Staffing Notice issued 4 March 1999 Date of last inspection 21st June 2005 Brief Description of the Service: Whiteoak Courst Is a two storey nursing home in Chislehurst registered to provde nursing care for up to 27 elderly persons. This is a well maintained building and a larger than average property for this size home. Accommodation in the home includes bedrooms on the ground floor and the first floor. There is a lift to the upper floor. All bedrooms are fitted with a hand basin. On the ground floor there is a large dining-room and separate lounge area. There is car parking to the front of the building and a garden to the rear. The Home is staffed throughout the 24-hour period with both Nursing and Care staff. There is always qualified nursing staff on duty. The Home has the regular support services of a GP, chiropody, optician and dentist. This is an overview of what the inspector found during the inspection. This was the second unannounced inspection for the year 2005/2006 carried out in the presence of the Manager. Previous inspections have always found the home to be well maintained and offering a good quality service and this is confirmed at this visit. All recommendations from the previous inspections are complied with promptly. The inspection took place over four hours and included a tour of the premises, the assessment of a sample of care plans, medicine charts, complaints and compliments and a sample of health and safety records. The inspector spoke to the staff group that was on shift that morning, the activity worker and three residents. The registered manager has been in post for some years now and the staff turnover within the home is low. The Commission has not received any complaints about this service since the last inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 3, 4, 5 The pre-admission assessment undertaken with potential service users is comprehensive, so that service users can be sure that the home will meet their needs. EVIDENCE: In a sample of care plans that were assessed there was evidence that a preadmission assessment is undertaken with potential service users prior to admission to the home. The assessment covers both physical and psychological needs and this forms the basis of the care plans. It also includes their lifestyle before moving into the home and this give staff an insight into the service user’s current needs. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 Residents are sure that they will be treated with respect, which encourages them to retain their individuality and dignity EVIDENCE: Each resident has an individual care plan and the inspector looked at a sample of these during the inspection. Each care plan contained information that included risk assessments, monitoring of risks that could lead to incidence of pressure sores, and manual handling protocols. Staff complete daily entries of care given and there was evidence that care plans are regularly reviewed. The remaining standards were assessed at the previous inspection and were met. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 The activities within the home are tailored to meet the needs of service users. Visitors are always welcome so that links with friends and families can be maintained and encouraged. The menus were evidence of nutritious and wholesome diet. EVIDENCE: The range of activities on offer in the home have increased since the last inspection and the appointment of an activities organiser. Visitors are always welcomed to the home and one visitor with whom I spoke said that the staff always made her feel welcome and that her husband has shown such marked improvement in his behaviour since being placed at the home. The lunchtime meal was observed and was well presented and looked appetising. All those spoken to agree that the food was varied and enjoyable. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17, 18 There is an appropriate complaints procedure in place. Service Users are confident that their complaints will be taken seriously and acted upon. EVIDENCE: There is a complaints procedure in place and service users are given a copy. The complaints book had no record of any complaints since the last inspection. The Commission has received no complaints since the last inspection. Compliments are also recorded and there have bee several made since the last inspection. Most of these commended the staff for their care and patience. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 25, 26 The home offers an exceptionally well-presented and maintained environment that meets the needs of its residents and allows them to live in a comfortable homely atmosphere. EVIDENCE: The home is very well maintained both internally and externally and attractively furnished and decorated. There is an on-going programme of redecoration and renewals. Each time a room becomes vacant it is redecorated. Eight rooms have recently been redecorated. Nine rooms have had new carpets. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 30 Staffing levels are sufficient to meet the needs of the service users. Training for staff is given priority ensuring that service users can be confident that staff with appropriate skills and qualifications is caring for them. EVIDENCE: Rotas seen indicated that there are always sufficient numbers of staff on duty to meet the needs of the residents. Approximately 90 of staff are now qualified to at least Level 2 NVQ. Training for all staff is a priority in this home and schedules for some future training were seen. Staff files for two members of staff were seen and were in order. Evidence of Criminal Records Bureau checks was in place and work permits had been obtained where necessary. The remaining standards were assessed at the previous inspection and were met. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 36, 37, 38 There are generally reliable systems in place to ensure the good health and welfare of residents. EVIDENCE: Mandatory training is all up to date. Fire equipment, electrics and other adaptations have been checked recently. Records are clear and regularly updated and stored appropriately. The quality assurance audit system is in place. All residents are able to participate in some way and letters are also sent to relatives. The business plan and accounting systems suggest the home is financially viable. Water outlets are regulated and radiators are covered. Supervision is ongoing and meeting the required standard. The Matron meets with staff every morning and the workload discussed at lunchtime. Liaison nurses are quite supportive to the staff team and provides good back up for the home.
