Conditions of registration: 1. Staffing to be provided in accordance with the residential forum staffing guidance. 2. A suitably qualified and experienced manager, who is registered with the Commission for Social Care Inspection, must be employed at all times. 3. All further admissions to the home must be of the Dementia (DE) category. 4. The home may continue to accommodate the four service users who are in Older Persons (OP) category. Date of last inspection Brief Description of the Service: Westport House is a care home for 11 elderly people who have dementia. The home is situated in Thornton, near Blackpool. All of the bedrooms within the home have en-suite facilities and there is one shared bedroom. A lift is available to the first floor and the home has a large conservatory at the rear of the house. A decking area has recently been constructed at the back of the house. There is a large convenience store across the road ffrom the home and there are other shops and ammenities close by. Bus services run into Cleveleys, Fleetwood and Blackpool from close by. There is parking available at the side of the house. This is an overview of what the inspector found during the inspection. This inspection commenced at 09.30 and took 6 hours. The Inspector spoke to four staff members, the home- owner and three visitors. The Inspector also spoke at length to two residents. The Inspector also used the information provided by the homeowner in the pre-inspection questionnaire, to gain evidence of the care and service provided. Relatives also provided two comment cards prior to the inspection. What the service does well:
The home provides an excellent service for people who have dementia. This is a home that specialises in the care of people with dementia and the care provided reflects this through specialist training and activities. The residents of Westport House are well looked after. The care staff work well as a team and show a good understanding of the needs of the people living at the home. Care documentation is well written and comprehensive. The home has a resourceful and committed activities coordinator who ensures that appropriate activities take place and that resources in the community were well used. Residents are stimulated, and their minds are kept active through good care planning and committed, well trained, care staff. Visiting arrangements at the home are informal and family and friends of residents are encouraged to maintain contact. Visitors spoken to during the inspection said that the management and staff were very supportive and made sure that they were kept up to date with any changes. The home was very well presented and very clean. All of the rooms have ensuite bathroom facilities. Most of the furniture is new but there is a good mix of traditionally styled furniture in the bedrooms to make residents feel at home. There is a large conservatory at the rear of the house and residents can walk out onto a patio area at the rear. One resident commented, “This is a well run, clean, good home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 4, and 5 Thorough admittance procedures and careful assessment ensures that the home can meet people’s needs. Written information provided to prospective residents is good enabling an informed decision about admission to the home to be made. EVIDENCE: The home’s owner had produced detailed information about Westport House and the service that the home provides. Because of this, prospective residents and their families were able to see if the home was right for them before moving in. The relatives and residents who were spoken to during the inspection also confirmed that they were able to visit the home before making a decision about whether or not it would meet their needs. The home’s owner also produced detailed assessments of residents prior to their admittance to the home. It was good to see that lots of information had been obtained from residents and their relatives about the lives, the families and the jobs that residents had done. This information would help carers within the home to personalise the service offered.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, and 10. Promotion of health is taken seriously. Resident’s welfare is closely monitored and health needs were met. Residents are treated with dignity and respect. EVIDENCE: Individual records were kept for each resident with a plan of care setting out in detail the action that needed to be taken by care staff to ensure all aspects of health, personal and social care needs of the residents were met. Significant events had been recorded and daily entries made setting out the care given. Each plan of care was reviewed on a monthly basis. The residents also had ‘Lifestyle Improvement Plan’ which identified the likes and dislikes of each resident and helped care staff build on the aspects of life that residents found enjoyable. Each resident had a ‘key-worker’ who encouraged the resident to keep these interests up and ensured that interests were continued and recorded when they weren’t there. This system helped to make the care offered more individual. One resident said, “The girls are really nice and always treat me properly”. All of the relatives that were spoken to were very complementary about the home

One relative commented, ”They go out of their way to treat people as individuals.” None of the residents were responsible for their own medication. Systems were in place that ensured that medication was handled correctly and professionally. The records of all the people who received medication were seen and there were no concerns. Each resident had a photograph attached to their record sheet to help prevent mistakes. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 15 Residents are motivated by regular and stimulating activities. Visitors are welcome at any time ensuring personal relationships are maintained. EVIDENCE: The activities available for residents in this home were excellent. A dedicated activities coordinator was employed who showed knowledge and enthusiasm in discussing her role and the variety of activities on offer. The residents who were spoken to confirmed that they thoroughly enjoyed the activities on offer. Activities were specific to residents who had dementia. At the rear of the home there was a ‘Dementia Garden’ where residents could sit out and discuss items of equipment that had been put there (such as an old mangle). There was a ‘fresh air’ rota to ensure that all of the residents went out into the local community on a regular basis. Two residents commented on a recent garden party that had been organised at the home and how enjoyable this had been. There were several visitors to the home during the inspection. Those that were spoken to said that they were always made very welcome and privacy and refreshments were always offered. One visitor commented, “If anything happened to me I’d love to come and live here, its so lovely.”

All of the residents that were spoken to said that they found the food enjoyable. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Arrangements for complaints are handled well and taken seriously ensuring people feel listened to. Procedures for dealing with and reporting abuse were satisfactory ensuring people are adequately protected. EVIDENCE: The complaints procedure was available in the Service User Guide and Statement of Purpose and the procedure was available to residents and visitors in the main part of the home. The carers on duty were aware of the procedure regarding helping residents and their relatives to complain if they wished to do so. Training had been accessed regarding the recognition of abuse, and staff were aware of what action to take should abuse be suspected. The home had good procedures and guidance for staff, as well as a ‘whistle blowing’ policy that protected staff when voicing their worries. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 24, 25 and 26. Residents live in a clean, well-maintained home. Residents’ bedrooms are attractive and homely. EVIDENCE: The communal rooms were comfortable and furniture was appropriate to meet the needs of the residents. The residents’ bedrooms were all individual, reflecting their preferences and contained personal possessions such as photographs and ornaments. The home had a large conservatory to the rear which lead out on to a patio area where residents were enjoying the sunshine. The whole home was tastefully decorated and looked comfortable and homely. All of the ladies that were spoken to said, that the home met there needs both with regard to their own rooms and communally. The home was very clean throughout. There were no unpleasant smells. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 Better support and training for staff means that they are more able to do their jobs correctly, which benefits the residents of the home. Staff morale is high resulting in an enthusiastic workforce that works positively with residents to improve their quality of life. EVIDENCE: The staff rotas for the home confirmed that sufficient staff were employed to ensure that the needs of the residents are dealt with correctly. There were no concerns from visitors and relatives regarding staffing levels and all of the residents that were spoken to said that staffing was sufficient. The recruitment procedures were good, and Criminal Records Bureau checks were seen for people employed at the home. The care staff that were spoken to confirmed that training was regular and they were being instructed about things that were relevant to their roles. Training had taken place in Dementia Awareness and understanding challenging behaviour. Care staff had also been trained in risk assessment and customer care. The home -owner confirmed that over 50 of the care staff had completed National Vocational Training at level 2 or above. This meant that staff were skilled and competent in their roles. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32 and 36 The home is well managed and run in the best interests of the residents. There is good leadership and direction to ensure that residents receive consistent care. EVIDENCE: It was clear that all staff were being supported to do their jobs effectively. Records of individual supervision were available within care staff files. The home -owner had recently completed her NVQ level 4 in management. She already had a nursing qualification, which meant that her mandatory training was now complete. Several relatives commented on how well the home was being run and how welcome they were made to feel when visiting. Care staff were also complementary about the way they were treated and how good the morale was amongst those employed within the home.
