The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

This home provides its service users with lots of opportunities to join in activities that are designed to stimulate and be of interest. The residents say that the staff are all friendly and know their individual needs and circumstances well. The residents also say that the building and its furnishings are pleasant and these could be seen to be in good condition.

What has improved since the last inspection?

Since the last inspection the home has developed its statement of purpose as well as its service user guide so that both contain all of the information that should be available to service users before and after they decide to live at the home. The information being obtained about people before they come to live at the home is being fully recorded and shows why the home had decided that it could meet the needs of the individual concerned. The records now show how those needs should be met. The home has also developed a training plan that identifies the training that staff need in order to meet the needs of the residents.

What the care home could do better:

The quality management of the home must be improved, not only to comply with the requirements of the Care Homes Regulations 2001, but also to ensure that errors like those found in the recording of the controlled drugs are quickly identified and addressed and to generally check that the service that is being received by the residents is of an acceptable standard.

Date of last inspection 29th November 2006 Brief Description of the Service: White Lodge is a large adapted building currently providing personal care and accommodation for up to 36 older people. The home is located in a rural position on the A 442 between Bridgnorth and Kidderminster offering both single and double bedroom accommodation. A large communal lounge and separate dining room are situated on the ground floor with a further lounge/dining area on the second floor. The home is owned by Oldfield Care Ltd with White Lodge being one of a number of care homes within this company. Weekly fees range from £430.00 - £ 450.00. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, and 3 Quality in this outcome area is good. Prospective residents and their representatives have the information needed to choose a home which will meet their needs made available in the form of a completed service user guide and statement of purpose. A robust admissions procedure ensures they have their needs assessed and they will have a contract which clearly tells them about the service they will receive. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The records of a person who had recently been admitted to the home were looked at and these showed that an assessment had been carried out that provided the home with the information that enabled them to formulate a plan of how they would meet that person’s needs. White Lodge Residential Home DS0000067278.V337577.R01.S.doc Version 5.2 Page 9 Those records also contained a signed copy of the care contract between that person and the home which showed what services were paid for and by whom as well as what was expected by both parties in honouring the agreement. A service user guide and a statement of purpose were also seen that outlined what both prospective and current service users could expect from the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome area is adequate. The health and personal care that a resident receives is based on their individual needs. These have been detailed in care plans that show how they should be met, however shortfalls in the management and administration of medication means that it cannot be said that this is done safely. The principles of respect, dignity and privacy are put into practice. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The records of a further two service users were examined. The care plans contained the information about their physical, mental and social needs and how they should be met. These care plans were seen to have been reviewed on a monthly basis. Manual handling risk assessments were seen to have been carried out as had risk assessments relating to behavioural issues. White Lodge Residential Home DS0000067278.V337577.R01.S.doc Version 5.2 Page 11 The home is regularly visited by District Nurses, one of which was there on the day of the inspection. She was complimentary about the way residents were cared for at the home. The management and administration of medication was looked at and it was seen that it is stored in a dedicated medication room that is equipped with hand washing facilities, a purpose designed drugs trolley that is locked to the wall when not in use, a controlled drugs cabinet and a medication fridge. The records relating to the medication were looked at and it was seen that a number of the controlled drugs that were found in the controlled drugs cabinet could not be accounted for in the records. One dose was also found to not have a pharmacist’s label that identified who it was intended for. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome area is good. Residents are able to choose their life style, social activity and keep in contact with family and friends. Social, cultural and recreational activities meet resident’s expectations. Residents receive a healthy, varied diet according to their assessed requirement and choice. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home employs a member of staff whose job title is activities organiser. Her job is to organise communal activities for the residents to take part in. Looking at the records that she maintains it could be seen that the home identifies what type of activities that individuals like and then organises activities that are designed to meet those wishes. Each person’s participation is then noted with a view to either developing those activities or not. Talking with a number of the residents established that most enjoyed joining in with what was organised and those that did not said that they appreciated the opportunity. White Lodge Residential Home DS0000067278.V337577.R01.S.doc Version 5.2 Page 13 Activities such as quizzes, card making, a session called ‘physio fun’ and craft session are amongst the activities offered. The Activities Organiser said that she is currently organising a trip to a safari park. The manager said that the home has access to a local community car and voluntary services minibus scheme. The Statement of Purpose states that a local vicar visits fortnightly and this was confirmed by some of the residents. The religious preferences of each resident was seen to be recorded in their records as was the name by which they liked to be known. Throughout the inspection a number of relatives were seen entering and leaving the home and being made welcome by the staff. Staff were seen knocking on doors before they entered rooms out of respect for the occupants privacy. The bedroom doors were all seen to have an appropriate style of lock although most of the residents spoken to said that they did not feel the need to leave their rooms locked. A number of the service users spoken to were very complimentary about the food provided by the home. They said the quality of the cooking and the size of the portions was always good. Looking at the records showed that the menus were traditionally ‘British’ but varied. The meal seen on the day of the inspection looked and smelled appetising. Talking with the cook established that she was aware of any special medical dietary needs as well as individual preferences and was therefore able to offer suitable alternatives when necessary. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Quality in this outcome area is good. Residents have access to a robust and effective complaints procedure, are protected from abuse and have their legal rights protected. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home was seen to have a complaints procedure and this was included within the service user guide. The manager stated that no complaints had been received since the last inspection. A copy of the local policies and procedures for the safeguarding of adults was seen in the carers’ office. Staff confirmed that they were about to receive training on this subject in the near future. The manager confirmed that no referrals into these procedures have been necessary. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is good. The physical design and layout of the home enables residents to live in a safe, well-maintained and comfortable environment that encourages independence. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Although sited in a rural area the White Lodge is easily accessible from a nearby main road and has plenty of car parking. The home is furnished, decorated and maintained to a high standard. When talking to residents they spoke of their satisfaction with their bedrooms as well as the communal areas. There is a seven bedded unit on the second floor and when asked the residents using the lounge in that area said that they found the area more personal and to their liking. White Lodge Residential Home DS0000067278.V337577.R01.S.doc Version 5.2 Page 16 All of the bedrooms looked at contained lots of personal items of decoration and furniture with the furniture that had been provided by the home being in good condition. All areas of the home were seen to be clean and a well equipped laundry room was sited in the basement. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Quality in this outcome area is good. Staff in the home are now trained, skilled and in sufficient numbers to fill the aims of the home and meet the changing needs of residents. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Talking to the service users as well as observing the care that they received showed that there were adequate numbers of staff on duty. Looking at the staff rotas and talking to the staff showed that the numbers on duty at other times were the same as those seen during the inspection. The services of catering and maintenance staff were also available along with those of an Activities Organiser making more time for the care staff to look after the health and personal care needs of the service users. The staff team reflected the same ethnic mix as the service users. Looking at the records of the staff training and talking with staff established that a training programme was in place that would give staff the skills to meet the care needs of the people living at the home. This included NVQ training to a number of levels. The records of staff who had recently started at the home were looked at and these contained evidence that all of the checks had been carried out that are White Lodge Residential Home DS0000067278.V337577.R01.S.doc Version 5.2 Page 18 required to ensure that people who are employed by the home are fit to work with vulnerable people. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38 Quality in this outcome area is adequate. An effective quality assurance system has not been developed and although a number of routine safety checks are carried out regularly the safety of the service users cannot be said to be fully monitored. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The manager is in the process of applying for registration with the Commission for Social Care Inspection. No evidence was seen during the inspection of the further development of quality assurance procedure for the home which would include a range of measures such as satisfaction surveys or the checking of controlled drugs. White Lodge Residential Home DS0000067278.V337577.R01.S.doc Version 5.2 Page 20 The manager stated that the monthly visits required when the registered provider is an organisation had not been carried out. The management of hazardous substances within the home was looked at. The storage was seen to be in the basement and therefore inaccessible to the service users but the risk assessments for their safe storage and use had yet to be developed. Various records relating to safety monitoring such as portable appliance testing and hot water temperatures were seen to be maintained with action being taken over issues identified as necessary. Accident record were seen to be kept within the individual’s files and contained notes about any actions needed as a result of an incident. An officer from the Shropshire Fire and Rescue Service visited during this inspection and although some minor recommendations were made she felt that, in respect of fire safety, the home maintained a safe environment for the service users. The manager stated that the home does store small amounts of cash for service users. Ledgers for each person were seen and the manager said that she balances these every week to ensure that they are accurate.
