What the care home does well

The home provides a homely atmosphere for residents. The staff know the individual needs of the residents and the relationship between staff and residents is good with staff being kind and courteous. The residents are very happy with the care they receive. "The carers do a damn good job" was one comment made and "the girls are like daughters to me"

What has improved since the last inspection?

The home works with health professionals to ensure service users receive the care they require and have access to equipment if necessary. Care plans now document any reviews by health professionals. A risk assessment has been carried out for a service user who uses an electric wheelchair.

What the care home could do better:

The fire risk assessment of the building should be more comprehensive in its content, used as a working document and reviewed on a regular basis.

The home is approximately four miles from the small town of Wragby and ten miles from the historic city of Lincoln. The home stands in its own grounds and gardens with car parking facilities to the front. The home has two floors and there is a stair lift to the bedrooms on the first floor. There is a variety of aids and adaptations around the building to allow residents to move round the home more independently. Twelve of the bedrooms are single, two of them have an en-suite toilet. There are five communal toilets and two communal bathrooms. The inspection took place over three hours and was the second of two statutory inspections for the year 2005/2006. Three residents were “case tracked” – a system which looks at the needs of the resident and follows this through by talking to the residents concerned and the staff who deliver the care, as well as observation of care practices. Two members of staff were spoken to and the owners. A sample of regulatory records and policies and procedures were seen and a tour of the premises took place. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 14 Service users are offered choice and control over all aspects of their lives and are provided with a variety of activities, which they can participate in if they wish to. EVIDENCE: One service user stated that they didn’t think that they could do what they wanted to whilst others knew that they could do as they wished and felt that they had choices about every aspect of their lives including where they had their meals, what time they rose in the mornings and what time they went to bed. One of the service users listed the sort of entertainments that are offered in the home; these included sing-a-longs, professional entertainers and indoor games. Inspectors were also informed that the residents had been taken on an outing to Skegness in recent times and they had enjoyed a picnic. A local vicar visits the home every month and a service user stated that that they enjoyed the service very much. On the day of the visit the inspectors were able to watch some of the service users enjoying a session of “hand-ball” with a large balloon, which they were enjoying. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 Service users live in safe, clean and pleasant surroundings, with staff understanding the need for infection control. EVIDENCE: Since the last inspection, some bedrooms have been upgraded with decoration and new furniture. The owners stated that they had prioritised this and would eventually upgrade every area of the home. Three bedrooms were observed and these were very personalised by the service users with pictures, ornaments and memorabilia. One service user commented that since the owners had bought the home they had been very busy and they had updated the plumbing and changed some toilets. They thought that their room would be on the list for upgrading soon. Observation of the home evidenced that all areas were free of obvious hazards and the home was well maintained with no malodours present. Staff on duty had received training on infection control. One service user stated that their clothes are laundered on a regular basis and that “they always come back clean” Willan House C53 C04 S59327 Willan House V245637 180805 Stage 4.doc Version 1.40 Page 13 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 Staffing levels are appropriate for the current needs of service users. The home has a thorough recruitment process, which protects service users and staff have the knowledge and competencies to care for them. EVIDENCE: All service users spoken to stated that they felt the staff met their needs and that they knew all about them as individuals and therefore knew what to do. Staff stated that the owners did “on-call” and were “always happy to help” if they needed them. Furthermore, that all staff worked as a team and that there was excellent communication between individual staff members. The owners employ separate cleaners and catering staff and will stand in and undertake any task when necessary. The staff knew the service users extremely well, understanding their individual needs and how to manage them. On the night of the inspection, one owner was going to be on duty in the home with the other being on-call in the adjoining house and available if necessary. Service users appeared relaxed and happy during the inspection and were being appropriately cared for by the staff on duty. The recruitment records of two staff were examined and showed that all the information was present to evidence a thorough recruitment process in order Willan House C53 C04 S59327 Willan House V245637 180805 Stage 4.doc Version 1.40 Page 14 to protect the service users. Recruitment policies and procedures were in place. Training records for staff indicated that courses had included abuse awareness, fire lectures, health and safety, basic food hygiene and information regarding Alzheimer’s: the last of which staff had found to be very useful. A staff training plan for the year is held on file at the Commission’s area office. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35 and 38 The home is run in the best interests of the service users and their financial interests are safeguarded. Service users and staffs’ health, safety and welfare are protected by the policies, procedures and practices of the home. EVIDENCE: One service user stated that the owners hold residents meetings once every three months in the main lounge and one of the carers takes the minutes so that everyone knows what’s been said. Relatives can also attend these meetings. The service user stated that they had never had any need to complain about anything, they said that the owners would “do something” if someone had a problem. Staff stated that they could approach the owners, say what they thought and get a favourable response. Willan House C53 C04 S59327 Willan House V245637 180805 Stage 4.doc Version 1.40 Page 16 Staff also spoke of the fact that the home is run “for the residents” The owners stated that they do not keep money for any resident; the families concerned handle this. One service user stated that the owners would look after their money if you wanted them too as well as any valuables. They would put it into the safe and give you a receipt for it. Policies and procedures were seen for infection control, food hygiene, and fire. Other documents examined included the servicing of the stair-lift and bath hoist. A reputable company disposes of clinical waste. A risk assessment has been undertaken for a service user using an electric wheelchair. The home’s fire risk assessment was not available during the inspection and was faxed to the inspectors following the visit. This was seen to be fairly brief and it has been recommended that this is more comprehensive in content and that it is used as a working document and reviewed on a regular basis.
