Conditions of registration: Date of last inspection 3/2/05 Brief Description of the Service: Woodlands Cottage is a purpose built bungalow for four service users who have learning and physical disabilities. The home is situated in the community of Choppington and is close to local amenities. Service users are provided with single room accommodation and access to a range of aids and adaptations in line with their personal needs. Communal areas are provided as follows:- a kitchen, dining room and lounge; toilets and specialist bathrooms. The homes furnishings and fittings are of a high standard and the building is well maintained. Attractive garden areas are available. A small car park is provided to the front of the building. This is an overview of what the inspector found during the inspection. This inspection was unannounced and took place over three and half hours. A tour of the premises took place and a sample of care records were inspected along with other records held in the home. All four residents were seen and discussions were held with the manager and one member of staff. What the service does well: What has improved since the last inspection? What they could do better:
The manager should continue to ensure a resident’s care plan is updated to reflect their changing care needs. There have been good arrangements in place to help residents enjoy educational and leisure activities in the community. However, due to recent staff shortages the residents have had fewer opportunities to enjoy activities outside the home in the evenings. This situation should be resolved when three staff vacancies are filled. The manager must carry out risk assessments for the bedroom doors that are sometimes chocked open.

Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7 and 9. The content and presentation of the care plans was good which helps to ensure that the staff team are well informed about the needs of the residents living in the home. The needs of one resident have changed and the assessments and care plans were out of date. Residents’ rights to make decisions about their day to day lives are respected by staff members which ensures that their lives are fulfilling and satisfying. They are supported to take risks and any limitations are recorded. EVIDENCE: During this inspection all the care plans were examined. The manager is currently updating one care plan to address the changing needs of the resident. The plans contained a great deal of information about the personal, social and health care needs of the residents. The members of staff on duty were well informed about the individual needs of the residents. Risk assessments are carried out and ensure that the residents are supported to participate in fulfilling activities. The staff on duty were observed to consult and communicate well with the residents throughout the inspection.

Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14. Links with the community and opportunities to participate in social and personal development activities had reduced at the last inspection. The manager has now taken steps to resolve this problem. EVIDENCE: Discussions with the staff and examination of records showed that residents attended day centres and enjoyed various activities in the community. These included shopping, meals out and visits to local places of interest. One resident enjoyed a holiday in Benidorm early this year and staff are hoping to arrange holidays for the other three residents. A 40th birthday party has also been arranged at the local community centre for one resident. The manager has arranged a meeting with one resident’s care manager to secure funding to access more activities. At present one resident cannot access the home’s vehicle. However a new vehicle will be available in the next few weeks which will be accessible to all residents. The staff agreed this would enable the residents to access evening activities of their choice. A nutritious menu plan is in place and the staff confirmed that alternatives are always available. Any changes to the menu are recorded. There are guidelines in place to ensure residents receive appropriate support at mealtimes. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 and 20. The staff give the residents the personal support they require and according to their preferences. They also seek professional medical advice, and reassessments are requested when necessary. An appropriate system is in place for dealing with medications. EVIDENCE: Staff were seen to provide personal support in such a way as to promote and protect residents’ privacy, dignity and independence. There was evidence that the staff team seek advice and support from relevant professionals in respect of the residents’ health and well being. A random sample of medication records and the system for storage and handling medications was looked at and found to be appropriate. The home has a satisfactory complaints system and staff receive training in adult protection to help protect residents from abuse. EVIDENCE: A complaints procedure was available and this is included within the service user guide. No complaints have been received since the last inspection. The complaints procedure would benefit from being made available in an alternative format to ensure all residents can understand this. All staff have received POVA training. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 26 and 30. The standards of the facilities and décor within the home is good, providing residents with an attractive and homely place to live. Bedrooms are personalised and provide the residents with the necessary facilities. All areas of the home were clean and hygienic. EVIDENCE: An inspection of the premises was carried out and the home was found to be well maintained and comfortable. Each resident has their own bedroom which was personalised and had recently been redecorated. The staff confirmed that the residents had chosen their own colour schemes. New curtains and bedding have also been provided. Since the last inspection bedroom carpets have been cleaned. However the manager agreed that the carpets are showing signs of wear and tear and should be replaced. The hot water supplied to one bath was tested and found to be safe. The staff test the water temperature before using it and it is also routinely tested on a weekly basis and recorded.

Protective clothing is provided. The manager confirmed that all staff receive infection control training as part of their induction programme. JUDGEMENT – we looked at outcomes for standard(s) 33. Minimum staffing levels are met, however, there have been some staff vacancies in the home which has led to residents having fewer opportunities to enjoy evening activities outside of the home. EVIDENCE: There are two members of staff on duty during the day and one sleep-in staff member from 9 pm to 9.30 am. Two enablers from a domiciliary care agency are employed in the home for two hours on three days a week to support one resident to access outdoor activities. There are currently three full time vacancies for home support workers to cover night duties. The manager confirmed that interviews are currently taking place in an attempt to fill these vacant posts. The home will then provide a waking member of staff overnight. The manager also confirmed that she is in the process of securing funding to provide a full time post during the day to assist residents to access further activities. The staff were seen to be kind and caring towards the residents and confirmed they enjoyed working in the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37, 40 and 42. The manager has a clear development plan for the home. Policies and procedures are in place to ensure the residents’ rights are safeguarded. There are systems in place to promote health and safety. EVIDENCE: The manager was appointed to the home in February 2005 and has recently been registered with the CSCI. She has experience is managing homes for adults with learning disabilities and has achieved a National Vocational Qualification, Level 4 in care and management. Since taking over the home she has addressed issues concerning staffing levels and to ensure the residents all have access to fulfilling activities. The staff confirmed that the manager is also available to provide support and guidance. At the last inspection not all the required policies and procedures were in place. The manager confirmed that these are now in place and available to

The management of Northgate and Prudhoe Trust has a programme to ensure these are reviewed and updated on a regular basis. The home was found to be generally well maintained and safe apart from the following matters: Bedroom doors were chocked open which could pose a fire hazard. The manager stated that self closing devices had been requested. In the meantime risk assessments should be carried out for each bedroom. The freezer was in need of defrosting.
