Care Homes for Older People Page 3 of 8 What we found:
The reason for this visit was to look at the arrangements for handling medication and to look at the action taken to address the requirement made at our previous visit. The visit lasted approximately two hours and involved discussing the handling of medication with the manager, examination of medicine records and storage arrangements. At the end of the inspection feedback was given to the manager. Overall, we found that although generally suitable arrangements were in place for handling medication, weaknesses in the record keeping need to be addressed to better support and evidence the safe administration of medicines. We looked at medicines administration. Since the previous inspection arrangements have been made to ensure medicines are safely administered from the original pharmacy labeled container. People choosing to self-administer medication are supported to do so, but it was of concern that written assessments and care plans were not completed. It is important that assessments are completed to help ensure people receive any support they may need to manage their medicines safely. The home had made arrangements for the use of homely remedies to enable treatment for minor ailments to be given without needing a prescription. Some were present but not currently being used. Consideration should be given to looking at how the home can help to make sure that they are suitable for use with any prescribed medicines already being taken. We looked at how medicines were recorded and found that it was not always possible to track the handling of medicines in the home because records of medicines received into the home and of unwanted medicines sent for safe disposal were not made. We found that records of healthcare professional advice were not always clearly recorded. One person had a box of antibiotics that had never been started, the manager explained that they had been advised not to start the course unless needed, but this advice was not recorded. Records showing the administration of medication were generally up-to-date but records were not made to show the application of external preparations (e.g. creams) and these were not listed on peoples administration records. We found that there was not always clear information within peoples care plans about the use of these creams. It is important that records of external preparations are completed, as for all medication administered. We looked at medicine storage and found that medicines were safely locked away but appropriate arrangments were not in place for the recording of controlled drugs. The home has previously handled controlled drugs although none were kept at the home on the visit day. What the care home does well: What they could do better:
Medicines record keeping needs to improve to better support and evidence the safe administration of medication.
