The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

From evidence from previous inspections, the home continues to provide a consistently high standard of care for its residents. Residents and visiting relatives spoken with were unanimous in their praise for the staff and the care they received. The premises are well furnished and decorated generating a comfortable and homely environment for the residents.

What has improved since the last inspection?

The home has employed new permanent care staff since the last inspection, thus reducing the amount of agency staff.

What the care home could do better:

The home should continue to develop its care planning systems and social activities. NVQ training needs to be developed to meet the standard of 50% of staff trained to NVQ level or equivalent.

It is located within reasonable walking distance of all local community services and facilities. Westcliff main line railway station and the seafront are close by. The resort of Southend on Sea is easily available. Westcliff Lodge is registered to provide care for twenty elderly people, no more than three of whom may be suffering from dementia. The home provides sixteen single and two double rooms. All bedrooms, except for one single, have en suite facilities. There are two lounges and a conservatory, parking to the front, and a rear garden for residents to use. The accommodation is on two floors; a passenger lift provides access. This was a routine unannounced inspection, which took place on the 31 August 2005, lasting for six hours. The inspection process included discussions with the manager and senior carer, six residents, three staff, three visiting residents, a tour of the building, inspection of a couple of policies, procedures and records. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1-5 The admission process is well managed by the home. Prospective residents and their families are given adequate information about the home to make an informal choice. EVIDENCE: The home has an appropriate statement of purpose and service users guide, providing comprehensive information on the home. Copies were seen in residents’ bedrooms. Appropriate contracts of residence were in place for each resident. Pre-admission assessments are carried out by the manager to ensure the home can meet prospective resident’s needs. Prospective residents and their families are invited to visit the home before making a decision. This was confirmed by visiting relatives. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7-10 Residents’ personal and health care needs are being met by the home and records provided good evidence of this. Residents are treated respectfully, maintaining their privacy and dignity. EVIDENCE: Care plans seen covered all residents assessed needs and contain comprehensive risk assessments. There was evidence that residents and/or their families were involved in drawing up the care plans. Positive discussions were held with the manager regarding further development to ensure greater clarity of information for staff to meet residents’ needs and to improve the daily records. Care plans evidenced that good use is made of the local health resources. Medication administration was seen to be well managed. The home should obtain information from their community pharmacist regarding shelf life of liquid medication after it is opened. Some residents choose to spend much of their life in their rooms and staff respect this. Residents’ mail is treated confidentially. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 Daily routines were generally flexible. Residents are encouraged to participate in a wide range of activities provided by the home. The home provides a good quality and quantity of food, which provides a well balanced diet for the residents EVIDENCE: A social events calendar has been devised from suggestions made in the home’s quality assurance questionnaires. Although a wide range of activities are offered on a daily basis, staff are finding it difficult to stimulate some of the residents. The home has continued to develop interaction between staff and residents who have dementia and the manager is continuing to investigate relevant activities through various professional organisations. The home operates an open visitors policy. relatives. This was confirmed by visiting Residents and their families have personalised bedrooms with residents’ own possessions. Residents spoken with were complimentary regarding the quality and quantity of food provided. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16-18 The home has appropriate policies, procedures and staff training in place in order to protect vulnerable adults. EVIDENCE: The home has a comprehensive complaints procedure. No complaints have been made to the home. Residents and relatives said they can raise any concerns they have with the manager and they are always sorted out. All the residents are included in the local voters register. One resident has an advocate and details of advocacy services are included in the service users guide. The home has appropriate policy and procedures regarding protecting residents from abuse. All the staff have attended training for the protection of vulnerable adults apart from two staff, who have recently started. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19-26 The home provides a safe, well maintained environment that is accessible to the residents, is homely and meets individual needs. EVIDENCE: The home is well decorated and furnished to a high standard, which creates a comfortable and homely environment. There are adequate adapted bathrooms, showers and toilets to meet residents’ needs. All bedrooms apart from one have ensuite facilities. Bedrooms were seen to be well furnished and evidenced items brought in by residents. Systems are in place for checking the premises on a daily basis and any repairs required are undertaken by the home’s handyman. Residents spoken with were very happy with their bedrooms. The premises was seen to be clean and tidy throughout and odour free. Good hygiene practices are in place and an ozone disinfection system is supplied in the laundry. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,30 Staffing levels and skills are appropriate to meet the needs of the service users. EVIDENCE: The home continues to meet the staffing arrangements agreed with the previous registering authority. These are one senior carer and two carers on each shift during the day and one awake and one sleeping in at nights. In addition a cook and a cleaner are employed. The home has recruited more permanent staff since the last inspection; however, there is still one vacancy, which is covered by agency staff. Staff receive a thorough induction programme, which is to the National Training Organisation standards. Staff are provided with a wide range of relevant training. However, none of the staff have achieved NVQ Level 2. Two staff are awaiting to commence training and NVQ Level 2. Staff spoken with were enthusiastic regarding the training they received and thought they had a good staff team. They confirmed that at present the staffing levels were sufficient to meet the residents needs. Residents commented that the staff were very good, kind and patient. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31-33, 35-38 Staff within the home are well supervised and supported by the manager. Records required to protect residents were generally well managed and securely stored. Health and safety issues are well managed. EVIDENCE: The manager is very experienced in working with the elderly and has managed the home for over five years. The manager is supported by the responsible individual/proprietor. The manager is well regarded by the residents, relatives and staff. She tried to achieve NVQ Level 4 but the local provider withdrew the course. She has now elected not to undertake further training. She holds City and Guilds 3/25 Advanced Management in Care. She attends other training to update her skills. Records seen throughout the inspection were generally well maintained and stored in the office. The home looks after small amounts of residents’ personal money. These were seen to be well managed and recorded. The manager is aware of her responsibilities under the Health and Safety regulations. Daily safety checks are made of the building and weekly checks are in place for the fire prevention systems and the water taps. Safety Certificates were in place for all services and equipment.
