The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Willowdale Lodge has a dedicated team of staff, many of whom have worked at the home for some considerable time, which helps ensure continuity of care for the people who live at the home. Information about the needs of the people living at the home is recorded clearly and kept up to date so as to ensure that all staff working at the home are aware of each persons needs and how these needs are to be met. Staff treat residents with dignity and act so as to meet their wishes about how they want to live their lives. Visitors to the home are made to feel welcomed and staff are available to speak with them and keep them up to date with any changes in residents condition. Relatives who were spoken with said that they could not speak highly enough about the staff and the care provided by the home.

What has improved since the last inspection?

The recording of information about residents has continues to improve so as to ensure that each persons needs are met. The staff and management of the home continue to work with the Commission for Social Care Inspection to maintain and improve standards of care provided by the home.

What the care home could do better:

The practices for storage of medicines at the home must be reviewed so as to ensure that all medicines are stored securely and to minimise the risk of mishandling of medicines at the home. In order to protect the people living at the home from potential harm all staff recruited to work at the home must be subjected to the appropriate checks including Criminal Records Bureau (CRB) disclosures and PoVA First checks before they commence employment at the home. The arrangements in respect of sluicing soiled material and equipment needs to be reviewed and appropriate hand washing facilities should be provided in the sluice room so as to promote appropriate infection control. This is being addressed and there are plans to modify the sluice facilities next year.

Date of last inspection 27th July 2005 Brief Description of the Service: Willowdale provides nursing care and accommodation for up to fourteen older people, including up to a maximum of seven people who have a terminal illness. The home is situated in a residential area of Southend on Sea, close to shops, transport and the seafront. The home provides five shared bedrooms and four single bedrooms, a lounge area and dining room. Residents also have accesss to a rear garden. This was a routine inspection carried out between 12.00 and 15.00 on 7th December 2005. Records including assessments, care plans, daily care notes and risk assessment documents in respect of three people living at the home were examined. Two residents and one relative were spoken with during the inspection. Five members of staff including the manager were spoken with and a number of records including policies and procedures in respect of the day-to-day management of the home were examined. Key standards as identified in the intended outcomes sections of this report are inspected at least once every twelve months. Where key standards have not been inspected on this occasion they will have been inspected at the previous inspection. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3 & 4 Prospective residents needs are fully assessed and they and / or their family and representatives are invited to visit the home so as to ensure it is suitable to meet their needs. Residents and their representatives are provided with up to date information in respect of the services provided by the home. EVIDENCE: Detailed assessments of prospective residents nursing, care and welfare needs are carried out and documented prior to each individual being offered a place at the home. Prospective residents and or their relatives are offered the opportunity to visit the home before making a decision to move in. Once residents move into the home they are provided with written and verbal information in respect of the services provided and are issued with a contract of terms and conditions in respect of the placement. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8 & 9 Information in respect of the nursing, general and safety needs of the people living at the home is recorded in detail, reviewed and updated regularly so as to ensure that all staff are aware of each individuals needs and how these needs are to be met. The storage of medicines, which are to be disposed is not consistent with the homes policies and procedures and current legislation in respect of the handling of medicines. EVIDENCE: There were detailed care plans in place, which clearly described each individuals nursing and general care needs in respect of daily activities of living. It was positive to note that wherever possible residents wishes and preferences were recorded as part of the care plan. Care plans were regularly updated and daily care notes were well maintained and recorded up to date information about resident’s daily activities. Willowdale Lodge DS0000015566.V253328.R01.S.doc Version 5.0 Page 11 Risks to resident’s health and wellbeing such as risks associated with the use of equipment such as hoists and bedrails and risks of injuries from falls were carried out and detailed plans written in respect of managing and minimising risks to each individual. It was positive to note that where appropriate that information and advice was sought from other healthcare professionals including Tissue Viability Nurse specialist and information was recorded within the individual’s plan of care. Records in respect of the administration of medication were not assessed during this inspection. There have been no issues identified in previous inspections. It was noted that medicines, which were due to be returned to the pharmacist, were stored in an open box in the office. The manager was advised that this practice was not acceptable and that all medicines must be stored appropriately. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 & 14 Staff working at the home provide a range of activities for the people who live there which are suited to their needs and take into account their wishes and preferences. Resident’s relatives and friends are welcomed and residents are supported wherever it is possible to make decisions about how they live their lives. EVIDENCE: Staff were observed to interact in a positive way with residents. On the day of the inspection residents were offered a choice of video film to watch. There was evidence that the home provides external entertainment events and organise parties for residents. Reflexology and massages are offered to residents on a weekly basis at no extra cost The majority of residents living at the home are limited in respect of the activities they can participate in due to their medical conditions. The manager said that staff do not consistently record the activities provided to residents and that this was being addressed. Residents families and friends are encouraged maintain contact and are made to feel welcome when they visit the home. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Willowdale Lodge has a detailed and robust policy and procedure in respect of dealing with any complaints received about the services provided by the home. EVIDENCE: There had been one complaint made to the Commission for Social Care Inspection since the last inspection. This was investigated by the lead regulation inspector and was not upheld. Staff and management at the home receive and deal with complaint in a positive manner according to the homes policies and procedures. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,25 & 26 EVIDENCE: The communal areas were decorated for the festive season and were warm and comfortable. Residents said that they were happy with the accommodation provided by the home. The home has suitable equipment to meet the needs of the people living there, including lifting equipment, specialist pressure relieving mattresses and syringe drivers. It was positive to note that advice and support is sought from the relevant healthcare professionals so as to ensure that the most appropriate equipment is obtained and provided to meet the needs of the people living at the home. The home was noted to be clean and free from unpleasant odours. There are dedicated cleaning and maintenance staff employed so as to ensure that the environment is maintained in a comfortable, safe and homely manner. Willowdale Lodge DS0000015566.V253328.R01.S.doc Version 5.0 Page 16 The home’s sluice facilities do not have hand-washing facilities for staff for when they handle soiled materials. There are plans to purchase a new sluicing machine and to install a hand basin during the next year. The regulatory requirement identified in respect of this at this and previous inspections will be brought forward for assessment at the next inspection. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 & 30 Staff are employed in appropriate numbers so as to ensure that the needs of the people living there are best met. Staff are not consistently recruited to work at the home in a robust manner so as to ensure the protection and welfare of the people who live there. Staff working at the home are supported and trained so as to enable them to meet the needs of the people who live there. EVIDENCE: It was positive to note that while the numbers of residents living at the home had been reduced that there had been no reduction in the numbers of staff employed to work at the home. Staff duty rotas indicated that staff do not work excessive hours and that all staff have appropriate off duty time. Willowdale Lodge benefits from a core group of staff who have been employed at the home for a number of years, which ensures continuity of care to the people who live there. One member of staff was undertaking National Vocational Qualification (NVQ) in care and the manager said that funding had been secured through a local college and that a number of staff were due to enrol to undertake this training early in the new year. Willowdale Lodge DS0000015566.V253328.R01.S.doc Version 5.0 Page 18 There was evidence that staff receive regular training and support both from one of the homes senior nurses who is a qualified assessor, and training provided by external organisations including the local Primary Care Trust (PCT), local hospice and training organisations. Three new members of staff had been recruited to work at the home since the last inspection. There was evidence of each individual’s identity, information in respect of each individual’s previous employment history and information in respect of their general health so as to determine their suitability to work at the home. For two of the three people who had been recently employed a Criminal Records Bureau (CRB) disclosure and PoVA First check had not been obtained prior to them being employed at the home. The manager said that these staff were supervised, however she was advised that all checks as required by current legislation must be undertaken prior to staff being employed at the home. It was noted that while prospective staff do attend interviews to assess their suitability to work at the home that no records are maintained in respect of these interviews. The manager was advised that it would be good practice to keep records in respect of staff interviews. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 35 & 36 Willowdale Lodge is managed so as to best serve the interests of the people who live and work there. EVIDENCE: A team of dedicated nursing, care and domestic staff supports the homes manager. The manager continues to work with the Commission for Social Care Inspection and others to improve and maintain standards of care at the home. Residents are offered secure lockable storage if they chose to keep monies and / or valuables in their rooms. Alternatively residents may hand over monies and valuables for staff to hold on their behalf. Records in respect of the receipt and expenditure of monies held on behalf of residents are maintained up to date and checked on a regular basis. Records in respect of staff supervision were not assessed on this occasion. It was noted that there were no records available in respect of the induction provided for the three new members of staff employed at the home. This was discussed with the homes manager.
