6As the name suggests the house was previously a vicarage. It is a big old building in its own grounds with an extensive grassed area and mature trees. The home has recently been extended to the back of the building but a large garden area still remains. The building is next door to the church it used to belong to. On the other side is a children`s play area. Church Road is a quiet street that runs parallel to the main road through Hetton. The interior of the house retains many of its original characteristics, such as wooden shutters on the windows and care has been taken during the extension work to maintain such features. The decor is bright and the furniture uncluttered, allowing freedom of movement. All of the service users have single bedrooms that are individually decorated. The extensive grounds are kept tidy by a gardener and the general repairs are addressed by a handyperson. The home is currently registered to provide care for six service users under 65 years all of whom have a learning disability and who have autism. It is the aim of the home to develop activities, including daily life skills that are designed to reduce anxiety while creating personal security for the individual. The service has developed a Service User Guide that is in picture format. This informs service users and other interested parties about the service. What the care home does well This service provides accommodation of a good standard. It is well maintained and as a result of effective cleaning routines provides service users with a clean and hygienic place in which to live. So that service users` healthcare needs are supported well and in an appropriate way, staff work closely with healthcare professionals involved in the person`s care. This means that staff can seek advice from people who know the person well and learn from their expertise and experience. Service users are supported to live a full and active lifestyle and as a result they take part in a variety of different activities both in their own activity centre and the local community.Some of the staff have worked at the home for many years and as a result, positive relationships have developed with the service users. The staff follow detailed care plans which means that service users` complex needs are met in a consistent way. Staff work with respect and skill and aim to involve service users in making choices about what they prefer to do. What the care home could do better: No requirements or recommendations were made. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: The aim of this inspection was to establish that; the standards assessed at the previous key inspection carried out 03/04/09 have been maintained; that the requirement and recommendations made at the previous inspection have been addressed and service users continue to be safely suppported; and that care practice issues brought to CQCs attention via an anonymous caller and that the home addressed following the local authoritys safeguarding procedures, have been approriately addressed. Service users continue to live a safe, valued lifestyle within a framework that continues to be well managed and that is guided in service users best interests. It was evident that the manager, staff and service users continue to work hard to maintain high standards throughout this service, resulting in service users living a life that is safe, valued and interesting. A detailed investigation, into allegations made earlier this year against practices in the home, was carried out by a senior member of the company resulting in none of the issues raised being upheld. However the home has taken a positive approach to the investigation and as a result has reviewed practices in the home and where they thought were needed positive improvements have been made. So that night shift workers can be supervised on the job, the manager now makes unannounced nightly visits, to carry out individual supervision sessions with the night shift team and so that a consistent approach is established by dayshift workers, detailed handovers have been established to be shared at the beginning of the day shift between the nightshift and the dayshift teams. Care plans clearly describe the nutritional needs of all service users and when needed advice is sought from healthcare professionals. There are guidelines in place for one service user who needs their food liquidised and these are clearly recorded so that staff know why this is needed and the process to use. One service user who experienced a fall out of bed was assessed by an occupational therapist who recommended foam wedges to be used at either sides of the bed to restrict falls and in addition to this a falls safety mattress to be used on the floor. The result of this has been a success and a safe solution. The service user guide has been reviewed and now includes a copy of the homes terms and conditions in respect of accommodation to be provided for the service users and the method of payment of fees. Care Homes for Adults (18-65 years) Page 4 of 9 Risk assessments regarding individual medical conditions have been developed and are an integral part of the care plans. This means that a medical condition does not impede on the lifestyle chosen by indivdual service users and that risk management plans are in place, so that staff are guided to safely support the individual service user. An improved system for recording staff supervision sessions is now in place and the staff training programme has been further developed. All staff now work through the Learning Disability Qualification (LDQ) and attend an Equality and Diversity training session and their individual training needs are monitored by the training manager. Care plans have been reviewed and a new approach is practiced. All care plans are written with a person centred approach and the individual service users are empowered to lead them. The manager explained that staff have commented that when reading care plans it is now easier to see service users as the individual with responsibilities and rights. There is an ongoing maintenance programme going on in the home that ensures that the decor and furnishings are kept up to a good standard. Due to the needs of the people living at this home repairs and redecoration is an ongoing task. As a result of one persons changing needs and an assessment carried out by an occupational therapist, the downstairs bathroom has been converted into a shower room. This is to promote the individuals independence, while at the same time to enhance the homes resources. Although the work is now complete there are still some minor cleaning up jobs to do regarding cleaning the new tiles. All of the above developments/improvements made to the service have been discussed with the service users families, who the manager confirmed have been very supportive. It is practice at All Saints Vicarage that families are encouraged to make contact with the home prior to their visits, so that it can be established that the service user is at home when they arrive and so that what for some might be a long drive, the journey has not been wasted.
