Agnes Court is a care home with nursing providing care/support and accommodation for 24 people who have a physical disability. Agnes Court is a purpose built home designed to meet the needs of people with a variety of physical support needs. The home is situated in central Banbury with easy access to the shops, pubs, the cinema, a local park and numerous community facilities. Annual Service Review Page 3 of 8 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment AQAA that was sent to us by the service. The AQAA is a self assessment that focuses on how well outcomes are being met for people using the service and numerical information about the service. Surveys were sent to the residents and to others with an interest in the service. Ten surveys, written by staff on behalf of the resident, two from residents relatives and friends, seven from staff members and five from health care professionals were received by the commission and have been included within this service review. Additional information is also about how the service has maintained standards, made improvements and plans for the future. Evidence has also been received about how the home manages any complaints and safeguarding concerns and what the service has told us about things that have happened in the home, these are called notifications and are a legal requirement. We looked at the previous key inspection details, the results of any other visits that we have made to the service in the last 12 months and any relevant information received from other organisations. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) and it was clear and gave us all the information we asked for. The AQAA advises that an individuals care and support needs are assessed prior to the individuals admission to the home. The assessment forms the residents individual support plan detailing their current needs and activities of daily living. The individual and their representatives and key worker are part of the assessment process. The prospective resident is invited to visit the home and make acquaintance with other residents and get to know the home and the staff and to help the individual settle in their new home if they choose to move in. Health care services remain available to all residents and include visits from the residents general practitioner, occupational therapists, physiotherapists, dietitian support, speech and language therapists and district nurses. Visits are either made at the home or to the health care professional to ensure the health and welfare of residents with specific needs are met. The AQAA advises that each individual has a health action plan, up to date risk assessments, review notes and other health information which the individual has access to and has the opportunity to discuss their plans with their key worker. Written comments from health care professionals associated with the home included Annual Service Review Page 4 of 8 The home provide excellent person centred care to the individuals enabling them to access the community and be as independent as possible. My impression of Agnes Court is that it is a well run home that promotes the independence of service users and ensures that their rights to make decisions for themselves is observed. The staff are knowledgeable and keen to progress and learn about topics that can enhance the care they deliver. I feel the organisations individual service plan paperwork is weighty and confusing yet this is currently being updated. This is truly a wonderful service. The home respect the residents autonomy I have no concerns about this organisation. The home have identified that they have improved by supporting residents to make informed decisions, be more involved in activities of daily living such as cooking and washing and have organised a nurse trained to provide nail care in order to promote residents welfare and wellbeing. The AQAA advises that the home has a medication policy and procedure and staff work with individual residents to promote their awareness of their medication and their side effects. The promotion of residents to self medicate is considered within a risk assessment framework to ensure the safety and well being of the resident. Written comments which had been written by staff on behalf of the residents included the staff are too busy to talk to you and I would like staff to have more time. the staff get you well and I like the music. the staff arrange trips into town, the home should make sure there is hot water late at night. I get well looked after and get good care , it would be good to have activities at weekends but I think its a brilliant home. Its homely and the food is good. The staff dont get enough pay for what they do. The lunches are good the staff could talk to me more. The AQAA advises that residents preferences regarding their leisure interests, hobbies and meaningful activities have been explored more fully since the previous inspection and improvements for the residents include supporting individuals to be more confident within their local community, improved access for residents to use the homes computer room, increased the range of activities within the home, organised a group of residents to raise funds for another charity and raising awareness within the local community regarding disability issues and building up a group of volunteers to assist with IT support for residents. Other activities provided by the homes staff include offering support from the homes volunteers to enable residents to take trips away from the home, offer group activities for example cookery, history, poetry and computing. Volunteers support residents on a one-to-one basis with activities for example language skills, jewelry making and also time with a PAT dog. The AQAA advises that the homes staff are aware of and give each resident as much support as possible to aid communication and are supporting one resident with an eye recognition system which will hopefully open up avenues of communication for the resident. The AQAA advises that residents are supported to assist with the planning of the homes menus and staff encourage and promote residents to have healthy food choices. Choices of meals are available and individuals dietary requirements are respected and catered for. Family and friends are actively encouraged to be part of their loved ones life. Annual Service Review Page 5 of 8 Comments received from relatives and friends about the care and support offered at the home included the homes staff do everything well. As far as I am concerned the home has met and indeed exceeded every expectation we had for our relative when they were taken into the care of Agnes Court. The home has a formal complaints procedure. No complaints have been received by the home within the last 12 months and no safeguarding vulnerable adult referrals have been made. The AQAA advises that staff receive regular safeguarding vulnerable adults training and residents are given opportunities in their meetings to raise any concerns they may have about the home or the care they receive. The home confirm they have a whistle blowing policy and procedure and staff are aware of their duties to inform and report any concern they may have regarding the protection of individuals from harm or abuse. The home continues to meet the needs of residents. The AQAA advises that improvements to the homes environment during the last twelve months include the increase in numbers of residents supported at the home. The redevelopment of the home has enabled each resident to have a large bedroom with en suite facilities which incorporates assistive technology for opening doors and windows and pulling curtains. The home has been divided into units which has given the home a family like atmosphere with the kitchen and dining areas providing more intimate mealtimes. A bathroom and lounge that were previously not greatly used have been redeveloped into a new bedroom with en suite facilities, improved structures outside to enable residents to join in gardening activities and Dorguards have been sited to hold doors open to give greater free flow of air within the home. The home remain mindful of infection control issues and staff have received appropriate training in promoting residents health and welfare. The AQAA advises that the home want to increase the cleaning hours at weekends to ensure the home maintains an appropriate level of cleanliness at all times. Recruitment and selection of staff ensures that all appropriate checks are made to make sure that residents are protected and safeguarded from any harm or abuse. New staff receive a structured induction training and the home have a planned mandatory training programme to make sure that staff are competent in supporting residents in their care. The AQAA advises that the home have established new four team leader posts which offer stability and continuity of care for the residents. Seventeen of the permanent care staff out of thirty have achieved their National Vocational Qualification Level 2 or above. Staff written comments about the home included Our home meets the needs of many people with all kinds of disabilities. Agnes Court is a new nursing home with the latest technology adapted to disabled residents. It is equipped with physiotherapy and IT room which residents love. It has an activities room which also makes residents productive. I am proud of belonging to this organisation and the staff are given loads of training and courses to make us work effectively. My manager is approachable and shes always got the time to discuss issues and problems. Very supportive. The home provides lots of activities for the service users. We meet everyones individual needs, it is a happy home and the manager you can call anytime to talk about anything. The home is very holistic in care and looks at the whole person. The residents have a varied social life and lots of activities within the home. The home have an extensive Annual Service Review Page 6 of 8 training programme for all levels of staff. Service users are involved in the running of the home and the recruitment of staff, there is good volunteer support and service users needs are listened to. The home could do better by providing domestic and catering staff at weekends instead of care staff. The home could also do better by providing their own trainer. We work as a team involving staff and service users. I enjoy working at Agnes Court and we work as a team, meetings on a more regular basis and improved nurses and staff communication could make the home better. The management of the home is robust and has remained consistent. The registered manager has achieved their Registered Managers Award Level 4 and continues to let the commission know about things that have happened since the last key inspection and she has shown that the home manages issues well. The home continue to work well with the commission and have shown us that their service continues to provide excellent outcomes for the residents. What are we going to do as a result of this annual service review? We are not going to change our inspection plan and will do a key inspection by 28th February 2011 however we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
