Conditions of registration: To admit 1 named person with dementia Date of last inspection 17/03/05 Brief Description of the Service: Westwolds is a purpose built single storey home set in the quiet village of Burton Joyce. It can offer accommodation for up to 33 older people within the category of old age only. A variation for registration has been agreed for one named service user with Dementia to be admitted to the home. There are two service users below 65 who have lived there for a number of years; the certificate does not reflect this, however a variation has been applied for. The accommodation is in single rooms with the exception of one double, currently being shared by a couple. All but six of the rooms have ensuite facilities; fourteen rooms have kitchenettes to enable independant living for those service users who are able. The centre of the village is a short walk away and provides access to shops, churches, public houses, a coffee shop and a supermarket. Service users can access public transport in the village, although there is a mini bus available, which belongs to the home. This is an overview of what the inspector found during the inspection. The inspection took place in one day. Four service users care files, four staff personnel files and other related records with regards to service users care and the environment were examined. One relative, three staff members and four service users were spoken with. What the service does well: What has improved since the last inspection? What they could do better:
The home has a dedicated training programme in place, however staff are required to undertake first aid training to ensure the health safety and welfare of service users. It is recommended that the pooled bank account system is reconsidered to ensure service users finances are securely safeguarded. Although staff are

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4 The statement of purpose ensures prospective service users have the required information to make an informed choice about entering the home. Service users may be assured the home is able to meet their needs. EVIDENCE: The statement of purpose is available in every room and is also located in reception for prospective service users and relatives to view. It covers all the requirements of this standard and is in an easy to read format. The manager or the care manager prior to prospective service users being admitted to the home carries out the preadmission assessments in the community. The assessment covers the requirements of the standard and evidence was available to demonstrate necessary equipment and adaptations are obtained prior to admission. Specialist services are accessed and offered within the home. Staff spoken with were able to discuss service users needs. Staff training is of a good standard and covers all mandatory areas. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 Service users health, personal and social care needs are met and they are enabled to make decisions about their lives with assistance as needed. Specialist input ensures there is relevant access to health care services. EVIDENCE: Assessments to ascertain individual needs based upon the activities of daily living are completed to underpin the plan of care. Care plans addressed highlighted needs and were personalised and individualised. Appropriate risk assessments were in place, these corresponded with the care plan. There was evidence of service user involvement and that choices and preferences are taken into account. Reviews take place monthly to ensure service users are satisfied with care received. Appropriate specialist services are accessed such as the dentist; optician and tissue viability nurse to ensure health care needs are met. These services are offered within the home and also within the community. Service users are facilitated to access these and there is a mini bus available to assist this

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 Service users are enabled to live the lifestyle that matches their expectations and preferences. The home is flexible and well managed which enables choices and control of service users to be upheld. EVIDENCE: There was evidence within plans of care that service users expectations, preferences and capabilities are documented and considered. On speaking with service users they substantiated that the routine within the home was flexible and they were happy and comfortable with life. They discussed that choice was offered at tea times and meals were good. There are 14 rooms within the home, which have kitchenettes, some service users use these facilities and remain independent with regards to making their own breakfast and tea and when entertaining visitor, thus enabling independence as they wish. There is an activities coordinator employed and activities are offered should a service user wish to participate. A local vicar visits the home to provide Holy Communion to those who require it. There are no restrictions on visiting and service users can choose who they see. There is not a specifically designated private area to receive visitors, however the manager stated that the sitting area adjacent to the dining room is available in the afternoon, however most service users chose to use their own rooms. One relative spoke with spoke highly of staff and the standards of care. There was evidence of a good rapport

Service users are facilitated to attend functions within the community. Advocacy services are available through age concern, however no one uses these at present. Service users are encouraged to bring in personal possessions and service users rooms were extremely personalised. JUDGEMENT – we looked at outcomes for standard(s) 16 Service users may be confident complaints and concerns will be taken seriously, listened to and acted upon. EVIDENCE: There have been no complaints received. The manager states she has an open door policy, is approachable and maintains relations with service users and their relatives so they feel they can discuss any issues with her. Service users spoken with stated they had no complaints. Staff were able to discuss how they would deal with any complaints should they receive them. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 Service users live in a safe, clean and well-maintained environment. EVIDENCE: Westwolds offers a comfortable and relaxed environment, it is centrally located within the village of Burton Joyce and is near to local amenities. There are specialist adaptations and facilities available. A routine maintenance programme is maintained and evidence of this was available as kitchens are being replaced in the room with kitchenettes. The grounds are well kept and there is a large garden to the rear. The home is wheelchair accessible. The premises were clean and hygienic and systems are in place for the prevention of the spread of infection. Staff spoken with were able to discuss the policies and procedures. JUDGEMENT – we looked at outcomes for standard(s) 30 Service users can be assured staff are trained and competent to do their jobs EVIDENCE: There are two training coordinators employed who deliver training to all employees. There is an in depth training plan and statistics for training were observed. The induction programme is in depth and covers the requirements of the standard. Staff spoken with stated that the training programme is good and they are supported in their development, it was noted however that first aid training now requires updating. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35,36 Service users can be assured financial interests are safeguarded, however this may be compromised by the use of the pooled account. Staff are appropriately supervised ensuring continued quality care. EVIDENCE: Service users are enabled to control their own finances if able. Personal allowances are also kept on the premises. Those checked were accurate, written records of transactions were maintained which two members of staff signed, receipts for transactions were also available. If large amounts of money are accrued, money is deposited in to a main bank account, where separate records are kept. This is then transferred if again a large amount accrues into service users individual bank accounts. This system is used so service users may access money more readily. The manager is no longer

Staff supervisions were evidenced to take place for fifteen members of staff, staff spoken with substantiated these take place and they are based upon best working practice and work performance. The manager stated she is looking at improving this system as on occasion informal supervisions take place without records being maintained.
