Date of last inspection Brief description of the care home Thornton Manor is a three storey building that has been extended and adapted as a care home for older people and people with a physical disability. There are bedrooms, Care Homes for Older People
Page 2 of 9 bathrooms and communal areas on the ground floor and the first floor. The second floor is used as office accommodation. The home is set in its own grounds in a rural location between Ellesmere Port and Chester. It is close to the motorway network but is not accessible by public transport. There is parking space to the front of the building and gardens to the front, side and rear. Care Homes for Older People Page 3 of 9 What we found:
The home is registered to provide care for frail older people and for people with a physical disability. From discussion with the acting home manager, and from our observation, it was apparent that some of the people living at Thornton Manor had care needs that did not fit the homes categories of registration. One person has very complex needs and challenging behaviour and to the best of our knowledge the home does not employ any staff who have the knowledge, skills and experience to ensure that this persons needs can be met. The care plans we looked at had not been written or maintained to a satisfactory standard and therefore did not provide evidence that peoples needs had been assessed, that the care had been planned, or that the care being given was reviewed. Risk assessments to show how individuals should be kept safe were not in place. The recording of wound care was poor. Some staff have poor written English skills and this makes some of the records difficult to understand. The home employs three nurses who each work 72 hours a week to cover day and night duties. We spoke with one of the nurses who said that she was happy to work these hours but we are concerned that the care provided to residents may be compromised if staff are tired. The rotas showed that some care staff also work 72 hours a week. Recruitment records showed that some new staff have started working in the home before a POVAfirst check (initial check from the Criminal Records Bureau) and written references have been received. Without these basic checks, there is a risk that people unsuitable to work with vulnerable people are working in the home. There were no written records of an induction process for any of these staff, at least one of whom has no previous experience in care work. We can therefore not be sure that they know how to work safely. Accidents were recorded on an accident pad. The pad was not being used properly and accidents had not been assigned a reference number. The format does not allow space for any detail to be recorded and there were no records to show that accidents had been followed up, or any consideration given to the prevention of further accidents. A considerable proportion of the incidents recorded resulted from challenging behaviour from residents. Records showed that a number of incidents had occurred in the home which should have been reported to CSCI, (as required by regulation 37 of the Care Homes Regulations 2001), but had not been reported. This means that we are not receiving confirmation that staff at the home are taking appropriate action to protect residents when serious incidents and accidents occur. What the care home does well:
