5 Trinity Street provides long term care to 3 adults with a learning disability who are of mixed gender. The house is sited near the centre of Old Hill in an established residential area that affords easy access by foot to a range of local facilities, including32009 public transport. Staff also have access to a car for transporting the residents between venues. The house is a terraced property that has been adapted for its current use and consists of two living rooms, a kitchen/diner and three single bedrooms as well as bathroom and toilets. The main stated aim of the home is to provide a service that reflects the expectations of the residents: identifying and fulfilling their individual needs by means that are valued by society; this in order to develop and support individual and personal experiences and characteristics which are culturally valued and maintained. There is a staff group that consists of a manager, senior support staff and support workers. There are waking staff available 24 hours per day. The manager is responsible to a service manager and directors of Inshore support who have a number of homes of similar size and purpose. What the care home does well Records kept by the service shows us it works well with health care professionals to ensure people`s health care needs are appropriately met and reviewed regularly. People are supported by staff trained in managing their medication and supporting people who sometimes display challenging behaviour. Comprehensive records are kept of situations were a person has displayed such behaviours, the actions taken by staff and whether or not "as and when required" medication has been taken. What the care home could do better: The manager should ensure written protocols are produced in a timelier manner whenever a person is prescribed "as and when required" medication. This should be kept on the person`s file for staff to refer when needed. This will ensure a consistent approach is taken by the staff team. The manager should carry out periodic assessments on staff who are responsible for managing medication to ensure they continue to be competent in this area, particularly for medication prescribed "as and when required". Information about staff who previously worked in another service within the company should be transferred with them and reviewed by the manager. This is to ensure any gaps in training are identified and, where necessary, a training programme is produced. This will ensure each member of staff is suitably trained to meet the needs of people living in the home. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: We looked at the health and medication records for two people living in the home. These tell us people are supported to access health care services. The information kept on their individual files are detailed and include daily monitoring records of health issues and, where changes have been, of their medication. The medication adminisration record (MAR) sheets we sampled were clearly recorded and, as required by the services procedures. The records show both people are prescribed as and when required medication. One persons file contains a protocol for administering this medication. Staff we spoke to demonstrated they understood this information and were clear about how to respond to the persons behaviour in accordance with her/his care plan. We were informed the second person had only recently been prescribed as and when required medication. A written protocol for administering this was not available. The senior member of staff on duty told us this was being produced and staff had been made aware of the new prescription and the reasons for this in the communication book and at the staff meeting held on 26th October 2009. We saw staff following the services procedures for administering medication when we arrived. A member of staff spoke in detail about procedures and practice showing us they had a good understanding of medication issues. The manager informed us prior to this visit that she would be assessing the practice of individual staff members responsible for managing medication. We looked at a senior member of staffs file and found records of practice assessments carried out in 2006 and 2007, but no evidence of any recent assessments. The training matrix shows us the support workers on duty have attended training in Safe Handling of Medication in July this year, one had attended additional specialised medication training and the other was booked to attend later this month. Both have attended training in managing challenging behaviour. The records also showed us staff have also recevied training in safeguarding vulnerable adults and first aid. These were recommendations made in the key inspection report for March 2009. There were no training records for the senior member of staff who was on duty. They told us they had recently returned to work at the home and were familiar with the needs of the people who live here. They said they had attended all relevant medication training and confidently explained the different procedures. We spoke to the manager following this inspection and she confirmed this information was correct.
