This home may accommodate 59 older people who require convelescent nursing care. 28th February 2006 Date of last inspection Brief Description of the Service: The building is a former vicarage with a large modern extension. The provider, Wymondley Nursing and Residential Care Home Limited, was first registered by Hertfordshire County Council under the Registered Homes Act 1984 in March 1990 prior to the current registration under the Care Standards Act 2000. The home provides nursing care and accommodation for older people, with or without physical disabilities and for older people with terminal illnesses. It is also registered to provide personal care for 3 older persons with dementia. The home is situated in the village of Wymondley, close to the towns of Hitchin, Letchworth and Stevenage, a short distance from the village centre with facilities such as the church and pub. The home has links with organisations in the village. Access to public transport is limited. Accommodation is provided on three floors served by 2 passenger lifts. All bedrooms are single rooms, 22 with en-suite toilets. There are several day rooms. The building stands in extensive, attractive grounds, greatly enjoyed by many service users. There is ample car parking space in front of the home. This inspection took place between over five hours during the day and was carried out by two inspectors. Questionnaires for service users had previously been sent to the home and eleven had been completed and returned to the CSCI. During a tour of the house the inspectors spoke with service users, relatives, staff members and the management team, and although the manager was not on duty, she was represented by a senior colleague throughout. Fees in the home range from £558 p.w. - £618 p.w. What the service does well: What has improved since the last inspection? What they could do better: The home needs to be able to show that it consults all service users and other stakeholders about the quality of care provided and that the findings are incorporated into the home’s planning. While medication is generally managed well, recording practice and storage need to be attended to. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. Service users’ care plans were looked at during the inspection and those seen showed that the home carries out an assessment of each person’s needs before they move into Wymondley Nursing Home. A Service Users’ Guide is given on admission to the home. EVIDENCE: Prospective service users are given full information to enable them to make an informed decision about moving into the home. Service users can be confident that the home can meet their assessed needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users care plans set out their individual and changing needs, ensuring that appropriate care is given. Medication is generally managed effectively and safely although the practice of recording administration before medication is given to service users could lead to unclear and inaccurate records EVIDENCE: Care plans seen at the inspection showed that service users health and social care needs are set out in their individual plans and that these are regularly reviewed and amended to reflect changing circumstances. Risk assessments are included and set out how identified risks are to be managed. Service users are helped by the staff group to maintain their independence as far as possible – an approach which was acknowledged by those service users who spoke with the inspectors. The inspectors saw examples of good care practice during the inspection – service users were helped discreetly and with regard to their dignity. Medication administration and storage were seen and were generally satisfactory, although the Medication Administration Record (MAR) chart was Wymondley Nursing Home DS0000019630.V302835.R01.S.doc Version 5.2 Page 10 seen to be completed before the medication had been given to one service user. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users dietary and nutritional needs are met by the catering staff; they are encouraged to maintain their independence and to keep their community links, enabling them to maintain the lifestyle of their choice. EVIDENCE: Service users were enthusiastic about the catering in the home – menus are varied and offer a balanced and nutritious diet and an alternative choice at both lunch and teatime is always offered. The lunch served during the inspection was attractively presented and generous in quantity; those people who needed help to eat were assisted discreetly and sensitively by care staff. Inspectors made some suggestions about how soft or liquidised food could be served to those people who need it. The chef meets regularly with service users to discuss menus and receive suggestions which are taken up where practical. Service users at Wymondley are able to have visitors at any time – several were visiting during the inspection and spoke with the inspectors. People are encouraged to maintain their links with their community, where possible, and there is a weekly communion service offered by a local church. There had been a comment on a questionnaire by a service user’s relative that activities in the home were not always given priority and that they relied on there being sufficient staff on duty. Activities include quizzes, sing-alongs, barbecues and festival celebrations throughout the year. The home has two part-time activities organisers who maintain records of who participates and what they enjoy. Care staff spend time with service users, often on a one-to-one basis, and there are table games, books and newspapers available. There are also regular visiting entertainers who are always popular with service users. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are protected from abuse and can be confident that their concerns and complaints will be listened to and dealt with appropriately. EVIDENCE: There is a comprehensive complaints procedure in place which is on display in the home and is included in the Service Users’ Guide. There have been no complaints recorded since the last inspection and service users confirmed that they would feel able to voice their concerns to the proprietors or the manager. There had been an exchange of correspondence with one service user’s family as the home felt that his needs were not being met at Wymondley. The matter had been dealt with appropriately and was resolved for the service user, who has since moved to another home. There is a whistle blowing procedure in place and the Hertfordshire Adult Protection procedure is available in the home. All staff are given adult protection and whistle blowing training. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users live in a clean, safe, well-maintained home. EVIDENCE: The home has a welcoming and homely atmosphere and is light and bright in the communal areas, particularly the small glassed-in seating areas which are well-used by service users. Decorations are maintained to a high standard throughout and the housekeeping staff ensure that all areas of the home are clean and that there are no unpleasant odours. Furniture and furnishings are in good condition. The laundry in the home is well equipped, clean and in good order. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users can be confident that staff have been safely recruited and that they are given relevant training. There are sufficient staff on duty to meet individual service users’ needs. EVIDENCE: Service users and relatives spoken with at the inspection confirmed that there are plenty of staff on duty and that they are responsive to their needs. This was reinforced by the responses on the written questionnaires that were returned to the CSCI. The recruitment process for several of the latest staff to join the home was looked at and while all the necessary documents were in place and security checks had been made, two files showed references obtained from friends of the applicants, despite another employer being listed on the application forms. The need to seek appropriate references was discussed with the management team. Training records were seen and showed that there is an induction programme for new staff and ongoing training takes place – both in-house and external – for all staff including housekeepers. The home has an ongoing programme for care staff to take the National Vocational Qualification (NVQ) in Care at both Levels 2 and 3. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users and other interested people are asked for their views on how the home is run and the outcomes of surveys inform the planning process Service users’ welfare is generally protected through the home’s health and safety procedures, although there is a need for staff to be vigilant to areas of risk in the home to ensure the continued wellbeing of both staff and service users. EVIDENCE: Staff spoken with reported that they receive regular supervision and this was confirmed by records seen; they confirmed feel supported and valued by the management team. While the catering team have regular recorded meetings with service users to discuss menus, there is nothing to show that service users’ views about other Wymondley Nursing Home DS0000019630.V302835.R01.S.doc Version 5.2 Page 17 areas of the home and how it is run have been sought. The proprietors are regularly in the home and they and the manager talk to service users on an informal basis. Service users are consulted by questionnaire about the quality of care provided and an analysis of the information drawn from the surveys is completed. A concern about the siting of hoists for recharging when not in use was raised by the inspectors – one was placed directly outside the door of a service user’s room and could present a hazard to someone leaving the room, four others were lined up on the ground floor. There is a need for risk assessments to be completed for these sitings and for the manager to be satisfied that risk to service users, staff and visitors is minimised. A minor health and safety infringement observed by an inspector was acknowledged and dealt with responsibly by a staff member, showing that there is an awareness of health and safety issues in the home.
