Bealeys Lane is a care home which is registered to provide care to a maximum of five people with severe learning disabilities. The home is located near to the centre of Bloxwich, opposite a park and close to shops, pubs and other amenities. The home was opened in 1995, originally being a residential dwelling and domestic in nature, that has been converted for its present use. All the homes bedrooms are single without en-suite facilities. The home has parking facilities to the front of the building and a private garden to the rear with a patio area. What the care home does well The home has person centred plans in place for all of the people who live there. What the care home could do better: The home needs to make sure that it recognises what is a potential deprivation of liberty and takes action to address this. The home needs to contact the district nursing service for advice about the management of pressure area care and supply of equipment. There must be regular checks of the hot water systems and where temperatures are not within recommended levels action is taken promptly to resolve this. This will make sure that people living in the home have a consistent supply of hot water and a reduced risk from legionella bacteria. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: We visited the home because we had been told that one person was being locked in their bedroom during bedrest without their consent. We were concerned that this person was being deprived of their liberty. When we arrived we asked the person in charge to show us to the bedroom of the person in question. The door was closed but not locked, the person was in bed sleeping. We were told that the door had been locked when this person had bed rest in the afternoon, this was to protect both the person in bed and the other people using the service who would enter the bedroom and attempt to eat toiletries. Information about locking the persons door was not included in their care plan or risk assessment. Staff recognised that this should not have been done and understood the implications of doing this. The home manager told us that this situation has been discussed with a social worker and the home will be making an application in line with the Deprivation of Liberty Safeguards. The home has also responded by making sure that the person who needs bedrest now has one to one care during this period of time. A staff member will sit outside of the bedroom in order to prevent other people entering and disturbing the person. This information will need to be included into the persons support plan. The home has also arranged for additional training in the principles of the Mental Capacity Act 2005 and the Deprivation of Liberty Safeguards. Once this training has taken place all staff will be trained and should have an understanding of what could possibly constitute a deprivation of a persons liberty. We were also concerned about the locking of the door as this created an additional risk in the event of fire breaking out. We looked at the personal evacuation plan for the person in question. The plan was very detailed and when we spoke to staff they were able to tell us what action they would take in the event of fire breaking out. We have made some good practice recommendations as a result of this inspection. We have noted that one person was judged to be at high risk of developing pressure sores. The home is making sure the person has regular bedrest and at present there is no pressure damage. The home cannot demonstrate how they have reached the decision about the person being at high risk of developing pressure sores. There is no risk assessment for this. We have recommended the home contacts the district nursing service who will be able to assist with this and also in the provision of equipment for pressure relief. We have noted the home has been without a vehicle since December 2009. This has meant a reduction in the social activity for people. We were told that people living in the home have been supported in using public transport but this has not been ideal and their social lives have been affected by this. The organisation has now arranged for a new vehicle but only after staff had sourced the quotes for this. The organisation should have been more proactive in managing this situation so that people living in the home did not have their routines disrupted. When we completed our inspection in June 2009 we made one requirement about the Care Homes for Adults (18-65 years) Page 4 of 9 storage of medication in the home. We have seen that a new medication storage cupboard has been purchased and installed. We have removed this requirement. We also made comments about the redecoration and refurbishment of the home. We are concerned about the length of time it takes the organisation to attend to maintenance repairs in the home. We have identified during this visit there is an inconsistent supply of hot water to one of the bedrooms. This has been on ongoing problem for three months and has not been addressed. We also note that three rooms in the home have the water supply turned off for safety reasons. When this happens we expect the home to flush through the system on a weekly basis to make sure the risk of legionella bacteria growing. This has not been done. There are only three recorded entries since January 2010 to show that checks have been made. We have sent a letter to the home asking them to take action to address this.
