultural and recreational activities and relationships meet individual’s expectations. EVIDENCE: Residents attend skills centres, college or work placements during weekdays. Conversation with residents and staff revealed a wide range of activities, which varied from resident to resident. One resident spoke of regularly visiting car boot sales, another of a cookery course and others spoke of shopping and holidays. Residents told the inspector that they are planning a holiday to Euro Disney. Discussions about activities and holidays take place during residents meetings and one to one time with staff. Contact with family and friends is encouraged within the home and residents are supported to be part of the community. Resident’s rooms were seen to be personalised to taste reflecting their interests and preferences. Comments received from residents included ‘I like going away, its fun’ and ‘we go out for walks on the seafront and shopping’. Residents told the inspector that they can pick what they eat and get a good variety either cooked at the home or a take away if they wish. Meal times are flexible to accommodate resident’s choice and activities. Staff were able to demonstrate that special diets can be catered for. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. This judgement has been made using available evidence including a visit to this service. The health and personal care that people receive is based on their individual needs. The principles of respect, dignity and privacy are put into practice. EVIDENCE: Staff were able to demonstrate through response to questions and observed interaction that they promote residents independence, whilst respecting peoples preferences and dignity. Responses in the surveys received by CSCI from residents indicated they feel well cared for and are treated well. Residents who spoke to the inspector confirmed this. During the inspection it was seen that medications are stored and recorded appropriately. All staff who administer medication have received training. The home has recently introduced competency checks in relation to the administration of medication. Evidence in the resident’s files indicated that they have access to GP’s and other health professionals such as district nurses, speech therapist and occupational therapists as necessary. Residents who spoke to the inspector confirmed this. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Outcomes for Standards 22 & 23 were looked at. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People who use the service are able to express their concerns and have access to a complaints procedure, are protected from abuse, and have their rights protected. EVIDENCE: The home has policies and procedures in relation to handling complaints and prevention of abuse. A service user-friendly version is displayed in the home. The inspector was told the home has not received any complaints since the previous inspection. Residents who spoke to the inspector confirmed they could speak to the manager and staff if they had any concerns. Staff who spoke to the inspector confirmed they have received training in relation to prevention of abuse and reporting concerns. All demonstrated through response to questions a commitment to protecting resident’s safety and wellbeing and were able to describe the action they would take if a concern were raised. An audit of residents personal monies held by the home evidenced that the system has been made safer and includes retention of appropriate receipts, two signatures, and a monthly check. Residents are encouraged and enabled to sign for their own monies where possible. A random sample of personal allowances found there were no discrepancies with the balance stated on the record sheet and the actual amount in the individual money envelope. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The environment of the home is generally well maintained, comfortable and safe. EVIDENCE: The inspector walked around the home and looked at a number of resident’s rooms and communal areas. The home was found to be clean, tidy and odour free. Improvements have continued to the maintenance and décor of the home. The dining room, lounge and a number of resident’s bedrooms have been redecorated. A bathroom was being decorated on the day of the inspection. Residents told the inspector they had been involved in the choice of the wallpapers used. Whilst the home offers a comfortable and ‘ homely ‘ environment it would benefit from an ongoing planned programme of refurbishment. The manager told the inspector that further bedrooms are to be decorated, carpets replaced and unused electric fires in lounge areas to be removed. Residents who spoke to the inspector told her they ‘liked’ their rooms. One said ‘Its nice here’ another spoke of picking their own wallpaper. This judgement has been made using available evidence including a visit to this service. Staff in the home are trained, skilled and in sufficient number to support the people who use the service, and support the smooth running of the home. EVIDENCE: The home met the standard in relation to recruitment at the previous inspection. The manager confirmed that no staff have been recruited since then. Residents who spoke to the inspector confirmed that there was sufficient staff to meet their needs and were happy with how this care was delivered. Staff records seen contained evidence of training in relation to Safe handling of medication, first aid, food hygiene, fire safety, no secrets, and falls awareness. Information received in the pre inspection questionnaire indicated all the staff have achieved NVQ at level 2 or 3. Staff who spoke to the inspector confirmed this. Residents told the inspector that staff support them, one said ‘ the staff are lovely’ another said ‘ they look after me well’. This judgement has been made using available evidence including a visit to this service. The management and administration of the home is based on openness and respect, and is developing quality assurance systems. EVIDENCE: Discussion with the residents, manager and staff confirmed that care continues to be provided on an individual basis supported by person centred planning. All residents and staff who spoke to the inspector said that the manager of the home was open and approachable; they felt that the home was well run and they were supported. Issues raised were listened to and acted upon appropriately. One resident told the inspector staff ‘ listen to me’. Evidence both seen and heard indicates the home seeks the views of the residents on a regular basis through meetings and one to one with key workers and other staff. Support workers at day care placements have been asked to help the residents complete this. The manager told the inspector it is planned to publish the results of the survey. Regulation 26 visits take place on a monthly basis and reports were available. Accidents were seen to be recorded appropriately. The manager carries out regular audits in relation to residents personal monies held by the home, accidents, and care planning documentation. Information in the pre inspection questionnaire indicated that the home and equipment are maintained as required, and fire alarms are tested weekly. The manager told the inspector that hot water temperatures are checked weekly and prior to any bathing, however these are not recorded.
