Date of last inspection 9th January 2006 Brief Description of the Service: The Lawns provides accommodation in a modern and well maintained, purpose built two storey brick building. The home has nineteen single bedrooms and two double bedrooms on the ground floor and twenty-nine single bedrooms situated on the first floor. both floors are accessible via a passenger shaft lift. The home is situated in the grounds of Springfield hospital, within three miles of Chelmsford town centre and mainline train station. There is a bus stop outside of the home. The Lawns caters for a variety of people including Older People, Younger Adults with Physical Disabilities and people who have a diagnosed Terminal Illness. This is an overview of what the inspector found during the inspection. This was a routine unannounced inspection carried out on 29th March 2006 between the hours of 08.30 and 13.00. Carolyn Delaney, Lead Inspector for the home, carried out the inspection. Records including care plans and assessments in respect of residents’ needs and risks to health, safety and welfare were examined. Four residents and three relatives were spoken with during the course of the inspection. Three residents were spoken with during the course of this inspection and the relatives of eleven residents at the home were contacted by post so as to offer them the opportunity to make comments about the services provided by the home. A summary of the comments made will be included in the final version of the report. Four members of staff were spoken with during the course of this inspection so as to determine their awareness of the needs and wishes of the people living at the home and the homes policies and procedures. The records in respect of the maintenance of the premises and equipment were examined. Key standards as identified in the intended outcomes sections of this report are inspected at least once every twelve months. Where key standards have not been inspected on this occasion they will have been inspected at the previous inspection. The Lawns provides a well-maintained and comfortable place for people with a variety of nursing and care needs to live. The home is well staffed with people who receive training and support in respect of the roles they are to perform and the needs of the people who live there. Information about the people who live at the home, and their individual nursing and care needs is clearly recorded so that staff are kept up to date with each persons needs and treatment. Residents are supported in making decisions about how they wish to receive their care and treatment and how they wish to spend their leisure time.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3&4 People are only offered a place at The Lawns following a detailed assessment of their specific nursing and other general care and safety needs and taking into account the people already living at the home it is determined that staff can meet these needs. Staff are supported and trained so as to ensure that they can provide care and assistance to the people living at the home. EVIDENCE: There were detailed records available in respect of the assessments carried out before people are offered a place at the home. Nursing staff working at the home had reviewed the practices for accepting emergency admissions where it has not been possible to visit them and carry out an assessment. Staff are aware of each persons nursing and care needs and each individuals preferences for how they wish to spend their days and the deployment of staff facilitates the continuity of care provided to the people who live at the home.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Information about each persons nursing, general care and health and safety needs is well-recorded and kept under regular review. Staff support residents in retaining control of administering their medicines where it is appropriate. Residents receive medicines as prescribed for their treatment. Staff working at the home seem generally caring, however some staff do not always treat residents in an appropriate manner. EVIDENCE: Care plans, assessments and other information recorded in relation to the people living at the home is very well written and kept up to date and accurate. There was evidence that residents are consulted about the way in which they prefer to receive care and treatment and this information is recorded so as to ensure that all nursing and care staff are aware of these preferences. There is a robust system in place for identifying risks to the health, welfare and safety of the people who live at the home. Where risks are identified there are

It was positive to note that wherever it is practicable that residents or their relatives have been involved in the development of care and risk management plans. A number of residents receive oxygen therapy as part of their treatment plan. The records maintained in relation to this treatment are very well maintained. Other records indicated that staff ensure that people living at the home receive the medicines, which have been prescribed for them. Staff were observed to treat residents appropriately during the course of the inspection and those residents who could express their views who were spoken with said that staff were kind and caring. However there have been a number of complaints made in respect of the attitude and care provided by some members of staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, & 14 The Lawns provides a good range of opportunities for residents to participate in meaningful activities. There is detailed information available to staff how residents wish to be cared for and staff generally support and assist residents according to their wishes. EVIDENCE: Residents who were spoken with said that they were happy with the activities provided by the home. The more able residents have opportunities to go out shopping and access activities outside of the home, supported by staff as required. Some residents said that they were looking forward to baking cakes for Easter and others said that they enjoyed exercise classes. Information about how people wish to receive care and treatment is recorded where available as part of each persons plan and relatives are encouraged to be involved in the provision of relevant information. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Records in respect of complaints are not consistently maintained in accordance with the homes policies and procedures and not all relatives are confident that their complaints will be dealt with satisfactorily. Some staff attitudes and practices are not conducive to the protection of residents from harm and abuse. EVIDENCE: The Lawns has a clear policy and procedure in respect of dealing with complaints. There had been a number of complaints received since the last inspection, two of which were made in respect of the attitude and lack of care by some members of staff. While records in relation to the investigation carried out by the deputy manager were detailed there were no records in respect of the outcome, action taken to prevent reoccurrence and the response made to the to the complainant. The complainant had referred their concerns to the Commission for Social Care Inspection at the time of writing this report. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 & 30 Staff are recruited robustly, trained and employed in sufficient numbers so as to meet the needs of the people living at the home. EVIDENCE: Staff were observed to carry out nursing and care tasks in a calm and unhurried manner during the course of the inspection. However there have been a number of complaints made about how some staff treat residents when providing personal care. The staff duty rota indicated that the home employs sufficient staff so as to meet the needs of the people who live there. It was also evident that staff do not work excessive hours without appropriate off duty time. The rota for night duty staff did not include the full names and designation of staff. The employment records for the two people who had been recruited to work at the home since the last inspection were examined. There was evidence that all the relevant checks had been carried out so as to determine the suitability and fitness of each individual to work at the home. All staff undertake mandatory training including safe moving and handling before they commence work at the home. There was information about the training provided for staff working at the home, including mandatory training and specialist training covering catheter care and complimentary treatment in palliative care

Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 The Lawns is maintained to a very high standard in respect of the maintenance, repair and renewal of equipment and systems so as to protect residents and the people who work in and visit the home. EVIDENCE: The home employs dedicated maintenance staff who ensures that the premises and equipment is maintained in good working order. Detailed and well-organised records were available in respect of the maintenance and repair for gas, electrical and mechanical equipment and systems in the home. Staff at the home receive regular training updates in respect of how to deal with an outbreak of fire at the home.
