What the care home could do better:

To ensure care plans tell staff about people`s preference of when they wish to wake up. To ensure the environment is suitable to enable people to have a restful sleep by not having the light and television on whilst they are sleeping. Unless this is the person`s expressed wishes.

key inspection was carried out on 27 January 2009 Woodcote Hall is a care home for older people, which also provides nursing care for individuals with complex nursing needs due to their physical health condition The home is located in a rural setting and is not easily accessible via public transport and amenities such as shops, Care Homes for Older People Page 2 of 8 2 7 0 1 2 0 0 9 Brief description of the care home pubs and restaurants are situated in Newport Town a short drive away The two storey property is situated within its own grounds, offering 47 seven bedrooms located on both the ground and first floor Fifty per cent of bedrooms are equipped with en suite Bathrooms and toilets are located on both the ground and first floor and are nearby to communal areas The dining room and kitchen is situated on the ground floor, having a laundry in the basement Equipment and adaptations are in place to assist people with restricted mobility Staffing is provided throughout the day and night to assist people with their care needs People do not have access to previous inspection reports Access to these reports should enable people to find out about the quality of service provided and peoples experiences of living there The fees charged for the service provided at Woodcote Hall was not made available to us The reader is advised to contact the service direct for this information Care Homes for Older People Page 3 of 8 What we found: The focus of inspections undertaken by the Care Quality Commission is based on outcomes for people who use the service The inspection process considers the homes capacity to meet regulatory requirements, national minimum standards and aspects where the service requires further development This was an unannounced random inspection that took place over 2 5 hours commencing at 05:30 in the morning The reason for this visit was to make inquiries about concerns we have received telling us people are being woken up in the early hours of the morning against their wishes We arrived at Woodcote Hall at 5:15am, we rang the front door bell at 5:30am several times and also the homes telephone We were not given entry to the home until 5:45am Staff informed us they were attending to peoples personal care needs and did not hear the door bell or the telephone Whilst awaiting entry to the home we observed from outside the property that one person was awake, dressed and sitting at the dining table This person told us, They wake me at 06:00am When we were talking to this person it was 5:50am They said, The staff dress me and bring me downstairs I have to put up with it Im not a grouch, dont worry about me The nurse in charge told us it was this persons choice to get up early, this was also confirmed by two other staff members The nurse in charge said one person required assistance with their personal care needs in the early hours of the morning This was also confirmed by a care staff We observed this person was dressed and looked comfortable in their bed but saw the television was on and the volume was loud The light had been left on and this person was asleep These practices do not ensure people will have a restful sleep and awake when they so wish The nurse in charge said she did not realise the staff had left the television on We looked at this persons care records to find out their sleep pattern, in the Personal centred preferences, it stated, X is bed bound This does not reflect the persons choice about when they wish to awake The care plan told us, People are allowed to wake up naturally The nurse in charge told us they commence assisting people who are awake at 5:30am She told us they do not wake people up against their wishes This was also confirmed by two care staff we spoke to On entry to the home at 5:45am the nurse in charge told us six people were awake and dressed We also observed two staff members assisting a person out of their bed Care Homes for Older People Page 4 of 8 Two out of these six people required assistance with mobility and all their personal care needs These people did not have the capacity to discuss the care they receive Three people required minimal assistance and one person was able to meet their own needs One person told us I want to get up early Ive always been like that I get up on my own Staff told us this person always gets up early in the morning This means their choice is respected and supported We saw one person in their room who had been washed and dressed The nurse in charge said she went into this persons room at 05:40am and they were awake, so assisted them with their personal care needs One staff member told us this person requires assistance with their mobility and all personal care needs The nurse in charge answered the door to us at 5:45am One staff member told us it would take about 30 minutes to attend to this persons care needs in the morning This would mean this person was assisted with their personal care needs before 05:40am as stated by the nurse in charge We looked at two care plans and saw information contained were generic and not specific to peoples needs They did not provide clear information relating to peoples preference of times they wish to retire to bed and awake This means people cannot be confident that all staff will know what their preferences are We observed the ceiling within one bedroom had water damage due to a leak from the roof The registered manager told us arrangements are in place to repair the ceiling and decorate this area This should ensure the persons comfort and provide a
