This inspection was carried out on 6th December 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

What has improved since the last inspection?

There is ongoing maintenance and redecoration at the home. The lounge area will be redecorated and have a new carpet before Christmas. Two resident`s rooms have been redecorated and a new roof will stop dampness down one side of the home. The storage of waste is better and the home are creating a recycling area. Residents all now have a support plan for their care and weights are being recorded. There has been some progress on the recording of finances for residents however this and quality assurance reports are still in the development stages.

What the care home could do better:

Residents support plans do not explain the physical needs of the residents in sufficient depth and the recording of medication in a resident`s drug chart was inaccurate. A gazebo is provided in the garden for smokers. This area is cold in the winter-time for those residents that smoke. There was also a lack of handwash and paper towels in the communal toilets that leave residents` and staff unable to maintain hygiene standards.

Westminster House is situated in a residential area of Westcliff on Sea and the premises are in keeping with the houses in the locality. Accommodation comprises of a large lounge with dining area. There are twelve single bedrooms situated on the ground and first floor. There is a five person passenger lift. The home has a non-smoking policy and staff and residents that wish to smoke make use of the garden. The inspection took place over one day and the manager and proprietor were present. The inspection consisted of a tour of the home, talking with staff, residents and others, observing the care given, and reading of documents. A resident’s friend that was visiting the home commented on how good he thought the care was at the home despite the furniture and decor being worn in places. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Residents are assessed by a competent person before they are accepted into the home. EVIDENCE: When a new resident is referred to the home, the home’s manager or proprietor visits the resident for an assessment before they are admitted to Westminster House. There was evidence of an assessment and of a discharge summary from the hospital to support this. There have not been any new residents admitted to the home since the last inspection and the home is currently full. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 9 and 10 Areas of medication practice are unsafe and there is a lack of detail in support plans for physical care, however residents rights and privacy is respected. EVIDENCE: The residents all had a support plan that identified their mental health care. However the support plans did not show in sufficient detail how to manage the residents’ physical care. For example weight loss has been recorded but there is no written plan regarding the steps to take to manage this area of care. The staff were aware of this problem but there is a lack of written guidance on what to do about it. On checking residents’ medication charts it was found that a medication had been given and not recorded. Also the medicine keys were held on the same bunch as the general house keys. This is unsafe practice and the manager was made aware of this immediately. Resident’s were seen to open their own mail and had their own clothes on. Staff reported that residents are seen by visiting professionals in the privacy of their own room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 Relatives are welcomed into the home. EVIDENCE: The home encourage relatives and visitors. A visitor spoken to at the inspection said that he was always welcomed into the home. There is no private visitors room, however families can see their relatives in the residents bedrooms if they want privacy. There is no set visiting times and staff are always available to speak with relatives. The proprietor and a staff member were in the process of organising a trip the next day for one resident to visit a family grave that was many miles away. This trip was meaningful to the resident’s spiritual needs and occurs throughout the year. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Furnishing and décor in areas of the home worn and the smoking area outside is cold. The standard of hygiene in the communal toilets and bathrooms is inadequate. EVIDENCE: The home have invested in a new washing machine and drier for the laundry and the clinical waste is kept secure and out of the reach of the residents. During a tour of the home it was found that the toilets and bathrooms were either without hand wash or paper towels. This leaves residents and staff unable to maintain standards of hygiene. The home are constantly maintaining the property and redecorating. Two residents rooms have been redecorated and the lounge area was in the process of redecoration, however the manager hoped to have this room finished by Christmas. Radiators on the upstairs landing have not been covered. These radiators are not currently used and have not been covered because the covers would protrude too much and block wheelchair access. This area is cold and there is no lighting or heating provided. This was discussed with the manager who will investigate other options for a warmer all weather outside area. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 30 The home are maintaining their commitment to training staff. EVIDENCE: On the day of inspection an NVQ assessor was in the home working with staff. Currently the home has 11 staff out of 17 undertaking NVQ in care. This qualification covers many areas of care including abuse. There was evidence that staff have gone on other training courses such as mental illness. Also any new staff receive a standard induction to the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35 and 38 Safety records were up to date and financial records are improving. The home is working towards a self-review of the home’s performance that will indicate that residents’ views are heard and acted on. EVIDENCE: As yet there is no written evidence that the home have undertaken an audit for the purpose of quality assurance. This is an area where the home are going to develop a format to follow which will allow them to measure their success in meeting the aims and objectives set out in their statement of purpose. Financial records for the residents were in the home however the documentation of these still did not provide a clear audit trail. Not all expenditure had a receipt to accompany it and the written records were not clear. A sample of safety records was checked. Portable appliance testing is now due. A service certificate for the hoist was unable to be found which the Westminster House Care Ltd DS0000062663.V272877.R01.S.doc Version 5.0 Page 16 manager will follow up. Other safety records including, gas fire equipment and the electrics were all up to date. The fridge and freezer temperatures were also checked and it was noted that the fridge temperatures were rising above 5 degrees Celsius. This was reported to the manager who agreed to monitor these records to ensure that food was stored at the correct temperature.
