If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service 14 Churchfields is a modern detached, four bedroomed family house, located in a residential area on the outskirts of Hethersett. The home is registered to provide accommodation for three people with a learning disability who wish to live in a family type setting. There are local community and social facilities nearby that are easily accessible. The current cost per week is £367.00. 1 7 0 2 2 0 0 9 Annual Service Review Page 2 of 6 Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The AQAA was returned to us in good time, and contained comprehensive information about the facilities and service provided to those living in the home. It provided information about the home, and details of staffing including training, changes that have been made in the home, and planned changes for the future. It also demonstrated how the service takes the views of the people living in the home into account in the decisions made, and involves them in everyday family life. They ensure that they listen to what the residents are telling them, and adapt as necessary, ensuring that any changing needs are identified and met. The AQAA indicates that the service has an admissions procedure in place, but has not had any changes in those living in the home, and does not envisage any changes in the coming year. Individual care plans are in place for everyone living in the home are centred on the person. The include details about the persons social background as well as likes and dislikes. The service continues to offer long term residential care to three people, funded by Norfolk County Council. People who live in the home have access to a range of activities on a daily basis, both within the home and in the local community and we were told in the AQAA about the activities they arrange, both work and leisure related and including annual foreign holidays, local trips to the pub and shopping and other more structured day services. The AQAA showed that there have been no concerns complaints or safeguarding issues had in the past year, and confirmed that procedures remain in place should any issues arise. The service is provided by the owner and family, and no staff are employed, procedures are in place and appropriate checks would be made should this situation change. Annual Service Review Page 4 of 6 The home continues to let us know about things that have happened since our last key inspection and they have shown that they have managed issues well. They work well with us and have shown us that their service continues to provide good outcomes for the people who use it. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 28th September 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
