What the care home does well

This is a home for older people where the residents are looked after well. The staff respect the service users and were seen to be following individual care plans encouraging each to maintain their independence and take part in a variety of activities that they enjoy and benefit from. The home provides a pleasant and comfortable place to live. It was evident through discussions with staff that there are lines of accountability within the homes management structure and through discussions and observations it was considered that the management approach created an open and positive atmosphere from which the residents benefit. The home communicates well with families, representatives and visiting professionals, has a group of staff who have worked at the home for a considerable time and training opportunities for staff remain on the agenda.

What has improved since the last inspection?

Attempts continue to be made in order to improve the number and variety of activities. All aspects of the paperwork and recording systems continue to be reviewed and updated and amended where necessary in light of the change of ownership. Staffing levels have been improved and staff reported that the `team work` approach and morale has also improved.

It is located some five miles south of Ludlow, in South Shropshire, on the A49 set back off the main road in its own grounds providing a peaceful location. The accommodation is provided in a large detached house, which has been adapted and extended to provide comfortable and homely accommodation for the people living there. Five bedrooms are situated on the first floor, accessed via a staircase, and eight are on the ground floor. All but one bedroom have en-suite facilities. The home is owned by Fidelity Care Services Ltd, Mrs Jan Gill being the Responsible Individual who works at the home on a full time basis. The Registered Manager is Mrs Barbara Cowell who is a qualified nurse. This inspection reviewed key standards only as the home is currently considered to be performing satisfactorily and thus warrants the application of a reduced methodology. The inspection was unannounced and commenced at 9.30am. It included observing activity within the home, inspecting the premises, looking at records and case tracking and talking to 5 staff, 2 visitors and a District Nurse. The Proprietor, Manager, Senior Carer and the staff on duty were welcoming and helpful throughout the inspection. It was found that the National Minimum Standards assessed had been met with only minor exceptions and that the overall quality of care provided continues to be good. All residents appeared happy, content and well cared for and those who were able expressed satisfaction with their quality of life at the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 3. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The policies and procedures that are in place, and have been professionally followed, ensure that the home undertakes all necessary assessments for successful and satisfactory admissions to take place. EVIDENCE: The home has a Statement of Purpose and a Service Users Guide, which has been updated and includes all the required information for prospective residents. They may visit the home, stay on a temporary respite basis and also for day care before making a decision. Documentation examined indicated that individuals have a comprehensive assessment of their needs, which is reviewed and amended as requirements change Woofferton House Residential Home DS0000065765.V292810.R01.S.doc Version 5.1 Page 9 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health and personal needs of the residents are met with evidence of regular review and of good multi disciplinary working taking place on a regular basis. EVIDENCE: It was evident during the inspection from looking at records, inspecting the facilities and chatting to staff and residents that individual health, personal and social care needs were being met. Individual records of care are maintained and the three seen were well organised. Close liaison has taken place, on the behalf of one service user in recent months, with the District Nurse, GP, The Hospice and Macmillan Nurses, who all have praised the staff at the home for the care the service user had received. Matters pertaining to medication appeared satisfactory at the time of this inspection and all relevant staff have received the necessary training. Residents were being treated with respect and staff were working both professionally and sensitively in meeting individual needs. Those residents and visitors spoken to were complimentary regarding the quality of their lives at Woofferton House Residential Home DS0000065765.V292810.R01.S.doc Version 5.1 Page 10 the home and visiting health professionals reported that they were satisfied with the management and care standards there. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All Quality outcomes in this area are good. The judgement has been made using available evidence including a visit to this service. The routines of daily living at Woofferton House are flexible and each resident finds the lifestyle experienced in the home meets their individual needs. A number of activities take place, there is an open visiting policy and the menu offers a choice of well balanced and wholesome meals. EVIDENCE: The residents are encouraged and enabled to personalise their bedrooms, enjoy good meals in the pleasant dinning room or in their own rooms and have daily activities arranged for them within the home. Individual needs, likes and dislikes are shown in the care plans. Visitors are made welcome, are included in events and are given all the necessary information on aspects of the home and the welfare of the residents. Visitors spoken to previously and at the time of this inspection were complimentary regarding the quality of life for the residents at the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. Quality in this outcome area is good. The judgement has been made using available evidence including a visit to this service. Concerns and complaints are dealt with promptly and professionally and procedures and practices are in place to ensure that individuals are protected from abuse. EVIDENCE: The home has a clear complaints procedure which is given to residents and their relatives before they move into the home. No complaints have been received since the last inspection. The home has all necessary documentation in relation to the protection of vulnerable adults and this subject is included in staff training. The proprietor and senior care are attending additional training on the subject in June 2006. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26. Quality in this outcome area is adequate. The standard of the environment within the home is generally satisfactory, providing residents with a warm, safe and homely place to live however it was identified and fully acknowledged that a number of improvements need to be made without undue delay. EVIDENCE: The location and layout of the home are suitable for elderly residents. Communal rooms are well equipped and are warm, homely and welcoming. Bedrooms are personalised and suit individual needs and the gardens and grounds are generally well maintained and accessible to residents and their visitors. At the time of the most recent Fire Officer and Environmental Health Officer’s inspections matters were reported to be satisfactory. It was identified that some areas of the home will need refurbishment, repair and redecoration in the near future and it was agreed that a full programme and plan for routine maintenance and renewal of the fabric and redecoration of Woofferton House Residential Home DS0000065765.V292810.R01.S.doc Version 5.1 Page 14 the premises will be forwarded to CSCI within the month and that this will be implemented within timescales agreed. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are generally supported by a well trained and committed staff group who are meeting the needs of each individual in a sensitive and professional manner. EVIDENCE: Three members of staff has been recruited since the last inspection and all elements required by Schedule 2 of the Care Home Regulations and evidence of induction training were maintained. The management continue to support staff to undertake their NVQ awards, a variety of other training has been undertaken and staff on duty indicated that they were sensitive to the service users needs and disabilities. Staff seen and the duty rota indicated that there were generally sufficient staff in numbers and skill mix to meet the service users needs. A senior care, three carers, one catering staff and cleaner were on duty at the time of this inspection. All care staff are over the age of 18. It was evident that on occasions in the afternoons at the weekends there are only two staff on duty to care, cook, clean, and undertake laundry tasks, however the proprietor reported that she or the Senior Carer were available at these times. Staffing has been increased in the mornings at weekends. Given the dependency levels of the service users and size and layout of the building, it was fully acknowledged that this situation should continue to be Woofferton House Residential Home DS0000065765.V292810.R01.S.doc Version 5.1 Page 16 reviewed to ensure that the critical care role is not impeded and that the service users needs are met. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. There are lines of accountability within the homes management structure and the management approach creates an open and positive atmosphere from which the residents benefit. The home regularly reviews all aspects of its performance through a programme of self review and consultations and meets the requirements of the Fire Officer and Environmental Health Officer, promoting the health, safety and welfare of the residents. EVIDENCE: Mrs Cowell the manager is qualified and experienced. The Senior Carer commenced the Registered Managers Award and NVQ4 in care in September 2005 and the Proprietor has now commenced NVQ 4 in care. An open, positive and inclusive atmosphere continues to be maintained at the home. The Proprietor involves herself fully in the day-to-day running of the home and works alongside her team offering support to all staff. Quality assurance systems are in place and there was evidence available to indicate the Proprietor ensures, so far as is reasonably practical, the health, safety and welfare of service users and staff. All staff have received necessary Health and Safety training and updates have now been booked. It was reported that a first aider is on site at all times and the accident records were satisfactory.
