The Home may also accommodate three persons under 65 years with a learning disability. The Home may also accommodate 1 person aged between 50 - 64 with a physical disability. 6th July 2005 Date of last inspection Brief Description of the Service: The Meadows Nursing home is one of two homes owned by the Worcestershire Care group. It is registered to accommodate a maximum of 36 residents. The home is divided into three units, Pine, Beeches and Willows. Pine and Willow units are on the ground floor and the Beeches unit is located on the first floor and caters for residents who have dementia related illness. A passenger lift provides access to first floor rooms. With the exception of one bedroom all rooms are single occupancy. This is an overview of what the inspector found during the inspection. Two inspectors undertook this inspection. The inspection was unannounced and commenced at 10am. It took place over a period of five hours. The main focus of this inspection was to assess care and medication practice and to review progress on requirements from the last inspection report. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. EVIDENCE: The requirement from the last visit for a Service User Guide to be updated and contain accurate information was not inspected at this time; the inspector is therefore unable to comment if this has been addressed. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, & 10 Individual care plans and risk assessments are of a poor quality and do not reflect the current care needs of residents. Medication is not administered on time, which places residents at risk. EVIDENCE: Care plans were of a poor quality, and care instructions did not reflect current care needs, a number of documents in the care plans inspected were unsigned and did not contain specific dates. One care plan had not been updated since September. Nutritional care plans seen had not been updated, it was noted in one care plan a resident had lost weight and was now deemed at risk however a specific care plan had not been implemented to address this risk. Nursing tools used to identify residents at risk of developing pressure sores had been added up incorrectly, placing residents at risk by not identifying

Risk assessments from various residents care files were inspected these included, a resident who smoked, a resident at risk of leaving the building and a resident at risk of falling these had not been reviewed within the past month and up dated and therefore placed these vulnerable residents at unacceptable risk. Records seen of residents requiring regular blood sugar monitoring for diabetes were not consistent with the care plan. There were a number of signature gaps on the medication records seen and general medication recording was of a poor quality. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them.
